Summary of comments and recommendations from the CDC consultation on the HIV/AIDS Epidemic and prevention in the Hispanic/Latino community.

In April 2008, the U.S. Centers for Disease Control and Prevention (CDC) hosted a national consultation meeting of academic researchers, public health officials, service providers, and community leaders to examine the HIV/AIDS epidemic and prevention needs of Hispanics/Latinos in the United States and its territories. The consultation engaged key stakeholders to review available information on HIV-related behavioral research and prevention efforts, describe gaps in current HIV prevention programs and research on Hispanics/Latinos, and identify community and societal-level factors that can incr

Developing strategies to control schistosomiasis morbidity in nonenrolled school-age children: experience from Egypt.

Schistosomiasis is a major health problem in school-age children in much of the tropical world. They harbour the most intense infections for both Schistosoma mansoni and S. haematobium. In Egypt, the Ministry of Health and Population (MOHP) has implemented a successful school-based treatment programme in which children are screened and those found to be infected treated with praziquantel. High nonenrolment rates in some rural areas have a negative impact on the coverage of this programme and on its ability to reduce transmission in the community. The main aim of our study was to introduce and

Understanding the context for long-range planning in hospitals.

Using their own experience in an urban hospital of national reputation, the authors describe and illustrate some of the background issues for long-range planning decisions pointing out that managers must steer a clear course among a number of internal and external pressures. The expectations of trustees, philanthropists, reimbursing agencies, the university, medical staff, patients and the surrounding community may sometimes be at odds. Managers can use long-range planning as a tool to ease the hospital into changing modes of operation and to achieve, over a time, a smoother interaction among

Can a regional government's social inclusion initiative contribute to the quest for health equity?

Despite decades of concern about reducing health inequity, the Commission on the Social Determinants of Health (CSDH) painted a picture of persistent and, in some cases, increasing health inequity. It also made a call for increased evaluation of interventions that might reduce inequities. This paper describes such an intervention-the Social Inclusion Initiative (SII) of the South Australian Government-that was documented for the Social Exclusion Knowledge Network of the CSDH. This initiative is designed to increase social inclusion by addressing key determinants of health inequity-in the study

Design of a randomized controlled trial using a community-academic partnership to improve medication adherence: MI-CARE study protocol and implementation.

The prevalence of hypertension has been observed to be disproportionately higher among African American and other minoritized populations compared with White Americans. Persistent compounding factors such as sociocultural barriers and medication nonadherence among minoritized populations exacerbate hypertension and cardiovascular disease disparities. The "My Interprofessional Care team for Adherence and Research Engagement (MI-CARE)" intervention focused on collaborative, culturally responsive tailoring of healthcare provision to improve medication adherence. A social-ecological framework was

Promising Practices to Promote Snap at Schools: A Quantitative Analysis of Snap Outreach Efforts.

BACKGROUND: In 2023, 17.9% of US households with children experienced food insecurity. While the Supplemental Nutrition Assistance Program (SNAP) aims to reduce food insecurity, persistent under-enrollment, labeled the "SNAP Gap," limits its impact. Schools represent a strategic yet underutilized setting to reach eligible families. This study examined school-based strategies to increase caregivers' awareness and knowledge of SNAP. METHODS: Twelve diverse school districts implemented stigma-free SNAP outreach initiatives, reaching 8601 participants across 37 outreach events statewide. Caregive

Community-academic implementation science partnership to examine adoption and impact of a patient-centered approach to sexual history.

Progress toward ending the HIV epidemic has been slowed by suboptimal utilization of effective biomedical interventions (e.g., HIV testing, pre-exposure prophylaxis), especially for populations with the highest incidence. In 2021, the New York City Health Department initiated a multilevel implementation strategy, focused on promoting the GOALS Approach to Sexual History and Health-an antistigmatizing, client-centered strategy for sexual history taking-as a lever for increasing HIV intervention adoption, reach, and equity. An Interim Report on the Provision of Prenatal Care for Pregnant Mothers Experiencing Homelessness in Hawai'i.

The State of Hawai'i ranks third in the nation for homelessness. Homelessness disproportionately affects the health care of pregnant mothers and their children. These homeless persons are at risk for malnutrition, physical and psychological trauma, injuries and chronic illnesses, and have difficulty accessing healthcare and social services. With the generous support of a Waiwai Ola grant from AlohaCare, a non-profit health plan in Hawai'i, the Maternal-Fetal Medicine physicians at the University Health Partners of Hawai'i created a pilot program with a midwife and medical assistant to provide

Hospital decision-making regarding the smallpox pre-event vaccination program.

