Infection and clinical xenotransplantation: Guidance from the Infectious Disease Community of Practice of the American Society of Transplantation.

This guidance was developed to summarize current approaches to the potential transmission of swine-derived organisms to xenograft recipients, health care providers, or the public in clinical xenotransplantation. Limited specific data are available on the zoonotic potential of pig pathogens. It is anticipated that the risk of zoonotic infection in xenograft recipients will be determined by organisms present in source animals and relate to the nature and intensity of the immunosuppression used to maintain xenograft function. Based on experience in allotransplantation and with preclinical models,

Cytomegalovirus in solid organ transplant recipients-Guidelines of the American Society of Transplantation Infectious Diseases Community of Practice.

Cytomegalovirus (CMV) is one of the most common opportunistic infections that affect the outcome of solid organ transplantation. This updated guideline from the American Society of Transplantation Infectious Diseases Community of Practice provides evidence-based and expert recommendations for screening, diagnosis, prevention, and treatment of CMV in solid organ transplant recipients. CMV serology to detect immunoglobulin G remains as the standard method for pretransplant screening of donors and transplant candidates. Antiviral prophylaxis and preemptive therapy are the mainstays of CMV prevent

Using Qualitative Research Methods to Codesign a Virtual Community of Practice for Global Oncology Early-Career Investigators.

PURPOSE: Cancer incidence and mortality are projected to increase disproportionately in low- and middle-income countries (LMICs), highlighting the need to strengthen global cancer research capacity. Supporting early-career investigators (ECIs) is central to this effort. We conducted a qualitative study to identify barriers and facilitators to global oncology research careers among ECIs and to inform the codesign of a virtual community of practice (CoP). METHODS: We conducted semistructured in-depth interviews with ECIs participating in the US National Cancer Institute's Global Cancer Research

Community pharmacists' referrals to General Practice with suspected need of antibiotics: an Australian cross-sectional pilot study.

BACKGROUND: Interventions to reduce antibiotic use focus on general practitioners (GPs) and patient behaviour, not pharmacists, who may inadvertently drive antibiotic expectations by referrals to GPs. No data are available on pharmacist referrals for suspected antibiotic-requiring infections. We conducted a feasibility pilot to provide data for robust sample size calculation and identify areas for further exploration. METHOD: Pharmacists and GPs were recruited independently using convenience sampling. They completed prospective data collection on 20 consecutive minor ailment encounters and co

Decoding prescriptions: A closer look at common errors in clinical practice across community pharmacies of Northern Cyprus.

Prescription documentation is essential for patient safety and medication efficacy. However, incomplete or illegible prescriptions can lead to errors, including drug-drug interactions (DDIs). This study evaluated the completeness, clarity and quality of handwritten and electronic prescriptions from community pharmacies in Northern Cyprus, assessing compliance with WHO guidelines. A total of 5,246 prescriptions from 23 pharmacies were analyzed over nine months, with 4,856 meeting inclusion criteria. Of these, 77% were handwritten and 23% electronic. Illegibility was an issue in 23.1% of handwri

BACKGROUND: Point-of-care ultrasound (POCUS) is increasingly used in emergency and adult primary care, yet its integration into pediatric community pediatrics remains limited. Barriers include limited training opportunities, insufficient hands-on practice, and inconsistent access to ultrasound devices. This study evaluated self-reported POCUS use, confidence, perceived practice impact, and barriers 12 months after completion of a longitudinal POCUS training program tailored for community pediatricians. METHODS: This mixed-methods study included 65 community pediatricians who completed a two-d

Practice-based research examining effectiveness of exposure-based CBT for youth in a community mental health setting.

We examined the naturalistic effectiveness of exposure-based cognitive behavioral therapy (Ex-CBT) for pediatric anxiety and obsessive-compulsive disorder in a community mental health setting. We also characterized adaptations made to Ex-CBT and whether treatment factors varied by whether youth were Medicaid recipients or not. To do so, we conducted a three-year, retrospective chart review of consecutively treated youth in an Ex-CBT treatment center embedded in a community mental health setting (N = 94; 72.3 % Medicaid recipients, 68.1 % female). We abstracted baseline demographic and clinical

Frontline Management of Epithelial Ovarian Cancer-Combining Clinical Expertise with Community Practice Collaboration and Cutting-Edge Research.

Epithelial ovarian cancer (EOC) is the most common histology of ovarian cancer defined as epithelial cancer derived from the ovaries, fallopian tubes, or primary peritoneum. It is the fifth most common cause of cancer-related death in women in the United States. Because of a lack of effective screening and non-specific symptoms, EOC is typically diagnosed at an advanced stage (FIGO stage III or IV) and approximately one third of patients have malignant ascites at initial presentation. The treatment of ovarian cancer consists of a combination of cytoreductive surgery and systemic chemotherapy.

Survey of Patient Satisfaction With Ingenol Mebutate Gel Treatment for Actinic Keratosis From a Community Dermatology Practice.

Actinic keratosis (AK) is a condition that can present as a discrete lesion or as areas of confluent lesions. It is generally treated either with cryosurgery or, less frequently, with patient-applied topical medications or photodynamic therapy. Topical therapies can offer benefits over cryosurgery, especially in patients with significant actinic damage. The long durations of topical treatment required and the persistence of associated local skin reactions (LSRs) may deter some patients from adhering to topical therapies and thus affect clinical outcomes. This study describes patient satisfacti

A Review of Opioid Overdose Prevention and Naloxone Prescribing: Implications for Translating Community Programming Into Clinical Practice.

BACKGROUND: As physicians have increased opioid prescribing, overdose deaths from pharmaceutical opioids have substantially increased in the United States. Naloxone hydrochloride (naloxone), an opioid antagonist, is the standard of care for treatment of opioid induced respiratory depression. Since 1996, community-based programs have offered overdose prevention education and distributed naloxone for bystander administration to people who use opioids, particularly heroin. There is growing interest in translating overdose education and naloxone distribution (OEND) into conventional medical settin

Candida infections in solid organ transplantation: Guidelines from the American Society of Transplantation Infectious Diseases Community of Practice.

These updated guidelines from the American Society of Transplantation Infectious Diseases Community of Practice provide recommendations for the diagnosis and management of Candida infections in solid organ transplant recipients. Candida infections manifest primarily as candidemia and invasive candidiasis and cause considerable morbidity and mortality. Early diagnosis and initiation of treatment are necessary to reduce mortality. For both candidemia and invasive candidiasis, an echinocandin is recommended for initial therapy. However, early transition to oral therapy is encouraged when patients
