Therapeutic Effect of BDNF-Overexpressing Human Neural Stem Cells (F3.BDNF) in a Contusion Model of Spinal Cord Injury in Rats.

The most common type of spinal cord injury is the contusion of the spinal cord, which causes progressive secondary tissue degeneration. In this study, we applied genetically modified human neural stem cells overexpressing BDNF (brain-derived neurotrophic factor) (F3.BDNF) to determine whether they can promote functional recovery in the spinal cord injury (SCI) model in rats. We transplanted F3.BDNF cells via intrathecal catheter delivery after a contusion of the thoracic spinal cord and found that they were migrated toward the injured spinal cord area by MR imaging. Transplanted F3.BDNF cells

[Traumatic intraventricular hemorrhage in severe head injury].

Among 329 cases with comatose state caused by severe head injury, 32 had primary intraventricular hemorrhage as revealed on initial CT scan (13.4%). We divided traumatic intraventricular hemorrhage into 4 types. Type 1: massive hemorrhage (9 cases). Type 2: subependymal hemorrhage (8 cases). Type 3: damage of fornix or septum pellucidum (8 cases). Type 4: Nieveau (7 cases). Most of these cases were caused by traffic accident. Shearing injury may be the most accurate mechanism to produce the intraventricular hemorrhage. Shear strain in severe head injury should cause tears of the subependymal v

Heterogeneous responses of aquaporin-4 in oedema formation in a replicated severe traumatic brain injury model in rats.

Aquaporin-4 (AQP4) is the most abundant water channel in the rat brain. In this study, the distribution pattern and mRNA expression levels of AQP4 were examined in a severe traumatic brain injury model by immunohistochemistry and reverse transcription-polymerase chain reaction. Oedema formation and blood-brain barrier (BBB) integrity were assessed by wet-dry weight measurements and immunostaining of endogenous IgG respectively. In the oedematous contusional cortex with impaired BBB integrity, negative immunostaining of AQP4 and down-regulation of its mRNA level were identified (P<0.05) at 1 da

[Pathogenesis of traumatic subdural hydroma].

Pathogenetic factors leading to the formation of traumatic subdural hygromas (TSH) are identified from analysis of the results of complex examination of 156 patients. Comparative biochemical tests of CSF, blood serum and the hygroma fluid in 15 patients showed the hygroma to contain CSF. The formation of TSH was always a consequence of brain contusion, usually of a severe degree, and rupture of the basal CSF cisterns. As the result of critical analysis of the existing theories of TSH pathogenesis a new conception is formulated.

Posttraumatic cerebral ischemia after fluid percussion brain injury: an autoradiographic and histopathological study in rats.

OBJECTIVES: Mild-to-moderate reductions in local cerebral blood flow (ICBF) have been reported to occur in rats after moderate (1.7-2.2 atm) fluid percussion brain injury. The purpose of this study was to determine whether evidence for severe ischemia (i.e., mean ICBF < 0.25 ml/g/min) could be demonstrated after severe brain injury. In addition, patterns of indium-labeled platelet accumulation and histopathological outcome were correlated with the hemodynamic alterations. METHODS: Sprague-Dawley rats (n = 23), anesthetized with halothane and maintained on a 70:30 mixture of nitrous oxide:oxyg

Fast micrographia and frontal lobe dysfunction post-traumatic brain injury: insights into pacing impairment and rehabilitation strategy.

Micrographia, characterised by small handwriting, is often linked to Parkinson's disease, but also resulted to injured brain lesions. The left-handed women in her 20s developed 'fast micrographia' after a traumatic brain injury from a traffic accident, showing bilateral subdural haematomas and frontal lobe contusions, but she had no paralysis and extrapyramidal symptoms. Neuropsychological tests showed reduced processing speed and memory deficits, aligning with frontal lobe damage. A 3-month handwriting training programme improved performance with cues, but issues persisted without them. This

Magnetic Resonance Imaging Findings Are Associated with Long-Term Global Neurological Function or Death after Traumatic Brain Injury in Critically Ill Children.

