Integrating soft skills into perfusion education: a necessity for modern clinical practice.

Soft, non-technical competencies are important in perfusion practice but are often inconsistently taught. Key domains include communication, teamwork, leadership, adaptability, emotional intelligence, conflict management, professionalism, and cultural awareness. Suggested methods are interprofessional simulation, structured debriefing with feedback, short courses on communication and conflict resolution, mentored rotations with behavioral checklists, and recognition of progress through competency milestones and continuing professional development. Clear learning objectives and simple, validate

Alopecia - New building blocks.

Hair loss is devastating to the patient, but the diagnosis and treatment of alopecia are also difficult for primary care providers and even dermatologists. Given recent advances in the field, there has never been a more pressing time to reevaluate and improve upon the significant clinical skills needed to accurately diagnose and treat our patients with hair loss. This CME activity on alopecia is designed to (1) educate dermatologists in a simple, succinct fashion on the identification of, and the distinction between, the diverse types of hair loss and (2) discuss how to manage them.

Evaluation of the position of the needle tip during thoracentesis: Experimental study.

INTRODUCTION: Thoracentesis is performed to both diagnose and/or treat pleural effusion, and several important complications of thoracentesis are occasionally observed. To assess precise thoracentesis procedures, we evaluated the position of the needle tip during thoracentesis by using a thoracentesis unit, comparing experienced and inexperienced groups. METHODS: Twenty eight physicians (19 board-certified pulmonologists as an experienced group and the remaining 9 as an inexperienced group) participated at Fukujuji Hospital in January 2021. All participants performed 2 punctures with a handma

Post-shift diagnostic insight in radiography: Incubation and the temporal extension of clinical reasoning.

OBJECTIVES: Diagnostic radiography requires sustained visual attention, rapid technical execution, and continuous evaluative judgement under conditions of high cognitive load. In practice, radiographers must integrate patient presentation, positioning, radiation safety, workflow demands, and image quality assessment in real time. Many radiographers also report that insights about positioning adequacy, subtle abnormalities, or possible oversight arise only after the clinical shift has ended. Although this experience is widely described informally, it has not been formally examined within the di

Self, Others and Context: Reframing Clinical Reasoning as a Socio-Cognitive Practice.

RATIONALE, AIMS AND OBJECTIVES: Clinical reasoning is often studied as if it occurred inside the clinician's head. Yet, in practice, clinicians think, decide and act through constant interaction with patients, colleagues, technologies, routines and local constraints. This article challenges predominantly individual and decontextualised accounts of clinical reasoning and reframes reasoning as a situated socio-cognitive practice. We aim to show how explicit and tacit knowledge interact with the social, material and organizational environments in which clinical practice unfolds. METHODS: We deve

Understanding context specificity: the effect of contextual factors on clinical reasoning.

Background Situated cognition theory argues that thinking is inextricably situated in a context. In clinical reasoning, this can lead to context specificity: a physician arriving at two different diagnoses for two patients with the same symptoms, findings, and diagnosis but different contextual factors (something beyond case content potentially influencing reasoning). This paper experimentally investigates the presence of and mechanisms behind context specificity by measuring differences in clinical reasoning performance in cases with and without contextual factors. Methods An experimental stu

Deconstructing Expert Cognitive Skills and Knowledge.

Training to achieve expert performance requires the trainees to have intense deliberate practice performing the physical and cognitive skills of experts. Designing effective training requires explicitly identifying what experts do and the reasons for their actions. This identification is deconstructing their expertise. We discuss methods for deconstructing expertise, particularly the cognitive skills of high performing endoscopists. Through structured interviews we determined what information they sought, and how they applied that information to make decisions. We particularly focused on key f

Factors Influencing Clinical Reasoning Among Clinical Nurses: An Integrative Review Using the Decision-Making Ecology Model.

BACKGROUND: Clinical reasoning (CR) is a complex, context-dependent process that is essential for safe nursing care. Existing reviews have primarily focused on nursing students rather than practicing clinical nurses. Rapid changes in patient acuity and the increasing integration of digital technologies into healthcare highlight the need for an updated synthesis that reflects contemporary clinical environments. AIMS: To identify and synthesize factors influencing CR among nurses in contemporary healthcare settings using the decision-making ecology (DME) model. DESIGN: Integrative review. MET

Practical methods to embed culture in dental education.

Background Culture is dynamic and multifaceted, influencing patients' beliefs, behaviours, and experiences of healthcare. In dentistry, a comprehensive understanding of culture, including social determinants, protected characteristics, and lived experience, is essential to avoid stereotyping and promote equitable care. The General Dental Council's Safe practitioner framework (SPF) integrates cultural competence across multiple domains of dental education.Aim To explore how cultural competence (or humility) can be embedded into the UK dental curriculum in alignment with the SPF, and to offer pr

Curriculum evolution and documentary evidence of competency alignment in a hospital-embedded undergraduate health services and management programme in China: a longitudinal case study.

