Renal osteodystrophy is a common and incapacitating complication of chronic renal failure in children. Standard therapy with oral calcium supplements, phosphate binders, and vitamin D preparations is often inadequate to control progressive bone disease. We report the use of parenteral calcitriol therapy in two children, aged 2 and 15 years, respectively, with chronic renal failure. This treatment effectively suppressed secondary hyperparathyroidism in both patients, causing a nearly 50% reduction in circulating parathyroid hormone level and a parallel decline in serum alkaline phosphatase acti

Ischemic stroke, caused by arterial blockages, is a major global cause of mortality and long-term disability. Current treatments primarily aim to restore cerebral blood flow through the use of anticoagulant therapy and thrombectomy. Recent research has explored vitamin D3, particularly its active form, calcitriol, as an adjunctive treatment due to its neuroprotective effects. However, the exact mechanisms by which calcitriol mitigates ischemia-induced brain injury remain unclear. This study investigated the therapeutic potential of calcitriol in ischemia/reperfusion (I/R) injury using a rat mo

This study aimed to investigate the effects of calcitriol on endogenous biomarkers taurine and pyridoxic acid (PDA) associated with the organic anion transporters Oat1 and Oat1/3, respectively, and compare these changes with those observed for the clinical substrate, methotrexate (MTX). Male rats were administered intraperitoneal (i.p.) injections of either vehicle (maize oil) or calcitriol (2.56 nmol/kg/day) for 4 consecutive days. Plasma, urine, and tissue samples were analyzed using liquid chromatography-tandem mass spectrometry (LC-MS/MS). Calcitriol markedly increased plasma taurine level

BACKGROUND: Patients with immunoglobulin A nephropathy (IgAN) and moderate proteinuria are at risk of progressive renal failure despite the maximal use of renin-angiotensin system blockers (RASBs). This study evaluated the efficacy of active vitamin D in reducing proteinuria in patients with IgAN receiving RASBs.
METHODS: In this double-blind, placebo-controlled trial, we enrolled biopsy-confirmed patients with IgAN and persistent proteinuria (0.5-3.5 g/day) after maximal use of RASBs. Patients were randomized to receive either 0.25 μg/day of calcitriol or placebo for 12 months. The primary o

Sarcopenic obesity (SO) manifests as intramuscular lipid deposition and regenerative impairment triggered by dysregulation of the mitophagy-associated axis. Mitochondria-associated membranes (MAMs) are critical for lipid metabolism and mitochondrial function regulation. The mechanistic target of rapamycin (mTOR) is a functional protein at the MAMs and a central metabolic regulator; however, the underlying mechanisms by which mTOR dysregulation affects SO remain unclear. Calcitriol possesses the potential to improve mitochondrial function and fatty acid oxidation. This study investigated the me

Human rhinovirus (HRV) infection is one of the main causes of respiratory injury. Recently, calcitriol has been reported to have protective effect against respiratory infections. In this paper, we aimed to explore the effects and mechanisms of calcitriol on HRV-induced respiratory infection. Participants including pediatric patients diagnosed with HRV-induced respiratory infection (n = 50) and paired healthy controls (n = 40) were recruited at the Weifang People's Hospital between May 2019 and May 2020. The serum 25(OH)D3 level was measured in participants using ELISA kit. The HRV-induced resp

Hypercalcaemia in malignancy is most commonly caused by paraneoplastic secretion of parathyroid hormone-related protein or osteolytic metastases. Very rarely (<1% of cases), the mechanism behind increased serum calcium is increased production of calcitriol (1,25-dihydroxyvitamin D) and even rarer is the occurrence of this phenomenon in solid malignancies, with few such instances reported in the literature. We present a case of a neuroendocrine malignancy originating in the oesophagus associated with calcitriol-induced hypercalcaemia, a phenomenon that has not been previously described. We revi

BACKGROUND AND OBJECTIVES: In the United States, intravenous vitamin D analogs are the first-line therapy for management of secondary hyperparathyroidism in hemodialysis patients. Outside the United States, oral calcitriol (1,25-dihydroxyvitamin D3) is routinely used. We examined standard laboratory parameters of patients on in-center hemodialysis receiving intravenous vitamin D who switched to oral calcitriol.
DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: We conducted a retrospective cohort study of adult patients treated within Fresenius Kidney Care clinics. During a 6-month period (Decemb

Calcitriol, the biologically active form of vitamin D, has been reported to cause both suppressive and protective immune effects in mice. Its immune effects in vivo in humans are unclear. We investigated the in vivo effects of topical calcitriol on minimal erythema dose and skin immune responses in healthy volunteers. We found that calcitriol did not protect from ultraviolet (UV)-induced erythema (sunburn) when applied either 24 h before or immediately after irradiation, although it decreased the density of sunburn cells and thymine dimers seen on biopsy when applied 24 h before and again imme

In patients on chronic hemodialysis (CHD), hyperparathyroidism (HPTH) is associated with anemia and resistance to erythropoietin (EPO). In the last few years, calcitriol intravenously (IV) has been used with success in the treatment of the HPTH, secondary to chronic renal failure. However, the effects of calcitriol on the hematological parameters of these patients have never been well evaluated. This study included 11 elderly CHD patients (f = 6, m = 5; mean age = 73.6 years, mean time on CHD = 42.8 months) with HPTH under EPO therapy (IV). They were treated for 12 months with calcitriol IV (m