The identification of children with traumatic brain injury (TBI) who are at risk of death or poor global neurological functional outcome remains a challenge. Magnetic resonance imaging (MRI) can detect several brain pathologies that are a result of TBI; however, the types and locations of pathology that are the most predictive remain to be determined. Forty-two critically ill children with TBI were recruited prospectively from pediatric intensive care units at five Canadian children's hospitals. Pathologies detected on subacute phase MRIs included cerebral hematoma, herniation, cerebral lacera

Delayed Cerebrospinal Fluid Cleft Formation Through the Midbrain and Thalamus Following Hemicraniectomy and Cranioplasty for Acute Subdural Hematoma.

BACKGROUND AND IMPORTANCE: Cerebrospinal fluid (CSF) cleft formation through brain parenchyma following nonpenetrating traumatic brain injury (TBI) is a rare phenomenon. Here we present a unique case of delayed CSF cleft formation months after initial injury. CLINICAL PRESENTATION: A 41-yr-old male presented after a fall with a right convexity acute subdural hematoma and ipsilateral frontal contusion. He underwent emergent hemicraniectomy with subsequent autologous cranioplasty 2 mo later. At 10-mo follow-up his neurological status had improved. His magnetic resonance imaging (MRI) at that ti

Measurement of serum alkaline phosphatase isozyme I in brain-damaged patients.

The authors measured alkaline phosphatase isozyme I (ALP-I) in sera of 24 brain-damaged patients and four with disorders other than brain damage. The study population comprised three patients with postresuscitation encephalopathy, four with ruptured cerebral aneurysms, 14 with acute subdural hematoma and cerebral contusion, and three with nontraumatic intracerebral hemorrhage. ALP-I detected in brain damage is physicochemically different from the other known ALP-Is that appear in patients with obstructive jaundice or hepatoma. In the brain-damaged patients, ALP-I became elevated about 7 days a

[Current management of anesthesia and intensive therapy of expansive processes with intracranial hypertension].

The transitive cerebral distension which is necessary for the neuro-surgeon during interventions is obtained by moderate controlled hyperventilation, deliberate arterial hypotension, application of the anti-sludge therapy for the cerebral microcirculation. Only the initial mannitol dose applied is useful for the subject with intra-cranial hypertension. Mannitol is not active in cases with cerebral oedema due to severe cerebral contusion. In such cases corticoids are the major therapeutic indication. The anti-sludge effects of mega-doses of thiamine in cases with severe disturbances in the micr

[Buccofacial apraxia without limb apraxia or aphasia after right premotor area contusion: a case report].

Facial apraxia, as well as aphasia, has been associated with lesions in the cerebral hemisphere contralateral to the dominant hand. We describe a patient with severe facial apraxia caused by contusion in the right frontal operculum, premotor area and primary motor cortex ipsilateral to the dominant hand. The patient had no aphasia or limb apraxia. Magnetic resonance images of the brain reveal no abnormality of the hemisphere contralateral to the dominant hand. Thus, in some individuals, facial praxis is controlled by the hemisphere non-dominant for both handedness and language.

[Unusual injuries of the abdominal aorta].

A 23-year-old patient was admitted after a car accident with multiple fractures of the extremities, contusion of the brain and a sonographically confirmed haematoma in the retroperitoneum. After 24 hours laparotomy was indicated on account of suddenly developed hypotension. On operation a haemoperitoneum was found and an extensive haematoma in the retroperitoneum, the cause of which was injury of the abdominal aorta due to rupture of the lower mesenteric and lumbar artery. The authors demonstrate the not quite typical picture of an injury of a major artery and recommended early angiography.

A review of brain retraction and recommendations for minimizing intraoperative brain injury.