BACKGROUND: Undergraduate Health Services and Management (HSM) programmes are expected to integrate management, medicine, public health, digital capability and workplace learning. Hospital-embedded delivery may provide access to authentic organizational settings and interdisciplinary expertise, but distributed teaching can fragment curriculum ownership, competency progression and assessment. This study examined longitudinal evolution and documentary evidence of competency alignment in a hospital-embedded undergraduate HSM programme in China. METHODS: A longitudinal document analysis and compe

Deconstructing Expert Technical Skill in Endoscopy: Principles, Methods, and Application.

This article highlights stepwise deconstruction within simulation-based mastery learning (SBML) as a framework for endoscopy training. Endoscopy requires technical precision, sound judgment, and nontechnical skills, but trainees often feel overwhelmed, and trainers may lack structured feedback tools. Deconstruction addresses this by breaking procedures into sequential, teachable steps that are observable, measurable, and defined by expert consensus. In esophagogastroduodenoscopy; for example, this includes scope neutrality, systematic navigation, and complete wall examination. Embedding decons

Redefined Core Competencies for Genetic Counselors in Canada to Reflect an Evolving Workforce.

The Canadian Association of Genetic Counsellors (CAGC) and the Canadian Board of Genetic Counselling (CBGC) undertook a joint review of the Practice-Based Competencies (PBCs) for Canadian genetic counselors in 2025 to ensure they reflect current practice expectations within an evolving genetic and genomic landscape. A working group of four certified genetic counselors conducted an evidence-informed revision process that incorporated national surveys of practicing genetic counselors, consultations with educational and diversity committees, and input from a competency framework expert. A Qualitative Textual Analysis of Feedback Comments in ePortfolios: Quality and Alignment with the CanMEDS Roles.

INTRODUCTION: Competency-based education requires high-quality feedback to guide students' acquisition of competencies. Sound assessment and feedback systems, such as ePortfolios, are needed to facilitate seeking and giving feedback during clinical placements. However, it is unclear whether the written feedback comments in ePortfolios are of high quality and aligned with the current competency focus. Therefore, this study investigates the quality of written feedback comments in ePortfolios of healthcare students, as well as how these feedback comments align with the CanMEDS roles. METHODS: A

Assessment Fit for (More Than One) Purpose? Reconciling the Dual Purposes of Assessments in Competency-Based Health Professions Education.

Assessments serve several purposes in competency-based medical education (CBME) and competency-based health professions education (CBHPE) programs. These purposes are commonly termed 'assessment for learning' and 'assessment of learning.' Historically, educational scholars have advised against using an assessment for multiple purposes, arguing that doing so undermines its primary purpose. However, at times, such an approach may be required from a pragmatic standpoint. In this Eye Opener, the authors consider the competing purposes of assessment in CBHPE programs from a practical lens. They pro

Systems Thinking: Identifying Leverage Points for Achieving a Continuum of Competency-Based Health Professions Education.

Competency-based education is widely endorsed in health professions education (CBHPE) as an outcomes-focused approach to ensuring that all graduates demonstrate the competencies needed to improve patient and population health. Despite adoption of outcomes frameworks, accreditation requirements, and CBHPE tactics, interventions remain siloed within programs and training levels rather than spanning the education to practice continuum. Consequently, learners and teachers experience fragmented educational journeys that can fall short of addressing each learner's needs for continuous learning and g

[When nursing expertise transforms vascular medicine care].

Vascular medicine is a relatively new field that became a cross-disciplinary specialty in 2017 with the creation of a specialized degree program. It addresses a wide spectrum of pathologies, ranging from arterial conditions to venous diseases, including microcirculation and certain rare diseases. It involves caring for acute or chronic patients-who are often in pain and anxious-with complex wounds and significant sensitivity in the affected areas of the body. In this context, nursing expertise plays a fundamental role in the quality and safety of care. Through their knowledge, observational sk

Protecting patients and learners: educational leadership on the fringes.

The development of high-quality clinicians takes place through immersion in clinical practice facilitated by educational supervisors with appropriate capabilities and organisational support to undertake the role. Learners give feedback on the quality of their learning experience on placement, which provides insights about the quality of clinical care received by patients. In this article, we explore the role of educational leaders in engaging with learner feedback about patient care, the challenges and tensions this generates, and the path through resolution. We use a lived example to showcase

Development of a training programme in individual case formulation skills and a scale for assessing its effectiveness.

BACKGROUND: Little is known about the skills involved in clinical formulation. The individual case formulation (ICF) approach, based on functional analysis, employs clinical descriptions that are theory-free and depicts formulations constructed according to a set of basic conventions. AIMS: We report a test of whether this method could be taught and if the quality of the resulting diagrams could be reliably rated. METHOD: Participants (n=40) participated in a training course in formulation. A draft rating scale was refined in the course of rating formulation diagrams and basic inter-rater re
