Surgical treatment of cloacal anomalies.

From 1989-1998 14 patients were treated with cloacal anomalies: 5 typical cloacas (PC), 5 posterior cloacas, and 4 cloacal exstrophies (CE); 12 underwent surgery. Four typical cloacas were resolved with posterior sagittal anorectovagino-urethroplasty (PSARVUP), whereas in the 5th total urogenital mobilization (TUM) was used. Three PCs were managed with transanorectal TUM and 2 with anterior TUM without opening the anal canal and rectum (without a protective colostomy). Two CEs were treated with atypical procedures. Two patients with CE died without surgery and 2 died after surgery due to compl

Lower limb movements and urologic function in fetuses with neural tube and other central nervous system defects.

Lower limb movements and urologic systems were evaluated by prenatal ultrasound in 120 fetuses with either neural tube defects or intracranial anomalies. Despite anticipated major lower extremity and bladder dysfunction, lower limb movements were seen in 100% of the fetuses with anencephaly and encephaloceles, 93% with isolated spina bifida, 60% with complex spina bifida and 90% with abnormal intracranial findings. In all the cases except those with cloacal exstrophy, fetal bladder and collecting systems appeared normal. Therefore, fetal lower limb movements and urinary tract integrity appear

[Diagnostic prenatal ultrasonography of malformations of the fetal anterior abdominal wall].

Abdominal wall defects include a broad spectrum of structural malformations with variable severity and prognosis. The purpose of prenatal ultrasound examination is to correctly diagnose and classify these malformations according to their localization (particularly their relation to the umbilical cord insertion), their contents, their size and associated malformations or karyotypic abnormalities. Based on this examination, two groups can be distinguished: gastroschisis or omphalocele (when the latter is isolated, in particular without karyotypic abnormalities) which can be surgically corrected

Exome Survey and Candidate Gene Re-Sequencing Identifies Novel Exstrophy Candidate Genes and Implicates LZTR1 in Disease Formation.

BACKGROUND: The bladder exstrophy-epispadias complex (BEEC) is a spectrum of congenital abnormalities that involves the abdominal wall, the bony pelvis, the urinary tract, the external genitalia, and, in severe cases, the gastrointestinal tract as well. METHODS: Herein, we performed an exome analysis of case-parent trios with cloacal exstrophy (CE), the most severe form of the BEEC. Furthermore, we surveyed the exome of a sib-pair presenting with classic bladder exstrophy (CBE) and epispadias (E) only. Moreover, we performed large-scale re-sequencing of CBE individuals for novel candidate gen

Fecal diversion in anorectal malformations: is there a place for ileostomy? The ARM-Net Consortium experience including a narrative literature review.

PURPOSE: While colostomy is the standard stoma in patients with anorectal malformations (ARM), indications for ileostomy remain poorly defined. Our aim was to evaluate indications and outcomes of ileostomy in patients with ARM. METHODS: A retrospective study was conducted selecting patients from ARM-Net Consortium registry who received ileostomy between 2007 and 2024. Additionally, a literature review was conducted to contextualize findings. RESULTS: Among 2,994 patients, 21 (0.7%) underwent ileostomy. Data was available for 15 patients. Ileostomy was used mainly in complex malformations (N =

From crisis to care: exploring the resilience of pediatric urologists in tackling complex urological challenges in a resource-limited country during volunteer campaigns: a qualitative study.

Surgeons operating in resource-limited settings encounter unique challenges due to the scarcity of materials and resources. Complex urological disorders (CUDs) such as bladder exstrophy, cloacal exstrophy, and posterior urethral valves, prevalent in these settings, often lead to varying surgical outcomes. This study aims to understand the experiences of surgeons treating pediatric patients with varying CUDs in a setting where resources are scarce through a qualitative phenomenological approach. We conducted six in-depth interviews with six pediatric urologists and surgeons who participated in

Robotic-Assisted Laparoscopic Approach to Removal of Müllerian Remnants.

BACKGROUND: Robotic-assisted laparoscopy (RAL) is an alternative to traditional laparoscopic surgery that might increase a surgeon's ability to offer minimally invasive removal of Müllerian remnants (MR) to patients with complex anatomy. We report on 2 cases of RAL resection of MR. CASES: RAL allowed for adequate resection of MR without complications in 2 cases. Case 1 was a 13 year-old female adolescent with VACTERL and uterine remnant close to the ureter of her ipsilateral single kidney. Case 2 was a 16 year-old female adolescent with cloacal exstrophy and omphalocele with remnant hindgut a

Diamniotic omphalopagus conjoined twins in a diamniotic pregnancy.

OBJECTIVE: To present antepartal diagnostic findings including 2D/3D ultrasonography in a rare case of conjoined twins with diamniotic placentation. METHODS: In ultrasonographic examination, a communicating structure containing solid and cystic components together with cord vessels was found between the fetuses. The long axes of the fetuses were not parallel and they moved independently. Bladder configurations were absent. A fetal membrane from the chorioidal plate was imaged. RESULTS: Postmortem pathological examination revealed that the twins were conjoined by way of fused cloacal exstroph

Will my Child Walk Funny? The Rotational Profile of Infants and Children With Classic Bladder Exstrophy.

BACKGROUND: Bladder exstrophy is a rare, complex disorder that involves the urinary, reproductive, gastrointestinal, and musculoskeletal systems and is marked by an incomplete lower abdominal wall, leaving the bladder exposed to the outside world. Skeletal findings in this condition include externally rotated innominate bones. While the resulting acetabular retroversion might be expected to cause out-toeing, there is no published data on the gait parameters or rotational profile of children with classic bladder exstrophy (CBE) in isolation. This study, therefore, characterizes the rotational p

Complex abdominal wall defects: appearances at prenatal imaging.

Abdominal wall defects are a complex group of anomalies, and many are incorrectly diagnosed. Evaluation of the defect relative to the umbilical cord insertion site is fundamentally important in differentiating among the various malformations. The two most common abdominal wall defects are gastroschisis, in which the defect is on the right side of the normally inserting cord and free-floating bowel loops are present, and omphalocele, in which the cord inserts on a membrane-covered midline defect. Omphalocele may also form a portion of a more complex defect that may remain undiagnosed without th

The exstrophy-epispadias complex.

Exstrophy-epispadias complex (EEC) represents a spectrum of genitourinary malformations ranging in severity from epispadias (E) to classical bladder exstrophy (CEB) and exstrophy of the cloaca (EC). Depending on severity, EEC may involve the urinary system, musculoskeletal system, pelvis, pelvic floor, abdominal wall, genitalia, and sometimes the spine and anus. Prevalence at birth for the whole spectrum is reported at 1/10,000, ranging from 1/30,000 for CEB to 1/200,000 for EC, with an overall greater proportion of affected males. EEC is characterized by a visible defect of the lower abdomina

Complex abdominal wall reconstruction combined with bladder closure in OEIS complex.

PURPOSE: Due to the large abdominal defect from the omphalocele and extreme pubic diastasis in cloacal exstrophy (CE), bioprosthetic material may be used to bridge this gap during abdominal closure in CE. This study examined presurgical factors associated with the use of bioprosthetic materials in CE closure and complications in these patients. METHODS: An institutional database of exstrophy-epispadias complex patients was reviewed for CE. Inclusion criteria included CE and primary closure performed at the host institution from 1998 to 2018. Data collection included demographics, presurgical
