Smoke inhalation injury: a narrative review.

Burn Units frequently provide care to patients who have burn injuries and concomitant smoke inhalation injury. Inhalation damage is a complex multifaceted lung and systemic disease process, and the leading cause of mortality and morbidity in victims of fire tragedies. The degree of airway injury depends on the composition of smoke and the duration of smoke exposure. The prevalence of inhalation damage and related mortality rates among fire victims is high all over the world. This article presents the potential clinical impacts of this syndrome and the most important factors to consider when ex

Clinical smoke inhalation injury: systemic effects.

Exposure to asphyxiant gases is a ubiquitous feature of the fire environment, particularly in structural (or closed-space) fires. The clinician evaluating the smoke-inhalation patient must keep in mind the manifold actions of systemic toxicants upon an individual whose pulmonary gas exchange may be compromised and whose circulatory and metabolic status may be stressed by burns, hypo- or hypervolemia, and intercurrent disease. Also to be considered is the likelihood of exposure to multiple toxicants whose physiologic effects may be synergistic and whose therapeutic demands may be conflicting. A

[Introduction of tracheal intubation with the aid of an fiberoptic bronchoscope in inhalation injury].

This paper reported the clinical use of endotracheal intubation with the aid of fiberoptic bronchoscope in 17 patients with major burns (TBSA 91.38 +/- 6.41%) and inhalation injury during the early postinjury period, pre-operation and acute respiratory failure. The observations revealed that the use of transnasal endotracheal with the aid of fiberoptic bronchoscope significantly improved pulmonary ventilation and respiratory functions when the patients suffered from acute upper airway obstruction during the early postinjury period and acute respiratory failure caused by airway obstruction and

Characteristics of patients with injury secondary to smoking on home oxygen therapy transferred intubated to a burn center.

BACKGROUND: The aim of this study was to compare outcomes of patients who sustained burn and ostensible inhalation injuries while on home oxygen therapy with those suffering equivalent injuries via other mechanisms. STUDY DESIGN: Between December 2002 and January 2006, 109 burn patients were transferred to our center intubated. Their charts were retrospectively reviewed. Patients who sustained injuries while on home oxygen therapy were age and total body surface area matched to patients with inhalation and burn injuries secondary to other mechanisms. RESULTS: Fourteen of 109 patients were in

Suicide deaths by gas inhalation in Toronto: An observational study of emerging methods of suicide.

BACKGROUND: Suicide death by gas inhalation has been the subject of global scientific interest due to a rapid increase in the use of helium and charcoal gas for suicide. These may be particularly amenable to means restriction strategies. There has been little scientific attention of this phenomenon in Canada. METHODS: A review of coroner records was conducted for all suicide deaths in Toronto (1998-2015). Deaths were categorized as due to inhalational asphyxia by compressed gas (i.e. helium or nitrogen), charcoal burning or motor vehicle exhaust, with suicide deaths by other methods as a comp

Associationbetween hyperoxia and mortality in severely burned patients.

INTRODUCTION: The use of oxygen is a key component of acute burn resuscitation, particularly when there is concern for carbon monoxide toxicity or inhalation injury. Prior studies of critically-ill patients have shown an association between early hyperoxia and increased mortality. There are no studies to date evaluating outcomes related to excessive oxygen administration in burn patients. METHODS: We conducted a retrospective analysis of 219 severely burned patients to quantify the average amount of oxygen given during initial resuscitation, the level of carbon monoxide exposure, and to deter

Fluid volumes infused during burn resuscitation 1980-2015: A quantitative review.

INTRODUCTION: 'Fluid creep' or excessive fluid delivered to burn patients during early resuscitation has been suggested by several studies from individual burn centers. METHODS: We performed a Medline search from 1980 to 2015 in order to identify studies of burn patients predominantly resuscitated with lactated Ringers with infusion adjusted per urinary output. Data was abstracted for 48 publications (3196 patients) that met entry criteria. RESULTS: Higher resuscitation volumes compared to Parkland estimates were reported, but the trend of increasing resuscitation volumes over the last 30 ye

[National experts consensus on tracheotomy and intubation for burn patients (2018 version)].

Airway edema, stenosis, obstruction and even asphyxia are easy to occur in patients with extensive burn, deep burn of head, face, and neck area, inhalation injuries, etc., which threaten life. Timely tracheotomy and intubation is an important treatment measure, but lack of knowledge and improper handling in some hospitals resulted in airway obstruction. The technique of percutaneous tracheotomy and intubation provides convenience for emergency treatment of critical burns and mass burn. The Burn and Trauma Branch of Chinese Geriatrics Society organized some experts in China to discuss the indic

A Comprehensive, Retrospective Analysis of Variables for Potential Mortality Impact in Patients With Thermal or Inhalation Injury.

