Do plain plastic and copper bearing intrauterine contraceptive devices have a central mechanism of action?

The notion that the intrauterine contraceptive device (IUCD) has a central, as well as a local action, is examined. Although the IUCD undoubtedly has a local action, certain IUCD related side effects, e.g. galactorrhoea, bloating and premenstrual syndrome and inadequate luteal function can be explained if the IUCD has some central action. The powerful postcoital anti-fertility effect of both copper-bearing and plain plastic IUCDs probably also depends to some extent on a central action. The central action of the IUCD is probably due to initiation of reflex hypothalamic activity following intra

Actinomyces israelii in the genital tract of women with and without intra-uterine contraceptive devices.

Actinomycosis involving the female genital tract is more common among IUD users than others. The diagnosis is difficult and often delayed. It has been suggested that the finding of Actinomyces-like organisms or A. israelii in cervical smears indicates a risk of developing actinomycosis. A. israelii has not been regarded as a part of the indigenous genital flora. A group of IUD users without symptoms of genital tract infections were compared with a control group without IUDs. No Actinomyces-like organisms were found on cytological examination of cervical smears. Immunofluorescent staining and c

The cells from intrauterine contraceptive devices: with special reference to the occurrence of mast cells.

A study of cytologic smears from 26 women using intrauterine contraceptive devices (IUDs) confirmed our previous findings for mast cells and further defined the averages, proportions and types of cells in IUD smears. The average number of cells was found to be 65.5 x 10(4), with a range of 1.3 x 10(4) to 280 x 10(4). The cellular proportions found were 49.0% macrophages, 42.7% polymorphonuclear leukocytes, 4.0% endometrial glandular cells, 1.3% mast cells and 3.0% other cells. Total nucleated-cell counts showed one group with a large number of cells (greater than 5 x 10(5) cells) and another g

Serum lipid variations in women on copper intrauterine devices, triphasic oral contraceptives and monthly injectable contraceptives.

This is a longitudinal, prospective, comparative controlled study. Patient selection excluded smokers, overweighed, hypertensive, and aged greater than 34 years. 168 women were distributed by random list into four groups: A) fitted with inert IUD (control); B) with Nova T (Cu-IUD); C) received triphasic gestodene (OC); and D) monthly norethisterone enanthate 50 mg + estradiol valerate 5 mg (injectable). Serum lipid patterns were assessed at the beginning of contraceptive use and at 1, 6 and 12 months. The results of the Cu-IUD group did not show any difference with the control group. The OC gr

From contraception to complication: Copper-T intrauterine contraceptive device migration leading to foreign body appendicitis.

Uterine perforation is one of the serious complications associated with the use of an intrauterine contraceptive device, and its migration can lead to complications involving neighbouring organs, including the appendix. We report an unusual case of a 22-year-old Pakistani woman, who presented to the emergency department with pain in the right iliac fossa and was diagnosed as a case of foreign body appendicitis resulting from migration of an intrauterine contraceptive device and entering the lumen of the appendix. This case adds to the limited literature, with only 17 previously documented case

Tubo-Ovarian Abscess With Bacteremia in a Patient With an Intrauterine Contraceptive Device.

A tubo-ovarian abscess (TOA) is a complication of pelvic inflammatory disease (PID) resulting from an ascending infection of the upper genital tract. It is characterized by an inflammatory mass involving the fallopian tube, ovary, and sometimes adjacent pelvic organs. It is diagnosed through clinical evaluation and imaging, and initially treated with broad-spectrum antibiotics. Despite conservative management, surgical intervention may be necessary if there is a leak or rupture of the TOA or if the patient experiences chronic pain. Rarely, a TOA can cause septicemia. Insertion of an IUD in a p

Estimating emergency contraception efficacy with levonorgestrel and copper intrauterine devices.

OBJECTIVES: This study aimed to determine observed vs expected pregnancy risk among levonorgestrel 52 mg and copper T380A intrauterine device (IUD) emergency contraception (EC) users. STUDY DESIGN: This is a secondary analysis of participants seeking EC randomly assigned to a levonorgestrel 52 mg or copper T380A IUD. Participants had at least one episode of unprotected intercourse (UPI) in the 5 days preceding enrollment and reported all episodes in the preceding 5 days. We report the proportion of pregnancies prevented of those expected at 1 month following IUD insertion using an established

Case report of an unusual finding of intrauterine contraceptive device in the rectum.