Brain retraction is required for adequate exposure during many intracranial procedures. The incidence of contusion or infarction from overzealous brain retraction is probably 10% in cranial base procedures and 5% in intracranial aneurysm procedures. The literature on brain retraction injury is reviewed, with particular attention to the use of intermittent retraction. Intraoperative monitoring techniques--brain electrical activity, cerebral blood flow, and brain retraction pressure--are evaluated. Various intraoperative interventions--anesthetic agents, positioning, cerebrospinal fluid drainage

[Importance of cerebrospinal fluid lactate determination in neurological diseases].

Lactic acid concentration has been determined in the cerebrospinal fluid (CSF) of 715 patients suffering from various neurological diseases. It was found to be most often elevated in cases of ischemic cerebral infarction, cerebral contusion, arteriosclerotic dementia, metastatic encephalitis, bacterial meningitis, menigiosis carcinomatosa and after epileptic seizures. In fewer cases lactate levels were increased with brain tumors, encephalitis, viral meningitis and radiculitis. Diagnostic relevance of CSF lactic acid determination is discussed with regard to ischemic cerebral disorders, differ

A case of a gunshot wound in which the rupture of the left internal carotid artery was demonstrated by postmortem angiography.

A 54-year-old man was shot into the face by a robber while sleeping in bed. Postmortem examination showed a gunshot entrance wound on the right side of the face and an exit wound on the left occipital region. Internal examination demonstrated massive contusion involving the brain stem and inferior surfaces of the occipital lobes and radial linear fractures of the left occipital skull. Although it was difficult to delineate the precise sites and extension of rupture in the craniocerebral vessels due to extensive brain damage and brain swelling, postmortem angiography indicated rupture of the le

Susceptibility weighted imaging in detecting hemorrhage in acute cervical spinal cord injury.

BACKGROUND AND PURPOSE: Susceptibility weighted imaging (SWI) is sensitive to deoxyhemoglobin and blood products such as hemosiderin in detecting microbleeds in the brain. However, there are no studies on SWI in the spine cord injury so far. The purpose of this study was to evaluate the role of SWI in detecting hemorrhage in acute cervical spinal cord injury (SCI). MATERIALS AND METHODS: Twenty-three patients with a history of acute cervical spine trauma were studied. High-resolution SWI, gradient-echo (GRE) T2* weighted-image (T2*WI) and conventional magnetic resonance imaging (MRI) were per

Cardiac troponin I as a predictor of arrhythmia and ventricular dysfunction in trauma patients with myocardial contusion.

BACKGROUND: Myocardial contusion during blunt chest trauma is common and may lead to potentially fatal cardiac complications. Therefore, it is useful to identify a serum marker reflecting the myocardial damage that can predict risk for cardiac complications. In this study, the authors determined the strength of the association between cardiac troponin I (cTnI) levels and the risk of arrhythmia or the development of left ventricular dysfunction in a cohort of patients with blunt chest trauma. METHODS AND RESULTS: In 187 multiply injured patients with blunt chest trauma, serial measurements of

[Diffuse axonal damages to the brain (the clinical picture, diagnosis and outcome)].

On the basis of complex clinical, computed tomographic, catamnestic, and pathomorphological examination of 336 patients with severe cerebrocranial injury the authors substantiate the need to distinguish, along with localized contusions and intracranial hematomas, diffuse axonal injuries of the brain whose biomechanics and pathogenesis are linked with trauma of angular or rotational acceleration-deceleration. The clinical manifestations and the course of diffuse axonal injuries are characterized by protracted coma occurring immediately after the injury with marked disorders of stem functions, p

Aspects on the cerebral perfusion pressure during therapy of a traumatic head injury.

An actively raised cerebral perfusion pressure by vasopressors is nowadays often advocated during therapy of a post traumatic brain oedema to improve oxygenation of the brain. In this paper we argue that the arterial pressure not uncritically can be raised as the subsequent increase in hydrostatic capillary pressure may favour transcapillary filtration if the blood-brain barrier is opened for solutes. Further, the use of vasoconstrictor drugs to increase the perfusion pressure may in fact impair oxygenation to the penumbra zones around brain contusions but also to other tissues of the body, li