Age, percentage TBSA burned, and the presence of inhalation injury have been used historically in the prediction of mortality in thermally injured patients despite other factors being also associated with mortality. Recent literature has identified novel factors associated with increased length of stay (LOS) and may provide a better prediction model for mortality in burn patients. The study objective was to perform a subset analysis of a multitude of known and novel variables for potential association with mortality. Demographics and injury characteristics along with during stay variables were

Predictors of infectious complications after burn injuries in children.

BACKGROUND: Infections are the major life-threatening complication of burn injury and occur with the greatest frequency in children. Knowledge of their occurrence and management, however, is extrapolated from studies in adults. We performed a prospective study of infectious complications in burned children. OBJECTIVE: To delineate epidemiology, risk factors and microbiology of infections in burned children where burn care and surgical interventions are optimal. METHODS: Children hospitalized for burns were entered into prospective study. Characteristics of the burn injury were assessed, and

Early, fatal disseminated intravascular coagulation in a patient with 60 per cent burns.

A 23-year-old, previously fit and healthy young man was admitted to the West Midlands Regional Burns Unit at the Birmingham Accident Hospital, with 60 per cent body surface area burns and smoke inhalation. On arrival, he was briskly tachycardiac, hypotensive and disorientated. He was admitted to the intensive care unit, intubated and started on intermittent positive pressure ventilation. Despite prompt commencement of resuscitation and the infusion of enormous volumes of colloids, the patient remained oliguric. He soon developed severe haemorrhage from his gastrointestinal, urinary and respira

Severe burn injury: Body Mass Index and the Baux score.

OBJECTIVE: The revised Baux score (age total body surface area (TBSA) burned and inhalation injury)) is predictive of mortality in burn patients. Our study objective was to assess whether the addition of body mass index (BMI) to the revised Baux score would be of value. We posited that increasing BMI follows a pattern similar to age and TBSA in the revised Baux score after severe burn injury. METHODS: Patient data from the burn registry was queried for patients admitted between 1/1/2013 to 8/31/2019. Patients 12 years or older with a TBSA of 20% or greater burn were included. Risk Factors for In-Hospital Mortality in Smoke Inhalation-Associated Acute Lung Injury: Data From 68 United States Hospitals.

BACKGROUND: Mortality after smoke inhalation-associated acute lung injury (SI-ALI) remains substantial. Age and burn surface area are risk factors of mortality, whereas the impact of patient- and center-level variables and treatments on survival are unknown. METHODS: We performed a retrospective cohort study of burn and non-burn centers at 68 US academic medical centers between 2011 and 2014. Adult inpatients with SI-ALI were identified using an algorithm based on a billing code for respiratory conditions from smoke inhalation who were mechanically ventilated by hospital day 4, with either a

Burns During Pregnancy: Implications for Maternal-Perinatal Providers and Guidelines for Practice.

IMPORTANCE: When a major burn is suffered during pregnancy, the obstetric provider is challenged to respond on an evidence basis because personal experience usually is lacking. Currently, there is a paucity of publications to inform the obstetrician, guide practice, and impact early critical decision making. OBJECTIVE: The aims of this study were to summarize the available information on early management of burns during pregnancy and to identify components of best practices for optimal outcome. EVIDENCE ACQUISITION: The PubMed database was searched for relevant titles and abstracts involving

Assessing inhalation injury in the emergency room.

Respiratory tract injuries caused by inhalation of smoke or chemical products are related to significant morbidity and mortality. While many strategies have been built up to manage cutaneous burn injuries, few logical diagnostic strategies for patients with inhalation injuries exist and almost all treatment is supportive. The goals of initial management are to ensure that the airway allows adequate oxygenation and ventilation and to avoid ventilator-induced lung injury and substances that may complicate subsequent care. Intubation should be considered if any of the following signs exist: respi

[Burns].

In the Netherlands, an average of 92,000 patients with burn injuries are seen annually by a general practitioners. A burn injury is defined as skin damage caused by heat, radiation, electricity, and/or chemical agents. It is crucial to make an accurate assessment of the severity of the burn. The severity is primarily determined by the size (also referred to as the total burned body surface area, or TBSA), the depth of the burn, as well as the location, any associated injuries (such as inhalation trauma) and patient factors such as age and comorbidities. What initial measures do you take, what

Burn and smoke injury activates poly(ADP-ribose)polymerase in circulating leukocytes.

The nuclear enzyme poly(ADP-ribose)polymerase (PARP) plays a significant role in the pathogenesis of various forms of critical illness. DNA strand breaks induced by oxidative and nitrative stress trigger the activation of PARP, and PARP, in turn, mediates cell death and promotes proinflammatory responses. Until recently, most studies focused on the role of PARP in solid organs such as heart, liver, and kidney. We investigated the effect of burn and smoke inhalation on the levels of poly(ADP-ribosylated) proteins in circulating sheep leukocytes ex vivo. Adult female merino sheep were subjected
