SPECT MIBI imaging for cardiac output and index in end stage renal disease.

To compare cardiac output (CO) and cardiac index (CI) and left ventricular ejection fraction (LVEF) in end-stage renal disease (ESRD) with a control group using gated single photon emission computed tomography (SPECT)/computed tomography (CT) imaging. Altered cardiovascular function with increased CO secondary to arterio-venous fistulas (AVF) for dialysis has been reported in patients with ESRD. Thirty-two patients (18 with AVF or graft) referred for pre-renal transplant cardiac assessment using SPECT/CT were studied with 2 comparison groups, 42 normal weight (body mass index<30) and 46 obese

Low cardiac output due to left main coronary embolism during mitral valve replacement.

A 55-year-old woman underwent elective mitral valve replacement due to severe rheumatic mitral valve stenosis. After valve replacement with a mechanical prosthesis, the patient failed to wean from the cardiopulmonary bypass. An aortotomy was performed and thrombotic embolic material was removed from the left coronary ostia with a Fogarty catheter allowing for successful discontinuation from cardiopulmonary bypass. Peripheral emboli are a known complication in patients undergoing mitral valve surgery for rheumatic mitral stenosis. In this report, we describe the case of a patient who sustained

Effect of bundle branch block on cardiac output: a whole heart simulation study.

The heart is an electrically controlled fluid pump which operates by mechanical contraction. Whole heart modelling is a computationally daunting task which must incorporate several subsystems: mechanical, electrical, and fluidic. Numerous feedback mechanisms on many levels, and operating at different scales, exist to finely control behaviour. Understanding these interactions is necessary to understand heart operation, as well as pathologies and therapies. A review of the components in such a model is given. The authors then present a framework for their electro-mechano-fluidic whole heart mode

Comparison of two cardioplegia solutions using thermodilution cardiac output in neonates and infants.

BACKGROUND: Information from clinical studies is limited regarding the optimal method for myocardial protection in immature hearts, and specifically the benefits of different cardioplegia (CP) formulations. We compared two CP techniques by evaluation of the cardiac index using thermodilution catheters. METHODS: The study cohort includes 102 neonates and infants (aged 2 to 269 days) undergoing biventricular repair surgery. A total of 52 patients were managed with commercially available crystalloid CP ("standard CP") and 50 had a custom mix of crystalloid CP with dilute blood ("custom CP"). Rep

Oscillations in stroke volume and cardiac output arising from oscillatory ventilation in humans.

Oscillations in the cardiovascular system have been observed in patients with periodic breathing. It is not clear whether these are driven by primary oscillations in the respiratory system or whether an intrinsic cardiovascular instability is required, as previous studies with subjects performing voluntary periodic breathing have failed to produce the cardiovascular oscillations. We investigated whether cardiovascular oscillations occurred in healthy controls performing voluntary periodic breathing. Six healthy subjects performed voluntary periodic breathing with guidance from a real-time comp

[Effects of cardiac output on PETCO2 and PaCO2 during combined inhalational-epidural anesthesia].

We investigated the effects of cardiac output on PETCO2 in anesthetized patients. We studied 8 adult patients undergoing long-lasting lower abdominal surgery. Anesthesia was maintained with epidural combined with inhalational anesthesia. The minute ventilation volume was kept constant at 10 ml.kg-1 x 10 cycles.min-1. PETCO2, PaCO2, and cardiac index, (CI) by thermodilution method were measured simultaneously. PaCO2 was corrected for body temperature for comparison with PETCO2. Approximate value of alveolar dead space to tidal volume ratio was calculated as VD/ VTalv = (PaCO2-PETCO2)/PaCO2. The

Thermodilution left-sided cardiac output for valve area determination after balloon mitral valvotomy.

This study was conducted to establish the validity of left-sided cardiac output measurement with a Swan-Ganz catheter and assess its accuracy in estimating mitral valve area (MVA) by the Gorlin formula. The use of right-sided cardiac output after balloon mitral valvotomy (BMV) can give inaccurate measurements for Gorlin-derived MVA because of the atrial septal defect (ASD) created during the procedure. The left-sided cardiac output was measured with a Swan-Ganz catheter (proximal port in the left atrium and then in the left ventricle, and distal port in the ascending aorta) in 10 consecutive p

Determination of cardiac output from pulse pressure contour during intra-aortic balloon pumping.

The reliability of cardiac output determinations from radial pulse pressure contour during intra-aortic balloon pumping (IABP) has been tested and compared with thermodilution measurements. During IABP the pulse pressure curve is distorted by preset volume changes in the aorta. The pressure decrease following balloon deflation was compared with pressure increase following systolic ejection to determine stroke volume and cardiac output. The analysis and calculations were computerized. Measurements were made in thirteen patients with left ventricular failure. The algorithmic analyser failed to p

Association between hydrogen gas inhalation and cardiac output in an asphyxiated piglet model.

About 80% of neonates with hypoxic-ischaemic encephalopathy (HIE) have cardiovascular dysfunction and are vulnerable to late-onset injury due to disrupted cerebral blood flow and metabolism. We previously reported the neuroprotective effect of hydrogen (H2) gas. This study investigated the effects of H2 gas inhalation on cardiac function in piglets after hypoxia-ischaemia (HI). Seventeen piglets ≤ 24 h old were subjected to HI insult for approximately 40 min and, after 10 min of resuscitation, were divided into an HI group (n = 10) and an HI-H2 gas inhalation group (2.2%–2.7% H2 gas for 6 h; n

Positive end-expiratory pressure impact on cardiac output and cerebral hemodynamics in newborns.