OBJECTIVES: To understand the factors underlying the decision by U.S. hospitals to participate or not in the U.S. Smallpox Pre-Event Vaccination Program (SPVP). METHODS: We conducted semistructured telephone interviews with a convenience sample of 123 hospital decision-makers in nine states between June and November 2003. RESULTS: Within our sample, 88 hospitals (72%) decided to participate in the SPVP and 35 (28%) decided against doing so. Nearly all hospital decision-makers considered the risk of a smallpox outbreak, risks associated with vaccination, hospital costs, and the reaction of ho

Delivering care to the streets.

St. Francis Hospital's management team has collaborated with other community organizations to respond to the very real needs of the homeless and indigent in Wilmington, DE. St. Francis Hospital has developed two innovative programs: St. Clare Medical Outreach and Tiny Steps. Recognizing that the poor and indigent find transportation, clinic schedules, and intimidating paperwork to be major barriers to healthcare, St. Francis Hospital and its partner in this collaborative ministry, Ministry of Caring, sought a creative approach that would bring healthcare to Wilmington's needy in a nonthreateni

Community partners as co-teachers in resident continuity clinics.

Standard approaches to teaching the management of psychosocial issues in pediatrics--visits to community-based organizations and stand-alone block rotations in developmental-behavioral pediatrics and community pediatrics--neither expose residents to models of interdisciplinary collaboration between faculty preceptors and community providers nor take advantage of the efficacy of learning in continuity clinics. The authors describe their project, developed from an existing Community Pediatrics Training Initiative with long-standing relationships with a domestic violence shelter, a community cent

REACH-Meharry community-campus partnership: developing culturally competent health care providers.

An important national health care effort is elimination of racial and ethnic disparities in six specific conditions: infant mortality, cancer screening and management, cardiovascular disease, diabetes, human immunodeficiency virus infection, and child and adult immunizations. To address this concern, several health entities in Nashville, Tennessee responded to a grant initiative from the Centers for Disease Control and Prevention to develop a Racial and Ethnic Approaches to Community Health (REACH) demonstration project. The resulting award is the Nashville REACH 2010 Project, charged to devel

Project həli?dx(w)/Healthy Hearts Across Generations: development and evaluation design of a tribally based cardiovascular disease prevention intervention for American Indian families.

American Indian and Alaska Native (AIAN) populations are disproportionately at risk for cardiovascular disease (CVD), diabetes, and obesity, compared with the general US population. This article describes the həli?dx(w)/Healthy Hearts Across Generations project, an AIAN-run, tribally based randomized controlled trial (January 2010-June 2012) designed to evaluate a culturally appropriate CVD risk prevention program for AI parents residing in the Pacific Northwest of the United States. At-risk AIAN adults (n = 135) were randomly assigned to either a CVD prevention intervention arm or a compariso

Identifying programmatic gaps: inequities in harm reduction service utilization among male and female drug users in Dar es Salaam, Tanzania.

INTRODUCTION: Current estimates suggest an HIV prevalence of 42% among people who inject drugs (PWIDs) in Dar es Salaam, while HIV prevalence is estimated to be 8.8% among the general population in the city. To address the HIV epidemic in this population, the government of Tanzania began establishing HIV prevention, treatment and care services including outreach and medication assisted treatment (MAT) for PWIDs in 2010. We assessed gender inequities in utilization of outreach and MAT services and evaluated differences in HIV risk behaviors between female and male PWIDs. MATERIALS AND METHODS:

Gendered Pathways to Missed and Zero-Dose Polio Vaccination in Children: A Narrative Review of Women's Autonomy, Household Decision-Making, Structural Barriers, and Health System Responsiveness in Afghanistan, Nigeria, Pakistan, and Sudan.

Background: Missed and zero-dose children remain major barriers to polio eradication in settings affected by insecurity, poverty, weak health systems, and social exclusion. This narrative review synthesized evidence on how gender norms and related social determinants influence childhood polio vaccination in Afghanistan, Nigeria, Pakistan, and Sudan. Afghanistan and Pakistan remain the only countries with endemic wild poliovirus transmission, while northern Nigeria, especially areas affected by insurgency, and war-affected Sudan continue to report substantial numbers of polio zero-dose and unde

Preparing for the real world. Program helps rehabilitation patients perform everyday tasks.

St. Francis Health Care Centre in Green Springs, OH, decided in 1991 to enlist local merchants in a program in rehabilitative medicine. The program, named for Green Springs's main street, is "Broadway: A Route Home." Broadway is a program for persons who, because of severe illness or injury, require extended rehabilitation. Along with care for continuing physical or cognitive problems, such patients often need help in performing tasks--buying groceries, cashing a check, renting a video--that most people take for granted. Under the Broadway program patients can practice these tasks safely in sh
