Pathology of isolated nonclassical and catastrophic manifestations of Takayasu arteritis.

Takayasu arteritis is usually defined as a chronic, progressive, inflammatory, occlusive disease of the aorta and its branches. However, we should remind Takayasu arteritis as a systemic disease. Here I describe nonclassical and catastrophic manifestations of the Takayasu arteritis, which often go unrecognized until after the event. Especially I stress that we should focus on cardiopulmonary complications in Takayasu arteritis.

Acalculous ischemic gallbladder necrosis in the catastrophic antiphospholipid syndrome.

A 29-year-old woman was referred for abdominal pain. Results of tests for lupus anticoagulant and antibodies to phosphatidylserine and to beta2-glycoprotein I were positive, but the patient had no features of systemic lupus erythematosus (SLE). Abdominal ultrasonography showed a thickening of the gallbladder wall without cholelithiasis. A surgical procedure revealed necrotic areas of the gallbladder wall, and a cholecystectomy was performed. Histologic examination of the gallbladder showed multiple thrombi and no vasculitis. Despite full-dose heparin, the patient developed a catastrophic antip

Catastrophic antiphospholipid antibody syndrome in primary systemic sclerosis.

We describe the first case of catastrophic antiphospholipid antibody syndrome (APS) in a patient with progressive systemic sclerosis (SSc). After initial presentation with digital gangrene the patient developed rapidly progressive multiorgan failure and died within 19 hours. Postmortem examination revealed extensive multiorgan arterial microthrombi of days' to months' duration. This suggests that a subclinical state of thrombosis existed before onset of catastrophic APS. Given the poor prognosis of established catastrophic APS there is a need for a means to detect subclinical thrombosis and tr

Catastrophic metabolic encephalopathies in the newborn period. Evaluation and management.

The newborn who presents with neurologic symptoms such as seizures or lethargy due to inborn error of metabolism is an important problem. Although each inborn error that presents in this manner is rare, these conditions are not rare as a group, and more than one in 1000 babies is affected with one of the more than 100 different inborn errors that are now known. Many of these conditions present with much the same features seen in sepsis or asphyxia and, when untreated, can lead rapidly to death or permanent neurologic damage. Early diagnosis and management may prevent some or all of this morbid

Rapid catastrophic onset of Wegener's granulomatosis in a renal transplant.

A 62-year-old man with chronic renal failure secondary to Wegener's granulomatosis received a cadaveric renal graft in December 1990 after 2 years' treatment with continuous ambulatory peritoneal dialysis. Early graft function was good and serum creatinine fell steadily. On day 5 he became anuric and the graft became swollen and tender. Isotope renography showed no perfusion and the kidney was removed. Pathological examination showed widespread acute arteritis consistent with recurrent Wegener's granulomatosis. Recurrence of Wegener's granulomatosis in renal grafts is rare. The 4 previously re

Catastrophic complication of simple cast treatment: case report.

We report the case of a 36-year-old man who developed a compartment syndrome after a minor ankle fracture was casted. The treatment was inadequately monitored and he subsequently developed the manifestations of a crush syndrome. This case demonstrates that relatively minor injuries can indeed be fatal if appropriate vigilance is not maintained.

Successful management of catastrophic gastrointestinal involvement in polyarteritis nodosa.

A patient with polyarteritis nodosa developed necrotizing enterocolitis, as indicated by pneumatosis intestinalis seen on computed tomographic scans of the abdomen. Despite immunosuppressive therapy and concomitant resolution of the intramural and portal venous gas and general clinical improvement, on 2 occasions (between 20 and 30 days later) the patient developed bowel infarctions and perforations that necessitated bowel resection. Leaks developed at anastomotic sites, but were not closed surgically. However, these sites and the lower quadrants of the abdomen were drained, and the patient wa

Thrombotic catastrophe in the patient with multiple Björk-Shiley prostheses.

Thrombosis of the Björk-Shiley prosthesis has been a recognized problem for many years. Review of 172 patients at the University of Kentucky Medical Center who had one or more Björk-Shiley valves inserted between January, 1975, and July, 1980, revealed special problems in those patients with multiple prostheses. Diagnosis and therapy prove more difficult, and the cumulative incidence of thrombosis in the patients with multiple prostheses is 26.8% at six years. Projected long-term use of multiple Björk-Shiley prostheses is discouraged.

Identification and validation of mitotic catastrophe-associated biomarkers for keloid.

