Distal radius malunion (DRMU) consists in a non-anatomical consolidation of a distal radius fracture. The resulting alteration of the articular or extra-articular radial anatomy impairs wrist function to a greater or lesser degree: Restricted ranges of motion, loss of strength, pain. There may also be nerve or tendon involvement. Adaptive carpal malalignment and ulnar-carpal impingement are also possible. Imaging assessment should at least include X-ray and CT; CT-arthrography is essential in intra-articular DRMU, which regularly progresses toward radiocarpal osteoarthritis. Surgical indicatio

Proximal tibia shaft fractures are often challenging to manage because of their intrinsic tendency toward valgus and apex anterior deformity. In recent years, intramedullary nailing (IMN) has become more frequently used to treat these injuries, allowing for biologic advantages such as load-sharing, immediate weight-bearing, and avoidance of disruption of periosteal blood supply. Several adjunctive techniques, such as semiextended positioning, blocking screws, and external fixation, have been developed to assist with fracture reduction during IMN. We describe a new adjunctive reduction techniqu

Intraarticular calcaneal fracture treatments that result in malalignment often require reconstructive surgery. Seven cases are used to demonstrate the intricacies of reconstructive case management. Reestablishment of calcaneal height, length, orientation, and position relative to the other tarsals is necessary to reestablish appropriate foot function. Inherent or acquired gastrocnemius equinus should be treated with recession to reduce destructive forces on the reconstruction.

BACKGROUND: A number of studies report on limitations of the screw arthrodesis in severe malalignment of the hindfoot, neuropathic deformity, poor bone quality and osteoporosis.
METHODS: Fourteen anatomically correct polyurethane foam models of the right leg (Sawbones Europe, Malmö, Sweden) and eighteen fresh-frozen human lower leg specimens (9 pairs) were used for the comparative biomechanical testing.
RESULTS: The statistical analysis of the stiffness of the fixation developed a significant difference in favor of the plate in all test directions.
CONCLUSIONS: The excellent biomechanical r

Corrective osteotomy is indicated in young manual workers with significant malalignment after fracture of the distal radius without associated degenerative changes in the wrist joint. Chronic pain, limited motion and impaired power can be avoided and the cosmetic appearance of the hand improved, by restoration of the functional anatomy of the wrist. The operation consists of an opening-wedge osteotomy at the fracture site, sometimes combined with resection of the distal ulna. The approach may be dorsal or volar. A small iliac cortico-cancellous bone graft is inserted at the osteotomy site. Fix

Tibial plateau fractures can result from direct trauma or indirect compressive forces. These injuries often result in significant soft tissue disruption, ligamentous disturbance in addition to bone fractures. Diagnostic imaging findings in plain radiographs include fat-fluid level in the suprapatellar bursa, malalignment of the femoral condyles and tibial edges, and increased trabecular density in the lateral epicondyle. Patients in whom a tibial plateau fracture is suspected with negative radiographs should have computerized tomography imaging performed. Most patients will require surgical in

Complications of joint fusions can include those applicable to any surgery such as bone and/or soft-tissue infection, wound dehiscence, and failure of fixation. Other complications are specific to fusion procedures and would include malalignment, proximal or distal joint deterioration, and delayed union or nonunion. The latter negative outcomes can occur from medical comorbidities, patient noncompliance, or inappropriate fixation. This article addresses many of these complications including techniques for evaluation, accurate diagnosis, and current treatment options. Special attention is paid

AIM: Post-traumatic humerus varus can develop after an insufficient reduction technique. Reduced abduction, anteversion or rotation is seen, often associated with painful impingement. The radiographic sign is a diminished shaft-head angle less than 140 degrees with a consecutive elevation of the greater tubercule above the head rim and a subacromial narrowing. We analysed the data of the Constant score pre- and postcorrectionem.
METHODS: Between 1992 and 1999 we treated 8 patients with post-traumatic humerus varus. The follow-up time lasted for 3 years after the correction procedure. By means

