When should I repeat the endoscopy in my patient with compensated cirrhosis, whom I just scoped and who had no or small varices?

Endoscopic surveillance for patients with compensated advanced chronic liver disease (cACLD) is currently guided by the presence or size of esophageal varices. Current guidelines recommend repeat endoscopy every 2-3 years for patients with no varices and every 1-2 years for those with small varices, but these intervals are based largely on expert opinion rather than objective risk stratification. We evaluated the Disease Severity Index (DSI) from the oral cholate challenge test as a predictor of clinical outcomes in 195 patients from the Hepatitis C Anti-viral Long-term Treatment against Cirrh

Analysis of the effect of phototherapy on intestinal probiotics and metabolism in newborns with jaundice.

BACKGROUND: In clinical practice, oral probiotics are often given to children with hyperbilirubinaemia who receive phototherapy, but the exact mechanism of the action of the probiotics on hyperbilirubinaemia remains unclear. It is unclear how the effects of phototherapy on the probiotic flora in the neonatal gut, in particular. MATERIALS AND METHODS: Fifty newborns who needed phototherapy from June 2018 to June 2020 were selected as the study subjects, and five healthy newborns in the same period were used as controls to analyse the changes in probiotic bacteria in their faeces. RESULTS: 1.

On-line preconcentration of fluorescent derivatives of catecholamines in cerebrospinal fluid using flow-gated capillary electrophoresis.

Flow-gated capillary electrophoresis (CE) coupled with microdialysis has become an important tool for in vivo bioanalytical measurements because it is capable of performing rapid and efficient separations of complex biological mixtures thus enabling high temporal resolution in chemical monitoring. However, the limit of detection (LOD) is often limited to a micro- or nano-molar range while many important target analytes have picomolar or sub-nanomolar levels in brain and other tissues. To enhance the capability of flow-gated CE for catecholamine detection, a novel and simple on-line sample prec

Effects of luminal nutrients and small bowel transplants on congenital indirect hyperbilirubinemia.

The Gunn rat is an excellent model of Crigler-Najjar syndrome, type 1. In previous studies we demonstrated that heterotopic 15-20-cm jejunal transplants from Wistar rats lowered serum bilirubin levels by 40%, and the reduction was transient (6 weeks). In contrast, orthotopic transplants decreased bilirubin levels by 60% and the effect persisted throughout the 8-week study. This study was initiated to identify the luminal substance(s) which are responsible for the persistent bilirubin-lowering effect of jejunal transplants. Thirty-one Wistar to Gunn 15-20-cm jejunal transplants were randomized

A high-fat diet with vitamin D and propylthiouracil produces a pro-atherogenic phenotype in rats.

BACKGROUND: Atherosclerosis begins with dyslipidemia, vascular inflammation, and endothelial dysfunction. Rodent models that capture these early events are needed for mechanistic and interventional studies. This study evaluated whether a cholesterol-rich, high-fat diet (HFD) supplemented with vitamin D and propylthiouracil (PTU) promotes a pro-atherogenic phenotype in rats, as evidenced by dyslipidemia, inflammation, markers of endothelial dysfunction, and early vascular remodeling. METHODS: Male Sprague-Dawley rats (n = 18) received standard chow or a HFD containing 2% cholesterol, 3% lard,

Targeted Sodium Acetate Liposomes for Hepatocytes and Kupffer Cells: An Oral Dual-Targeted Therapeutic Approach for Non-Alcoholic Fatty Liver Disease Alleviation.

Background/Objectives: Sodium acetate (NaA) has demonstrated potential in improving non-alcoholic fatty liver disease (NAFLD) by targeting hepatocytes and Kupffer cells. However, its clinical application is hindered by low oral bioavailability and insufficient liver concentrations. Liposomes, with their capacity to encapsulate water-soluble drugs and be surface-modified, offer a promising solution for targeted oral drug delivery. Methods: We designed NaA-loaded liposomes modified with sodium cholate (SC) and mannose (MAN) (NaA@SC/MAN-LPs) to target hepatocytes and Kupffer cells. Results: The N

Clinical significance of high-density lipoproteins and the development of atherosclerosis: focus on the role of the adenosine triphosphate-binding cassette protein A1 transporter.

Low levels of high-density lipoprotein (HDL) cholesterol constitute a risk factor for coronary artery disease, and there is evidence that increasing HDL cholesterol levels reduces cardiovascular risk. The phenotype of low HDL cholesterol with or without elevated triglycerides is at least as common in patients hospitalized for cardiovascular disease as is hypercholesterolemia, and it is characteristic of diabetes and the metabolic syndrome, conditions associated with increased cardiovascular risk. Recent studies have elucidated mechanisms by which HDL acts to reduce cardiovascular risk, bolster

Absorption of bile acids from the large bowel in man.

The absorption of bile acids from the human large bowel was studied in eight patients. All patients had cholecystitis and cholelithiasis and had to undergo cholecystectomy. Cholic acid-(14)C was injected during surgery into the lumen of the cecum, hepatic flexure of the colon, or transverse colon in six patients, under the visual control of the surgeon. Common duct bile was collected by T tube daily for 5 days, and bile acids were extracted. Significant amounts of radioactivity appeared in T tube bile in each patient. T tube bile acids contained a total of 43.6-84.6% of the administered radioa

Mechanistic insights into mixed bile salt-weak base disinfectants against wild-type and multidrug-resistant Staphylococcus aureus biofilms.