PURPOSE: Exploring the role of mitotic catastrophe-related genes (MCRGs) in the development of keloid, a benign skin disorder. METHODS: Differentially expressed genes (DEGs) between keloid and normal tissues were identified. Candidate genes at the intersection of DEGs and MCRGs were selected. Protein-protein interaction network, machine learning, and expression analyses allowed the identification of keloid-associated biomarkers. Nomogram construction, Gene Set Enrichment Analysis (GSEA), and immune cell infiltration, molecular regulatory network, drug prediction, and molecular docking analyse

Reflections of a Catastrophic Earthquake Disaster on Public Mental Health.

OBJECTIVES: We aimed to reveal the psychological effects of old and young individuals after the earthquake by looking at their post-traumatic stress disorder (PTSD) and depression situations. METHODS: Data collected by face-to-face survey method were analyzed. The 4 provinces (Hatay, Kahramanmaraş, Adıyaman, and Gaziantep) most affected by the 7.7 and 7.6 magnitude earthquakes centered in Kahramanmaraş, Turkey on February 6, 2023 were included in the study. Peritraumatic distress inventory, impact of events scale, and Beck depression inventory were applied to 300 people (150 old, 150 young).

Catastrophic Acute Aortic Dissection in a Young Healthy Patient.

BACKGROUND: Acute type A aortic dissection (ATAAD) typically affects older patients with hypertension or connective tissue disorders; occurrence in young adults without identifiable risk factors is rare and diagnostically challenging. CASE SUMMARY: A healthy 41-year-old man presented with sudden severe chest pain, markedly elevated blood pressure, and tachycardia. Computed tomography angiography revealed extensive ATAAD involving multiple major vessels. Emergency surgery, including aortic root replacement and frozen elephant trunk repair, was complicated by severe postoperative coagulopathy,

Pain Catastrophizing and Substance Misuse: A Scoping Review of the Literature.

BACKGROUND: Pain catastrophizing, or the interpretation of pain as unbearable or intolerable, can increase pain-related anxiety and severity. High levels of pain catastrophizing have also been linked to substance use, particularly for substances with analgesic properties. Importantly, behavioral treatments can reduce pain catastrophizing, making them promising interventions for mitigating pain-related substance use. The aim of this scoping review is to summarize literature investigating the relationship between pain catastrophizing and substance use in adults. METHODS: We performed a scoping

Thrombotic Storm: A Challenging Case of Catastrophic Antiphospholipid Syndrome.

We present the clinical case of a 68-year-old woman with a history of cerebrovascular disease, primary Sjögren's syndrome, and suspected antiphospholipid syndrome, admitted for acute diarrhea and fever. She showed good clinical and laboratory improvement under ciprofloxacin until the fifth day of hospitalization, when she developed abdominal pain with guarding, fever, and vomiting. Laboratory tests revealed elevated inflammatory markers and rhabdomyolysis. An abdominopelvic CT scan showed multiorgan ischemia, raising the suspicion of catastrophic antiphospholipid syndrome (CAPS).

Perforating invasive hydatidiform mole a catastrophe: A case report.

A hydatidiform mole, also known as molar pregnancy, is a form of gestational trophoblastic disease (GTD) characterized by abnormal trophoblastic proliferation originating from the placenta. An invasive mole, a rare but potentially life-threatening variant of GTD, is defined by the invasion of trophoblastic tissue into the myometrium and, in some cases, beyond the uterus. We present the case of a 30-year-old multiparous woman who presented with persistent vaginal bleeding and lower abdominal pain approximately one month after self-administering an oral abortifacient. Emergency surgical interven

A Systematic Review of Pain Catastrophizing and Chronic Musculoskeletal Pain.

OBJECTIVES: This study was designed to synthesize recent randomized controlled trials examining the associations between pain catastrophizing and four key pain-related outcomes (i.e., pain intensity, disability, pain interference, and physical function) among adults with chronic musculoskeletal pain. This review clarifies the role of pain catastrophizing in pain outcomes to inform targeted interventions. DESIGN: A systematic review was performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. METHODS: The Cumulative Index of Nursing and Allied Hea

Ruptured uterus: a rare and catastrophic complication of molar pregnancy.

INTRODUCTION: Gestational trophoblastic disease is the term used to describe the heterogeneous group of interrelated lesions that arise from abnormal proliferation of placental trophoblasts. The clinical presentations of the gestational trophoblastic tumor include vaginal bleeding, uterine enlargement greater than expected for gestational age, hyperemesis, and secondary clinical hyperthyroidism. Less common presentations include pregnancy-induced hypertension in the first or second trimester of pregnancy and theca lutea cyst torsion. It is a rare incident to find uterine rupture because of a m
