Short chain fatty acids stimulate Cl secretion in rat descending colon in vitro via an enteric reflex involving mucosa and cholinergic nerves. We used the short circuit current as the measure of Cl secretion caused by Na propionate (NaP) (0.5 mM) in luminal bath fluid and studied the mechanism of the response. The NaP response was decreased 81% by atropine and 76% by lidocaine. It was unaffected by tetrodotoxin, omega-conotoxin or by tachyphylaxis to capsaicin, CGRP, substance P, histamine or PGE2. It was not reduced by inhibitors of 5-HT2 or 5HT3 receptors or by partial tachyphylaxis to 5-HT.

An unusual localization of localized colitis cystica profunda in a 31-year-old man is described. The patient presented as anal bleeding and a protruding mass at the descending colon; the mass was polypoid and was made up of papillary epithelial hyperplasia with downward herniation of glands into the submucosa. An 81-year-old woman presented with melena. Colonoscopy showed descending colon cancer, but the oral side of the tumor was excluded from observation because of easy bleeding related to the stenosis. CT colonography to evaluate the oral end of the intestine revealed 2 lesions in addition to the lesion observed on endoscopy. Before the surgery, the patient was suspected to have 3 synchronous primary descending colon cancers and underwent left hemicolectomy. Because CT colonography revealed synchronous multiple colon cancers in the patient, we performed surgery in one stage.