Staphylococcus aureus biofilms on stainless-steel surfaces show high tolerance to disinfectants, particularly in multidrug-resistant (MDR) strains, posing risks in food and pharmaceutical processing. Repurposing Generally Recognized as Safe (GRAS) compounds for low-dose biofilm control is therefore attractive. This study evaluated the antibacterial and antibiofilm efficacy and key mechanisms of mixed bile salt-weak base disinfectants against wild-type and MDR S. aureus biofilms. In brief, sodium cholate (CHO) and sodium chenodeoxycholate (CHE) were combined with ammonium hydroxide (AMH) or sod

The resilience of Salmonella to bile stress is impaired due to the reduced efflux pump activity mediated by the antioxidant enzyme YqhD.

UNLABELLED: Bile salts play a critical role in modulating the host gut. They possess antimicrobial properties wherein they disrupt the bacterial membrane and produce reactive oxygen species (ROS), causing DNA damage. Pathogens like Salmonella regulate their metabolic activity to counteract the effects of bile. This study investigates the role of YqhD, an aldehyde reductase, in Salmonella's bile salt susceptibility. Our findings reveal increased survival of the yqhD mutant in the in-vitro studies in LB media with bile, liver cell line HepG2 and C57BL/6 mice on treatment with 8% sodium cholate i

Integrated UHPLC-Q-exactive orbitrap HRMS and serum pharmacochemistry for the investigation of anti-hepatic fibrosis effect of Baoganning Decoction.

BACKGROUND: Early intervention in hepatic fibrosis (HF) is critical to reducing the risk of cirrhosis-related mortality and hepatocellular cancer. However, treating fibrosis has proven to be more challenging, with no approved anti-fibrotic therapies currently available for HF. Traditional Chinese medicines (TCMs) hold significant potential for the management of HF. PURPOSE: This study aims to propose a systematic approach for investigating the pharmacological basis of Baoganning (BGN) Decoction, providing empirical evidence to support future research on its targets and mechanisms of BGN. Investigating causal associations among gut microbiota, metabolites, and liver diseases: a Mendelian randomization study.

OBJECTIVE: There is some evidence for an association between gut microbiota and nonalcoholic fatty liver disease (NAFLD), alcoholic liver disease (ALD), and viral hepatitis, but no studies have explored their causal relationship. METHODS: Instrumental variables of the gut microbiota (N = 13266) and gut microbiota-derived metabolites (N = 7824) were acquired, and a Mendelian randomization study was performed to explore their influence on NAFLD (1483 European cases and 17,781 European controls), ALD (2513 European cases and 332,951 European controls), and viral hepatitis risk (1971 European cas

Transient increase in atherosclerotic plaque macrophage content following Streptococcus pneumoniae pneumonia in ApoE-deficient mice.

INTRODUCTION: Despite epidemiological associations between community acquired pneumonia (CAP) and myocardial infarction, mechanisms that modify cardiovascular disease during CAP are not well defined. In particular, largely due to a lack of relevant experimental models, the effect of pneumonia on atherosclerotic plaques is unclear. We describe the development of a murine model of the commonest cause of CAP, Streptococcus pneumoniae pneumonia, on a background of established atherosclerosis. We go on to use our model to investigate the effects of pneumococcal pneumonia on atherosclerosis. METHOD

Inhibition of neutrophil function by aspirin-like drugs (NSAIDS): requirement for assembly of heterotrimeric G proteins in bilayer phospholipid.

Non-steroidal anti-inflammatory drugs (NSAIDs) inhibit neutrophil functions via mechanisms that are independent of their effects on prostaglandin biosynthesis. We examined the effects of sodium salicylate and piroxicam on GTP/GDP exchange by a regulatory G protein (G alpha i). Plasma membrane and cytosol of human neutrophils were prepared by nitrogen cavitation and discontinuous sucrose density centrifugation. Salicylate (3 mM) and piroxicam (50 microM) reduced [35S]GTP gamma S binding to purified plasma membranes [65 +/- 3.7 and 75 +/- 5.3% (P < 0.003) of control, respectively]. Membrane-asso

Effect of ethynylestradiol on biliary excretion of bile acids, phosphatidylcolines, and cholesterol in the bile fistula rat.

The effects of ethynylestradiol on endogenous bile acids, their capacity to conjugate and excrete intravenously infused cholic acid, the concentrations of biliary cholesterol and lecithin, and the individual molecular species of phosphatidylcholine have been determined in male and female Sprague-Dawley rats. Endogenous biliary bile acids were analyzed by gas-liquid chromatography-mass spectrometry. Eleven bile acids were identified and several minor bile acids, primarily muricholates, could not be completely characterized. After 5 days of treatment with ethynylestradiol (1 mg/kg per day), the

MALDI-MSI reveals shared lipid signatures in sarcoid-like granulomas of mice and in a human case.

Sarcoidosis is a complex inflammatory disease of unknown cause, with diagnosis often complicated by a lack of definitive biomarkers. This study used matrix-assisted laser desorption ionization mass spectrometry imaging (MALDI-MSI) to spatially profile lipids in sarcoid-like granulomatous tissues from animal and human samples. ApoE-/- mice (n = 23) were fed a cholate-containing high-fat diet for 16 wk, inducing sarcoid-like granulomas. Granulomas were characterized through hematoxylin-eosin, Masson's trichrome, and immunofluorescence staining, whereas lipidomic profiling of mouse hearts (n = 10

An effective method for preventing cholestatic liver injury of Aucklandiae Radix and Vladimiriae Radix: Inflammation suppression and regulate the expression of bile acid receptors.

ETHNOPHARMACOLOGICAL RELEVANCE: Aucklandiae Radix (AR) and Vladimiriae Radix (VR) were used to treat gastrointestinal, liver and gallbladder diseases at practice. In most conditions, VR was used to be a substitute of AR or a local habit may attribute to the same main active ingredients Costunolide and Dehydrocostus lactone, which presented many similar pharmacological activities. However, other different lactone compounds in AR and VR also play a role in disease treatment, so the difference in therapeutic effects of AR and VR in related diseases needs to be further studied.