INTRODUCTION: Intrauterine contraceptive device (IUCD) is a safe and effective method of contraception. It is however rarely associated with complications. Migration of this device to the rectum is very rare. We report a case of IUCD migrating to the rectum with the history of missing IUCD strings. PRESENTATION OF CASE: A 32-year-old multipara presented 8 weeks following IUCD insertion with missing thread, ultrasound scan done showed a viable pregnancy with IUCD in-situ. Following vaginal examination, IUCD could not be retrieved. Pregnancy was allowed to continue for IUCD to be retrieved at d

Erosion of an intrauterine contraceptive device into the urinary bladder: A case report.

Intrauterine contraceptive devices may rarely erode into the urinary bladder, usually shortly after insertion. This case report describes the presentation and management of a copper-bearing intrauterine device which had eroded into the bladder. The patient presented with dysuria, dyspareunia and groin pain. The device had been inserted 10 years previously following a termination of pregnancy. A bladder stone had formed on the arm of the T-shaped device. The calculus was successfully lasered transurethrally and the intrauterine device was removed transvaginally. A urinary catheter was left on f

Autoimmune hemolytic anemia and thrombocytopenia attributed to an intrauterine contraceptive device.

BACKGROUND: Evans syndrome is a rare condition manifested by combined autoimmune hemolytic anemia (AIHA) and thrombocytopenia or neutropenia. It is often associated with other autoimmune disorders, immunodeficiencies, and non-Hodgkin's lymphoma. CASE REPORT: We describe a patient with Evans syndrome that may have been related to exposure to a polyethylene-based intrauterine contraceptive device (IUD). A 26-year-old white female presented with severe, symptomatic AIHA and subsequently developed severe thrombocytopenia. She had a refractory course resistant to multiple treatments including cort

Women's experience with postpartum intrauterine contraceptive device use in India.

BACKGROUND: Postpartum intrauterine contraceptive devices (PPIUCD) are increasingly included in many national postpartum family planning (PPFP) programs, but satisfaction of women who have adopted PPIUCD and complication rates need further characterization. Our specific aims were to describe women who accepted PPIUCD, their experience and satisfaction with their choice, and complication of expulsion or infection. METHODS: We studied 2,733 married women, aged 15-49 years, who received PPIUCD in sixteen health facilities, located in eight states and the national capital territory of India, at t

[Secondary psoas actinomycosis: a complication of an intra-uterine contraceptive device].

Genital-pelvic actinomycosis is an uncommon disease which often arises on women with long term use of intrauterine device. Its diagnostic remains difficult. Even if different clinical locations has been reported, location to the psoas muscle is exceptional. We report such a case, diagnosed on a 53 years old woman. Both a tumoral and a severe infectious syndrome appeared three months after an intra-uterine device removal. The septic syndrome led to a laparotomic approach. Unusual clinical presentation, delay between device removal and septic syndrome and lack of genital infection explain the is

Ease of intrauterine contraceptive device insertion in family planning settings.

BACKGROUND: Intrauterine devices (IUDs) provide highly effective contraception for women worldwide. Reluctance to insert IUDs in the primary care setting may relate to concern about potential difficulty and complications, particularly in nulliparous women. AIMS: To determine the practitioner, patient and procedural factors associated with abandoned IUD insertion, practitioner-reported difficulty of insertion and adverse events during IUD insertions in the family planning setting. METHODS: This was a prospective study over a 12-month period of consecutive IUD insertions in four family plannin

Translocation of an intrauterine contraceptive device: incidental finding in the rectosigmoid colon.

The presence of an intrauterine device (IUD) within the colon is rare. Complications have been reported with IUDs among which uterine perforation. Translocation of IUDs to the uterine cavity, to the bladder and also through the wall of the bowel, and sigmoid colon has been reported. We believe there may be a case that surgeons should know the result of despite being a priori gynaecological complication. This paper reports on a case of colon perforation by an IUD.