Hyperphosphatemia, calcitriol deficiency, and secondary hyperparathyroidism (SHPT) are common complications of chronic kidney disease (CKD). Fibroblast growth factor-23 (FGF-23) is a novel phosphaturic hormone that also inhibits renal 1alpha-hydroxylase activity and thus may be involved in the pathogenesis of SHPT. Several hypotheses were tested: that FGF-23 increases as renal function declines; is linearly associated with serum phosphate levels; is associated with increased phosphaturia independent of parathyroid hormone (PTH); and is associated with decreased calcitriol levels independent of

Phosphorus retention favors secondary hyperparathyroidism by decreasing calcitriol synthesis and serum calcium levels. However, a direct effect of high extracellular phosphorus on parathyroid (PTH) function, gene expression, and cell proliferation is still controversial. Normal rats were fed standard (St; 0.6% calcium, 0.6% phosphorus) or high phosphate (HP) diet (0.6% calcium, 1.2% phosphorus) for 18 days. To rule out transient decreases in serum calcium or calcitriol levels, sets of animals were sacrificed at different time periods after the last feeding (2, 4, 8, 12 or 24 hr). The HP diet l

Vitamin D metabolites in serum and calcitriol receptor concentration in parathyroid tissue were examined in 52 patients operated on for primary hyperparathyroidism. The calcitriol receptor levels were not different in parathyroid adenomas (mean 224 fmol/mg of protein, range 29-509, N = 43), normal parathyroid tissue (mean 245, range 31-690, N = 20), and primary parathyroid hyperplasia (mean 172, range 46-477, N = 9). Preoperative serum levels of calcitriol concentration correlated inversely to the calcitriol receptor in normal parathyroid tissue in patients with adenoma (r = -0.57, N = 17, p =

Calcitriol oral pulse therapy has been suggested as the treatment of choice for secondary hyperparathyroidism, but its efficacy and safety are still under discussion. The present randomized multicenter study compares the effect of an 8-week course of daily versus intermittent (twice weekly) calcitriol therapy on parathyroid hormone (PTH) suppression in 59 children (mean age 8.4+/-4.7 years) with chronic renal insufficiency (mean Ccr 22.4+/-11.6 ml/min per 1.73 m2) and secondary hyperparathyroidism. After a 3-week washout period, the patients were randomly assigned to treatment with daily oral

Hypercalcemia of malignancy is a recognized complication of diffuse large B-cell lymphoma (DLBCL) and is associated with decreased survival time and more advanced disease. We present a case of DLBCL complicated by severe refractory calcitriol-mediated hypercalcemia and renal failure that was unresponsive to conventional treatments. Treatment with glucocorticoids and concurrent antiresorptive therapy was also ineffective. Ultimately, hemodialysis was necessary to treat the patient's hypercalcemia and tenuous volume status. This case demonstrates that calcitriol-mediated hypercalcemia of maligna

A new series of highly biologically active (20S,22R)-1α,25-dihydroxy-22-methyl-2-methylene-vitamin D3 analogs, possessing different side chains, have been efficiently prepared as potential agents for medical therapy. Design of these synthetic targets was based on the analysis of the literature data and molecular docking experiments. The synthetic strategy involved Sonogashira coupling of the known A-ring dienyne with the C,D-ring enol triflates, obtained from the corresponding Grundmann ketones. All synthesized vitamin D compounds were characterized by high in vitro potency and, moreover, they

BACKGROUND: Secondary hyperparathyroidism (SHPT) is common in end-stage renal disease. Our primary objective was to evaluate the efficacy of oral paricalcitol versus oral calcitriol on serum intact parathyroid hormone (iPTH) and mineral bone parameters in continuous ambulatory peritoneal dialysis (CAPD) patients with SHPT. The secondary objective was to analyze highly sensitive C-reactive protein (hsCRP) and peritoneal membrane function in both groups.
METHODS: This was a prospective randomized control trial. CAPD patients with SHPT were randomized to paricalcitol or calcitriol for 15 weeks.

OBJECTIVE: To review the inter-relationships between calcium, phosphorus, parathyroid hormone (PTH), parent and activated vitamin D metabolites (vitamin D, 25(OH)-vitamin D, 1,25(OH)2 -vitamin D, 24,25(OH)2 -vitamin D), and fibroblast growth factor-23 (FGF-23) during chronic kidney disease (CKD) in dogs and cats.
DATA SOURCES: Human and veterinary literature.
HUMAN DATA SYNTHESIS: Beneficial effects of calcitriol treatment during CKD have traditionally been attributed to regulation of PTH but new perspectives emphasize direct renoprotective actions independent of PTH and calcium. It is now a

AIMS: To determine whether low-dose calcitriol attenuates atherosclerosis in apoE-null mice and, if so, through which predominant mechanism.
METHODS: Starting at the age of 6 weeks, mice received intraperitoneal injections of either 0.25 ng/g body weight of calcitriol or the vehicle, every other day for 8 weeks.
RESULTS: Calcitriol treatment resulted in 35% reduction of atherosclerosis at the aortic sinus, and in a significant decrease in blood pressure. These effects were possibly mediated by downregulation of the renin-angiotensin system (RAS), as there was a 64% decrease in the aortic lev
