Changing trends and outcomes associated with the adoption of minimally invasive hepatectomy: a contemporary single-institution experience with 400 consecutive resections.

BACKGROUND: Several studies published mainly from pioneers and early adopters have documented the evolution of minimally invasive hepatectomy (MIH). However, questions remain if these reported experiences are applicable and reproducible today. This study examines the changing trends, safety, and outcomes associated with the adoption of MIH based on a contemporary single-institution experience. METHODS: This is a retrospective review of 400 consecutive patients who underwent MIH between 2006 and 2017 of which 360 cases (90%) were performed since 2012. To determine the evolution of MIH, the stu

Minimally invasive management of pulmonary artery bleed in robotic left upper lobectomy.

While robotic-assisted thoracoscopic surgery offers some advantages over open surgery, the minimally invasive management of intraoperative bleeding has some limitations, which can necessitate open conversion. We present a case of minimally invasive management of a pulmonary artery bleed in a patient who underwent a robotic left upper lobectomy for a Pancoast tumour. An additional working port was created to allow placement of a vascular clamp for proximal control of the pulmonary artery, thereby improving visualization and primary suture repair. With appropriate experienced surgical assistance

[The ways for reduction of complication rate after laparoscopic cholecystectomy at acute cholecystitis].

Results of 1831 laparoscopic cholecystectomies (LCE) are analyzed. Symptoms of obstructive jaundice were at 170 (9.3%) patients. All these patients underwent endoscopic papillosphincterotomy before LCE for removal of bile hypertension. Conversion to open surgery was necessary in 44 (2.4%) cases. Intraoperative injuries of extrahepatic bile ducts occurred at 5 (0.27%), bleeding--at 10 (0.6%) patients. Postoperative complications were seen at 36 (2.0%) patients including intraabdominal bleeding (4), drainage bile less (8), subhepatic abscess (4), epigastric wound infection (16). There were 2 (0.

What are the contraindications for laparoscopic cholecystectomy?

Acute cholecystitis, morbid obesity, and previous upper abdominal surgery have been reported as relative contraindications to laparoscopic cholecystectomy. An analysis of 706 laparoscopic cholecystectomies performed at our institution was undertaken to determine if these relative contraindications led to increased morbidity, an increased rate of conversion to the open technique, or longer operating time. One hundred ninety-seven patients demonstrated one or more relative contraindications to laparoscopic cholecystectomy. Morbidity was not increased in patients with these risk factors, but conv

Laparoscopic or open treatment for hepatic alveolar echinococcosis: A single-institution experience.

OBJECTIVE: The aim of this study was to evaluate the safety and efficacy of laparoscopy in the treatment of hepatic alveolar echinococcosis (AE). METHODS: Between January 2018 and December 2019, 213 hepatic AE patients were admitted to the authors' institution. Among them, 165 patients (77.46%, 165/213) underwent surgery. Of 13 patients who underwent laparoscopic treatment, two required conversion to open surgery. The remaining 11 patients (group 1) were analyzed. During the same period, 154 patients underwent open surgery, but only 14 records were compatible with the criteria for the laparos

The effect of increasing body mass index on laparoscopic surgery for colon and rectal cancer.

AIM: Obesity is common in Western countries and its prevalence is increasing. Colorectal cancer is common, and surgery for colorectal cancer is technically more challenging in obese patients. Laparoscopic surgery for colon cancer has been shown to be oncologically equivalent, with improved short- term outcomes. Laparoscopic surgery for rectal cancer has proven technically challenging, and recent results have raised concerns about oncological equivalence. Our aim was to evaluate the effect of body mass index (BMI) on the clinical and oncological outcomes of surgery for colorectal cancer, includ

Disease complexity, not surgical approach, predicts outcomes in xanthogranulomatous pyelonephritis.

PURPOSE: Xanthogranulomatous pyelonephritis (XGP) is a destructive renal infection typically requiring nephrectomy. Although laparoscopy is increasingly feasible, morbidity remains high and predictors of adverse outcomes are not well defined. We aimed to explore predictors of postoperative complications and compare laparoscopic versus open approaches. MATERIALS AND METHODS: We retrospectively analyzed 42 XGP nephrectomies (2014-2024) at a tertiary referral center. Demographics, imaging features, intraoperative findings, and 30-day outcomes were collected. Complications were graded by Clavien-

Safety and Feasibility of Sleeve Gastrectomy Conversion to Roux-en-Y Gastric Bypass Versus Single Anastomosis Duodeno-Ileal Bypass: An MBSAQIP Analysis.

BACKGROUND: Sleeve gastrectomy (SG) has become the most common bariatric procedure worldwide. Unfortunately, a significant number of patients require conversion to Roux-en-Y gastric bypass (RYGB) or single-anastomosis duodeno-ileal bypass (SADI) because of recurrent weight gain. This study aimed to determine the rates of complications and mortality associated with conversion from SG to RYGB (SG-RYGB) compared with conversion from SG to SADI (SG-SADI). METHODS: This retrospective analysis used data from the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program database

Impact of previous abdominal surgery on minimally invasive radical resection of colorectal cancer: A meta-analysis.

The suitability of minimally invasive surgery (MIS) for colorectal cancer (CRC) in patients who have undergone previous abdominal surgery (PAS) is controversial. This study aimed to analyse the effect of PAS on MIS in patients with CRC. We performed a meta-analysis by searching the Web of Science, MEDLINE, Embase, and Cochrane Library databases from their inception until March 2024. All published studies exploring the effects of PAS on laparoscopic or robot-assisted radical resection for CRC were included. In total, 1271 studies were retrieved, of which 12 were included in the meta-analysis, i

Minimally invasive approach to peritonitis from left colonic perforation: a retrospective multicenter observational study.

BACKGROUND: Laparoscopic procedures have nowadays become the gold standard in multiple abdominal diseases, but in the emergency setting, especially in major surgery, laparoscopy still represents an obstacle for most surgeons. This multicentric study aims to define the surgical approach for emergency resective surgery in left colonic perforation peritonitis, determine the factors influencing the choice between MIS and open surgery, and identify factors associated with laparotomic conversion. METHODS: Retrospective data from 516 patients treated for left colonic perforation between January 2019

Minimally invasive approach in a rare emergency surgery, gallbladder perforation.

BACKGROUND: Gallbladder perforations are challenging to manage for surgeons due to their high morbidity and mortality, rarity, and surgical approach. Laparoscopic cholecystectomy (LC) is now included with open cholecystectomy in surgical managing gallbladder perforations. This study aimed to evaluate the factors affecting conversion from laparoscopic to open cholecystectomy in cases of type I gallbladder perforation according to the Modified Niemeier classification. METHODS: Patients who met the inclusion criteria were divided into two groups: LC and conversion to open cholecystectomy (COC).

Thoracoscopic Repair of Congenital Diaphragmatic Hernia After Extracorporeal Membrane Oxygenation: Feasibility and Outcomes.

INTRODUCTION: Thoracoscopic repair of congenital diaphragmatic hernia (CDH) has been associated with faster recovery, earlier extubation, and decreased morbidity. Nevertheless, thoracoscopic repair is rarely attempted in the post-extracorporeal membrane oxygenation (ECMO) patient. Commonly cited reasons for not attempting thoracoscopy include concerns that the patients' respiratory status is too tenuous to tolerate insufflation pressures or that presumed defect size is so large that it precludes thoracoscopic repair. Our purpose is to review our experience with post-ECMO thoracoscopic CDH repa
