Penalizing Parenthood and Perpetuating Poverty: Black Parents' Perspectives on Wisconsin's Birth Cost Recovery Policy.

Social safety-net policies and programs are often a double-edged sword—they provide necessary resources, but also punish poor Americans. In Wisconsin, one policy that exemplifies this pattern is Birth Cost Recovery, a state-enforced cost recovery policy through which unmarried, non-custodial fathers are required to repay Medicaid-covered delivery expenses. Supporters assert that Birth Cost Recovery promotes paternal responsibility and increases the sustainability of safety-net programs, but opponents contend it has detrimental effects on families. Yet few studies have explored Birth Cost Recov

Preparing Families of Technology-Dependent Children for Emergencies.

OBJECTIVES: To characterize the baseline level of emergency preparedness among families of technology-dependent children admitted to our PICU and to determine if an ICU-based planning intervention can sustainably improve families' disaster preparedness. METHODS: A single-arm, survey-based study used to assess participants' preparedness to handle a 72-hour home power outage on the basis of a novel 8-point checklist. Parents of patients in the study completed the survey questions when their child was admitted to the PICU, discharged, or transferred from the PICU, after at least 2 weeks at home,

Forensic and medical legal issues in addiction psychiatry.

There is a broad range of forensic issues in addiction psychiatry. For many psychiatrists, what is most challenging about addressing these issues is their origin in the law, which is a system and a way of thinking that may feel foreign to the medical practitioner. To address forensic issues, addiction psychiatrists should learn and understand the specific legal questions that arise in each forensic context. They should become familiar with the relevant definitions, criteria, and legal requirements that apply in each specific area of their practice, rather than assume that clinical definitions

Postdivorce living arrangements, parent conflict, and long-term physical health correlates for children of divorce.

The authors tested a biopsychosocial model in which young adults' long-term relationships with fathers and ongoing distress surrounding their parents' divorces mediated the relationship between disrupted parenting (i.e., exposure to parent conflict before the divorce and up to 5 years after, and amount of time with father postdivorce) and indicators of their physical health. University students whose parents divorced before they were 16 (n = 266) participated. Findings supported the model. The more time children lived with their fathers after divorce, the better their current relationships wer

"Violence. Enough already": findings from a global participatory survey among women living with HIV.

INTRODUCTION: Women living with HIV are vulnerable to gender-based violence (GBV) before and after diagnosis, in multiple settings. This study's aim was to explore how GBV is experienced by women living with HIV, how this affects women's sexual and reproductive health (SRH) and human rights (HR), and the implications for policymakers. METHODS: A community-based, participatory, user-led, mixed-methods study was conducted, with women living with HIV from key affected populations. Simple descriptive frequencies were used for quantitative data. Thematic coding of open qualitative responses was pe

Volatile substance abuse and crime: data from U.K. press cuttings 1996-2007.

Volatile substance abuse (VSA, solvent abuse, 'glue sniffing'), carries a risk of sudden death (some 700 deaths in the UK, 1996-2006). However, mortality data take no account of the social cost of the habit. From press cuttings we have identified 508 instances (569 individuals: 507 male, median age 25 yr, range 8-51 yr and 62 female, median age 18 yr, range 11-36 yr) where VSA, either alone or together with alcohol/other drugs, was reported in association with criminal or antisocial behaviour that resulted in a criminal conviction or caution. The frequency of reports decreased from 84 per annu

When is the personal professional in public child welfare practice? The influence of intimate partner and child abuse histories on workers in domestic violence cases.

OBJECTIVE: The objective of this article is to examine children's services workers' (CSWs') personal histories of abuse and their influence on professional responses to allegations of domestic violence. METHOD: Using an anonymous questionnaire, data were collected from CSWs in two large urban counties in Southern California who participated in a domestic violence training program (n=303). It was hypothesized that CSWs' responses to domestic violence cases would be affected by personal histories of abuse and the degree to which the CSWs identified with battered women. RESULTS: Approximately o

The Effects of Virtual Reality Telemedicine With Pediatric Patients Diagnosed With Posttraumatic Stress Disorder: Exploratory Research Method Case Report.

BACKGROUND: Trauma-focused cognitive behavioral therapy (TF-CBT) strategies are common interventions to treat child trauma and a posttraumatic stress disorder (PTSD) diagnosis in children with histories of sexual and physical abuse. With the advent of COVID-19, the disruption of child development combined with intense exposure to technology and screen time indicate a need for delivering other novel approaches to treat pediatric PTSD. Virtual reality (VR) has been used with evidence-based TF-CBT as an intervention in lab-based settings, but never as telehealth. Such technologies, including a VR

Study protocol for screening and diagnosis of fetal alcohol spectrum disorders (FASD) among young people sentenced to detention in Western Australia.

