Update on rupatadine in the management of allergic disorders.

In a review of rupatadine published in 2008, the primary focus was on its role as an antihistamine, with a thorough evaluation of its pharmacology and interaction with histamine H1 -receptors. At the time, however, evidence was already emerging of a broader mechanism of action for rupatadine involving other mediators implicated in the inflammatory cascade. Over the past few years, the role of platelet-activating factor (PAF) as a potent mediator involved in the hypersensitivity-type allergic reaction has gained greater recognition. Rupatadine has dual affinity for histamine H1 -receptors and P

Carcinoid syndrome: comparison of pretreatment regimes in the same patient.

Severe difficulties were encountered when a 47 year old man, with symptomatic carcinoid pretreated with ketanserin 80 mg twice daily was anaesthetised for hepatic arterial embolisation. The same patient, having had three days pretreatment with parachlorophenylalanine 500 mg four times daily and cyproheptadine 4 mg three times daily in addition to the ketanserin, was anaesthetised a week later without any difficulties. On the second occasion much better symptom control had been achieved. It is suggested that pretreatment with ketanserin alone is insufficient in severely symptomatic carcinoid an

A pharmacological analysis of levonantradol antinociception in mice.

Using the hot plate test, the potency and mechanism of the analgesic activity of levonantradol was studied in mice. Levonantradol is 10 to 30 times more potent than morphine; the antinociception can be only partially blocked by naloxone. Similar limited antagonism by cholinergics indicates possible opiodergic mechanism. The role of serotoninergic pathways is unclear; antinociception is partially blocked by 5,7-dihydroxytryptamine, unaffected by p-chlorophenylalanine, and potentiated by cyproheptadine. Levonantradol blocks naloxone-induced signs of abstinence in morphine-dependent mice, being 3

Crystal violet structural analogues identified by in silico drug repositioning present anti-Trypanosoma cruzi activity through inhibition of proline transporter TcAAAP069.

BACKGROUND: Crystal violet (CV) was used for several years in blood banks to eliminate the parasite Trypanosoma cruzi in endemic areas in order to prevent transfusion-transmitted Chagas disease. One mechanism of action described for CV involves inhibition of proline uptake. In T. cruzi, proline is essential for host cell infection and intracellular differentiation among other processes, and can be obtained through the proline permease TcAAAP069. METHODOLOGY/PRINCIPAL FINDINGS: CV inhibited proline transporter TcAAAP069 and parasites overexpressing this permease were 47-fold more sensitive to

Neurotransmission of the antidepressant-like effects of the growth hormone-releasing hormone antagonist MZ-4-71.

MZ-4-71 is an antagonist of growth hormone-releasing hormone (GH-RH) which suppresses the secretion of GH-RH. It has been shown that MZ-4-71 has antidepressive-like effects in a modified forced swimming test (FST) in mice, exerts anxiolytic effects in an elevated plus maze test, improves memory consolidation in passive avoidance learning, and corrects the impairment of memory consolidation caused by β-amyloid 25-35 in mice. However, little is known about the mechanisms of action of MZ-4-71 on brain functions. The involvement of the adrenergic, serotonergic, cholinergic, dopaminergic or GABA-er

Characterisation of headshaking syndrome--31 cases.

Headshaking is a maturity onset condition with the most commonly reported clinical signs being 'flipping' of the nose, nose rubbing, snorting or sneezing, and acting like a bee is flying up the nostril. A questionnaire was completed by owners of 31 horses with headshaking syndrome. The history, time of onset, clinical presentation and treatment of this condition were reported. Headshaking appeared to be light-stimulated in approximately 60% of the horses. The condition is seasonal and recurring in the majority of horses. Treatment with cyproheptadine produced improvement of symptoms in 76% of

Behavioral function of GABA in the median raphe nucleus.

Locomotor hyperactivity of rats was induced by injection of muscimol into the median raphe nucleus. Intra-raphe injection of picrotoxin or bicuculline failed to influence locomotor activity, however, muscimol-induced hyperactivity was inhibited by simultaneous intra-raphe injection of picrotoxin and muscimol. These data indicate an involvement of the GABAergic system in the median raphe nucleus in induction of locomotor effects. Systemic and intra-raphe injection of cyproheptadine depressed muscimol-induced hypermotility, whereas lesion of serotonergic raphe neurons by 5,7-DHT or intra-raphe i

The role of serotonergic and cholinergic systems in the aggression-stereotypy complex produced by amphetamine in mice.

The aggression--stereotypy complex (ASC) was produced in mice by administration of 15 mg/kg of amphetamine. The elevation of cerebral serotonin level in the mouse brain by administration of L-tryptophan or DL-5-hydroxytryptophan suppressed the aggressiveness without affecting significantly the stereotypy. Serotonin antagonists: methysergide and cyproheptadine, did not potentiate clearly the aggressive component of ASC. Cholinomimetics, pilocarpine and physostigmine, strongly suppressed the aggressive component, while a cholinolytic, scopolamine, evidently potentiated the elements of aggressive

Neurotransmissions of antidepressant-like effects of kisspeptin-13.