UNLABELLED: This study aimed to clarify the extrapulmonary effects of positive end-expiratory pressure (PEEP) in neonates using noninvasive monitoring techniques to monitor cardiac output (CO) and cerebral hemodynamics. This was a prospective, nonrandomized, consecutive enrollment, single-centre study. Newborns weighing ≥ 1,000 g admitted to the neonatal intensive care unit for invasive mechanical ventilation from April 2023 to December 2024 were included. PEEP levels increased sequentially to 5 cm H2O, 7 cmH2O, and 10 cm H2O or peak inspiratory pressure not exceeding 25 cmH2O (incremental pha

Urine Output and Mortality in Patients Resuscitated from out of Hospital Cardiac Arrest.

BACKGROUND: Limited data exist regarding urine output (UO) as a prognostic marker in out-of-hospital-cardiac-arrest (OHCA) survivors undergoing targeted temperature management (TTM). METHODS: We included 247 comatose adult patients who underwent TTM after OHCA between 2007 and 2017, excluding patients with end-stage renal disease. Three groups were defined based on mean hourly UO during the first 24 h: Group 1 (<0.5 mL/kg/h, n = 73), Group 2 (0.5-1 mL/kg/h, n = 81) and Group 3 (>1 mL/kg/h, n = 93). Serum creatinine was used to classify acute kidney injury (AKI). The primary and secondary outc

Cardiac Output and Cerebral Blood Flow: A Systematic Review of Cardio-Cerebral Coupling.

Control of cerebral blood flow (CBF) is crucial to the management of neurocritically ill patients. Small studies which have examined the role of cardiac output (CO) as a determinant of CBF have inconsistently demonstrated evidence of cardio-cerebral coupling. Putative physiological mechanisms underpinning such coupling include changes in arterial blood pressure pulsatility, which would produce vasodilation through increased oscillatory wall-shear-stress and baroreceptor mediated reflex sympatholysis, and changes in venous backpressure which may improve cerebral perfusion pressure. We sought to

Normative Reference Values of Cardiac Output by Pulsed-Wave Doppler Echocardiography in Adults.

Cardiac output (CO) is routinely assessed by pulsed-wave Doppler echocardiography, yet reference values in adults are lacking. We aim to establish normative values of CO and cardiac index (CI) by pulsed-wave Doppler-echocardiography and to analyze their relation with gender and age in nonobese and obese adults. We included 4,040 adults (mean age: 55 years, 53% women, 950 obese [body mass index ≥30 kg/m²]) with normal blood pressure, no history of cardiovascular disease, and normal transthoracic echocardiography. Normative reference CO and CI values for were calculated in 3,090 nonobese patient

Lactate acidosis and cardiac output during initial therapeutic cooling in asphyxiated newborn infants.

AIM: We hypothesized that compromised cardiac output in asphyxiated infants may influence on the rate of disappearance of lactate due to insufficient perfusion. METHODS: The study was a prospective, observational study, where infants with perinatal asphyxia who met the criteria for therapeutic hypothermia were included. Cardiac output, stroke volume and heart rate were measured by electrical velocimetry in 15 newborn infants with perinatal asphyxia during the first six hours of active therapeutic hypothermia. Results from routine blood samples were collected retrospectively. Cardiac parameter

Worsening of nose bleeding heralds high cardiac output state in hereditary hemorrhagic telangiectasia.

BACKGROUND: Hereditary hemorrhagic telangiectasia is characterized by arteriovenous malformations or telangiectasias in multiple organs. Nose bleeding is the most common manifestation of hereditary hemorrhagic telangiectasia and can be debilitating. Cardiac involvement in the form of high-output cardiac failure is a poorly studied complication of hereditary hemorrhagic telangiectasia. The objective of this study is to describe the natural history of high-output cardiac failure in hereditary hemorrhagic telangiectasia and define the relation between bleeding complications and high-output cardia

Percutaneous mitral valvuloplasty in patients with low cardiac output at high surgical risk.

Among 1146 patients undergoing percutaneous mitral valvuloplasty for symptomatic mitral stenosis, 8 (4 men and women) were at high risk for surgery on the basis of the New York Heart Association functional class IV (n=8), severe pulmonary hypertension (n=5). All these patients aged 30+/-23.6 years had signs of right heart failure, high echocardiographic score (9.6+/-3.6) and low mitral valve area (0.50+/-0.19 cm(2)). The procedure resulted in an increase in mitral valve area (1.55+/-0.17 cm(2)) with a concomitant reduction in pulmonary artery systolic pressure (58.7+/-9.9 mm Hg) and decrease i

Recurrent gastrointestinal bleeding and high output cardiac failure caused by hereditary hemorrhagic telangiectasia.

Hereditary hemorrhagic telangiectasia, commonly known as Osler-Weber-Rendu disease, is a systemic autosomal dominant inherited disorder, that occurs in Caucasian populations. We report the case of a 56-year-old housewife who was admitted to the gastrointestinal and cardiovascular ward because she had suffered from recurrent gastrointestinal bleeding and heart failure from 1994 to 1997. Panendoscopy showed vascular ectasia scattered over the tongue, larynx, esophagus, and posterior wall of the gastric body. Colonoscopy showed clusters of telangiectasia over the cecum and ascending colon. Arteri