INTRODUCTION: Prenatal alcohol exposure can cause lifelong disability, including physical, cognitive and behavioural deficits, known as fetal alcohol spectrum disorders (FASD). Among individuals with FASD, engagement with justice services is common. Little is known about the prevalence of FASD among young people engaged with the Australian justice system. This study aims to establish FASD prevalence among sentenced young people in detention in Western Australia (WA), and use the findings to develop a screening tool for use among young people entering detention. Translation of these results wil

Contact and truth: the unfolding predicament in adoption and fostering.

It is now time to examine the crystallizing results of the social experiment of contact, direct and indirect, and to scrutinize the arguments supporting the predominant conviction that the benefits of regular contact are certain. Many adopted and fostered children who have suffered maltreatment at the hands of their birth parents have regular face-to-face meetings with them as well as routine communication in the form of cards or letters. It is commonly maintained that contact has to be beneficial because it is necessarily in the child's best interests in the long run, if not also immediately.

Long-term effects of nurse home visitation on children's criminal and antisocial behavior: 15-year follow-up of a randomized controlled trial.

CONTEXT: A program of home visitation by nurses has been shown to affect the rates of maternal welfare dependence, criminality, problems due to use of substances, and child abuse and neglect. However, the long-term effects of this program on children's antisocial behavior have not been examined. OBJECTIVE: To examine the long-term effects of a program of prenatal and early childhood home visitation by nurses on children's antisocial behavior. DESIGN: Fifteen-year follow-up of a randomized trial. Interviews were conducted with the adolescents and their biological mothers or custodial parents.

The psychosocial treatment of parental alienation.

Parental alienation occurs in divorces when one parent indoctrinates the child to dislike, fear, and avoid contact with the other parent. Mental health professionals who treat children and adolescents are likely to encounter victims of parental alienation in clinical practice, and it is important to identify and treat these youngsters earlier, when the condition is mild, rather than later, when the parental alienation is almost intractable. This article presents an overview of the treatment of parental alienation, which is called reunification therapy. All the parties involved in the case have

Antipsychotic-induced somnolence in mothers with schizophrenia.

Although it is known that many antipsychotic drugs, at the doses prescribed for schizophrenia, are sedative and cause daytime drowsiness, the effect of potentially diminished vigilance on parenting parameters has not been studied. The aim of this paper is to advise clinicians about sedative load in mothers who are prescribed antipsychotic medication. A Medline search was conducted into the sedative effects of antipsychotics, with the following search terms: sleep; sedation; somnolence; wakefulness; antipsychotics; schizophrenia, parenting, maternal behavior, and custody. The results showed tha

Professionals' decision-making about out-of-home placements following instances of child abuse.

OBJECTIVE: The goal of this research was to study how different groups of child welfare professionals prioritize and use information to make placement decisions following instances of child abuse. METHOD: A total of 90 juvenile court judges and guardians ad litem, Court Appointed Special Advocates (CASA), social workers, and mental health professionals responded to a detailed questionnaire describing four case studies of child physical abuse in which a parent was the perpetrator, the child was either 2 years or 6 years of age, and the abuse was either first time or chronic. Externalizing behaviors among children of HIV seropositive former and current drug users: parent support network factors as social ecological risks.

Children affected by their parents' dual drug use and HIV/AIDS face considerable challenges to their psychosocial development, including parent dysfunction and foster care placement. While HIV/AIDS may increase parents' mobilization of social support, their drug use may restrict who is available to help them, with potential implications to the adjustment of their children with whom they remain in contact. This study sought to identify dually affected children's living situations, and parent and parent's support network factors as correlates of children's externalizing problem behaviors. An urb

Barriers to opioid replacement therapy in pregnant women with opioid use disorder: a systematic review.

BACKGROUND: Opioid use disorder (OUD) during pregnancy has increased dramatically, paralleling epidemic trends in the general population. Pregnant women with OUD face considerable risks of adverse outcomes. While medications for opioid use disorder (MOUD) is the mainstay of treatment, substantial barriers limit its implementation. Despite the critical need for effective interventions, a comprehensive synthesis of the challenges to MOUD during pregnancy remains lacking. This review examines these barriers to improve treatment access and outcomes for this vulnerable population. METHODS: This re