Kisspeptins are G protein-coupled receptor ligands originally identified as human metastasis suppressor gene products that have the ability to suppress melanoma and breast cancer metastasis and which have recently been found to play an important role in initiating the secretion of gonadotropin-releasing hormone at puberty. In the brain, the gene is transcribed within the hippocampal dentate gyrus. Kisspeptin-13, one of the endogenous isoforms, consists of 13 amino acids. In this work, antidepressant-like effects of kisspeptin-13 were studied and the potential involvement of the adrenergic, ser

Therapeutics for Equine Endocrine Disorders.

Equine endocrine disease is commonly encountered by equine practitioners. Pituitary pars intermedia dysfunction (PPID) and equine metabolic syndrome (EMS) predominate. The most logical therapeutic approach in PPID uses dopamine agonists; pergolide mesylate is the most common. Bromocryptine and cabergoline are alternative drugs with similar actions. Drugs from other classes have a poor evidence basis, although cyproheptadine and trilostane might be considered. EMS requires management changes as the primary approach; reasonable justification for use of drugs such as levothyroxine and metformin m

Participation of opiate delta-receptors in immunomodulation.

The activation of the opiate delta-receptors by the highly specific agonist DSLET evokes an immunoinhibitory effect in CBA mice which disappears with preliminary blockade of S2 serotonin receptors by cyproheptadine. The blockade of delta-receptors by the antagonist ICI 174,864 leads to stimulation of the immune response, which is tested on the basis of the number of rosette-forming cells. Neither stimulation nor suppression of immunogenesis are manifested in animals with a transsected hypophyseal stalk; this suggests the central action of the opioids. The increase in the immune response obtain

Binding of agonists and antagonists to mineralocorticoid receptors in human peripheral mononuclear leucocytes.

The binding of agonists (liquorice derivatives) and antagonists (spironolactones and cyproheptadine) to Type I aldosterone binding sites was evaluated in human mononuclear leucocytes and compared with data previously obtained using kidney cytosol or kidney slices from adrenalectomized rats. The affinity of spironolactones (spironolactone and canrenone) is equivalent in all the preparations. In contrast, carbenoxolone and glycyrrhizic acid show no affinity for aldosterone binding sites in mononuclear leucocytes, but bind to receptors in kidney cytosol. The possible explanation of the discrepanc

Centrally mediated drug-induced hyperglycemia in mice.

In mice, the hyperglycemic response to a stress, such as repeated orbital puncture, evoked a hyperglycemia through activation of the adrenal medulla and consequent release of catecholamines. This hyperglycemia could be blocked by pretreatment of the mice with the ganglionic blocking agent, chlorisondamine. The hyperglycemic response evoked by single doses of serotonin receptor blocking agents, such as cyproheptadine and methysergide, or by dopamine receptor blockings drugs, such as haloperidol or chlorpromazine, could also be blocked by pretreatment with chlorisondamine. Administration of a si

Serotonin Toxicity: Associated Agents and Clinical Characteristics.

BACKGROUND: Serotonin toxicity is a common cause of drug-induced altered mental status. However, data on the causes of serotonin toxicity, symptomatology, complications, and rate of antidotal treatment are limited. METHODS: This study evaluated cases of serotonin toxicity in the ToxIC registry, an international database of prospectively collected cases seen by medical toxicologists. Serotonin toxicity was diagnosed by bedside evaluation of medical toxicology specialists and explicit criteria were not used. The database was searched for "serotonin syndrome" between January 1, 2010, and Decembe

Animal models for the study of cancer-induced anorexia.

Walker carcinoma 256/B transplanted sc in CD-COBS rats induce a decrease of food intake when the tumor size is less than 5% of the body weight. This anorexia is accompanied by a decrease of the adipose tissue and, to a lesser extent, of muscular tissue. The mechanism involved in cancer-induced anorexia seems to be different from that of classic centrally acting anorectic agents. Among the drugs tested to counteract this anorexia only cyproheptadine shows a modest effect. Cyclophosphamide reduces tumor growth and prevents decrease in food intake. It is suggested that Walker carcinoma 256/B may

[Effect of serotonin and its antagonists on bladder detrusor function].

In anesthetized dogs, intravenous administration of serotonin causes biphasic contraction of the urinary bladder followed by relaxation. The m-antagonist of serotonin, morphine inhibits the first phase of contraction but does not affect the second phase and relaxation. The T-antagonist tipindole exerts no effect on the serotonin-induced contraction and relaxation of the detrusor. The D-antagonists, LSD-25, dihydroergotamine, droperidol and cyproheptadine produce no effect on the first phase of contraction but inhibit the second one and successive relaxation of the urinary bladder detrusor. Cold-induced urticaria and angioedema: diagnosis and management.

The physical urticarias compromise a heterogeneous group of disorders characterized by their propensity to form urticarial lesions after exposure to certain physical stimuli, such as heat, vibration, or cold. Although generally benign, systemic hypotension and angioedematous airway compromise are known life-threatening complications. We present the case of a man who experienced benign angioedematous changes in response to cold exposure, and discuss the clinical course, diagnosis, and management of cold urticaria.