Post-traumatic malalignment can occur on one or several planes following fractures in close proximity to the knee joint. In a clinical and radiological analysis of deformities, the frontal, sagittal and longitudinal alignment, as well as differences in femoral or tibial length and torsion, must be taken into account. The location of the correctional osteotomy is usually defined by the center of the angular and torsional deformity. Of decisive importance when choosing the site of the osteotomy are the local quality of bone and the soft tissue envelope, the condition of the articular cartilage i

Angular limb deformities are developmental bone-shape anomalies that typically occur due to abnormal physeal growth before skeletal maturity and result in musculoskeletal malalignment leading to abnormal limb appearance and mechanical dysfunction. The conformational and mechanical changes resulting from a bone deformity often require surgical correction. Surgical correction of a bone deformity after skeletal maturity is typically an invasive, open procedure requiring a corrective osteotomy followed by stabilization of the bone using implants. Early intervention in the skeletally immature anima

Herein, we report a case of focal periphyseal edema in a 12-year-old female tennis player. The onset of chronic knee pain in adolescents has differential diagnoses, including fatigue fracture, epiphysis (Osgood-Schlatter disease), growth plate injury, Brodie abscess, patella-femoral malalignment, neoplasia, and meniscus tears. In this case, magnetic resonance imaging revealed focal bone marrow edema around the center of the epiphyseal plate of the distal femur. Three months later, the patient experienced residual knee pain, with no improvement visible on re-imaging. However, symptoms of the pa

Despite advances in staged protocols and fixation techniques, treatment of pilon fractures remains a significant challenge in orthopedic trauma, with up to 21% of patients requiring revision surgery. Management of a pilon fracture that has failed initial treatment involves navigating a myriad of complicating variables, including infection, bone loss, malalignment, and nonunion. Although no single surgical approach can be aptly applied to the broad range of pathology and severity spanned by these patients, this narrative review provides a systematic framework for developing a revision pilon rec

The subchondral bone is involved in a variety of diseases affecting both the articular cartilage and bone. Osteochondral defects in distinct locations and of variable sizes are the final results of different etiologies. These include traumatic osteochondral defects, osteochondritis dissecans, osteonecrosis, and osteoarthritis. Traumatic osteochondral defects are caused by osteochondral fractures, separating an osteochondral fragment that includes articular cartilage and both subchondral and trabecular bone from the joint surface. In osteochondritis dissecans, the disease originates in the subc

We present a rational, scientific, low-risk approach to patellofemoral pain (anterior knee pain) based on an understanding of tissue homeostasis. Loss of tissue homeostasis from overload and/or injury produces pain. Bone overload and synovial inflammation are common sources of such pain. Chondromalacia and malalignment are findings that almost always do not need to be "corrected" to relieve pain. Patience and persistence in nonoperative care results in consistent success. Surgery should be rare and done only after extensive nonoperative management and in the setting of clearly defined patholog

Fracture malalignment and nonunion are not infrequent after treating subtrochanteric fractures with intramedullary nails. The use of a cerclage wire with a minimally invasive approach to aid and maintain reduction in certain subtrochanteric fracture patterns can be an effective surgical strategy to improve outcome. It allows the surgeon to obtain and maintain an anatomic reduction with more bone contact, which will aid in fracture consolidation. This has the added advantage of optimizing the greater trochanteric starting point. It minimizes malreductions of the proximal femoral fragment, and,

Tibial fractures are the most common long-bone fractures. Orthopaedic surgeons, regardless of their subspecialty, often must treat these injuries, which range from low-energy, minimally displaced fractures to limb-threatening injuries with neurologic and vascular damage and significant damage to the soft-tissue envelope. Tibial shaft fractures are often prone to complications, such as apex-anterior and valgus malalignments after nailing of the fractures in the proximal one third of the tibia, infection after open fractures, and aseptic nonunions. Understanding the common complications will aid
