Arthroscopic cystectomy for popliteal cysts through the posteromedial cystic portal.

In the treatment of patients with popliteal cysts (Baker's cysts), we generally need to address the intra-articular pathologies, most commonly, medial meniscal tears and concomitant connecting valvular mechanisms responsible for the formation and recurrence of the cyst. We introduce here an arthroscopic technique that can treat the associated intra-articular pathology and correct the valvular mechanism of the capsular fold. The most important step is to locate the opening connection between the joint cavity and the popliteal cyst at the posteromedial compartment. The opening is found at the po

Patterns of initial transitional cell recurrence in patients after cystectomy.

PURPOSE: We characterized initial relapse patterns in patients who underwent cystectomy for transitional cell carcinoma of the bladder at our institution. MATERIALS AND METHODS: Between June 1995 and December 2001 a total of 417 patients underwent radical cystectomy for transitional cell carcinoma. Of the 399 patients treated with orthotopic diversion or incontinent cutaneous diversion, recurrent TCC developed in 86 (21.6%). We retrospectively examined this population to determine if any perioperative variables influenced the first site of disease recurrence. RESULTS: Of the 86 patients in w

Long-term results of orthotopic neobladder reconstruction after radical cystectomy.

BACKGROUND: Immunobullous diseases are uncommon in childhood. In contrast to adults, the most commonly seen is IgA-mediated chronic bullous disease of childhood (CBDC), while IgG-mediated bullous pemphigoid (BP), cicatricial pemphigoid (CP) and epidermolysis bullosa acquisita (EBA) are rare. We have demonstrated both IgG and IgA autoantibodies to basement membrane zone target antigens in eight children with 'mixed immunobullous disease of childhood'. OBJECTIVES: To elucidate whether a dual antibody response makes these patients distinct regarding their presentation, immunopathology, course an

Rotundovaginopexy in orthotopic ileal neobladder following radical cystectomy in women.

PURPOSE: The technique of rotundovaginopexy after cystectomy in females is described. MATERIALS AND METHODS: Four cases, two of them with over 12 months of follow-up are presented in whom this technique was used. RESULTS: Rotundovaginopexy stabilizes the vagina and thus prevents descent of the neobladder. Therefore the position of the neobladder is truly orthotopic. Thus continence and micturition are optimized. CONCLUSIONS: With this technique a truly orthotopic neobladder position can be achieved. The technique is simple to accomplish, even if hysterectomy has been performed long before.

Toxic shock syndrome after pilonidal cystectomy. Report of a case.

Toxic shock syndrome is an uncommon disease associated with staphylococcal infections. Although most frequently reported in menstruating women and associated with tampon use, toxic shock syndrome has been described following many types of surgical procedures. In this report we describe a case of toxic shock syndrome occurring in a previously healthy young man after elective surgery for a pilonidal cyst.

Salvage cystectomy for bladder and gynaecologic cancer after irradiation failure.

From November 1981 through 1983, salvage cystectomy was performed on five men with recurrent bladder carcinoma after definitive irradiation; three of the men were 75 years or older. There were no deaths, but the complication rate was fairly high, though acceptable. Salvage cystectomy was further performed, together with radical extirpation of reproductive organs, in two women with recurrent gynaecologic malignancy and a history of local irradiation. The authors conclude that salvage cystectomy is a fairly safe procedure, even in elderly patients, relieving discomfort and probably also prolongi

Recovery after cystectomy and urinary diversion: patient experiences and challenges.

PURPOSE: Radical cystectomy with urinary diversion is the standard treatment for muscle-invasive bladder cancer, yet patients often face substantial recovery challenges post-surgery. Previous research has identified common issues; however, the prevalence and predictors of these challenges remain underexplored. This study was aimed at assessing the frequency and nature of post-operative challenges experienced by bladder cancer patients and at identifying clinical and demographic factors associated with higher unmet needs. METHODS: Participants from three Kaiser Permanente regions who underwent

Uretero-enteric strictures after cystectomy: revealing the modifiable risk factors.

OBJECTIVES: Uretero-enteric strictures are feared complications following cystectomy. Despite surgical advancements, particularly the rise of robot-assisted approaches, the risk factors associated with these strictures remain poorly defined. This study aimed to identify the risk factors associated with uretero-enteric anastomotic strictures after cystectomy, according to the surgical approach and type of urinary diversion (extracorporeal vs. intracorporeal). METHODS: We conducted a single-center retrospective study including 340 patients who underwent cystectomy between 2016 and 2024 at Tours

Female Sexual Function After Radical Cystectomy: A Cross-sectional Study.

BACKGROUND AND OBJECTIVE: Radical cystectomy (RC) is the gold-standard treatment for muscle-invasive bladder cancer (MIBC). Approximately 25% of MIBC patients are women. In women, RC includes removal of the ovaries, uterus, and the anterior vaginal wall, during which nerve damage can occur, potentially impacting sexual function. Studies on sexual function among females following RC are sparse. Our aim was to investigate the impact of RC on female sexual function. METHODS: A cross-sectional registry study and a questionnaire survey were used. The Danish Cancer Registry was used to identify all

Detailed cadaveric analysis of perivesical lymph nodes in cystectomy specimens.

INTRODUCTION: Perivesical lymph nodes were added to the 8th edition of American Joint Committee on Cancer (AJCC) staging for bladder cancer. Currently, these nodes are inconsistently evaluated at the time of radical cystectomy. The objective of this study was to provide a detailed anatomic evaluation of perivesical lymph nodes. MATERIALS AND METHODS: A radical cystectomy was performed on six un-embalmed cadavers with wide resection of perivesical tissue and meticulous care to separate the pelvic sidewall lymph nodes (e.g. obturator, external iliac) from the bladder and perivesical en-bloc spe

Machine learning decision support model for radical cystectomy discharge planning.

PURPOSE: Timely and appropriate discharge placement for patients who have undergone radical cystectomy (RC) remains challenging. Our objective was to improve the discharge planning process by creating a machine learning model that helps to predict the need for non-home hospital discharge to a higher level of care. MATERIALS AND METHODS: Patients undergoing elective radical cystectomy for bladder cancer from 2014-2019 were identified in the ACS-NSQIP database. A gradient boosted decision tree was trained on selected predischarge variables to predict discharge location, dichotomized into home a

Bladder paraganglioma treated with open partial cystectomy: a case report.

BACKGROUND: Bladder paraganglioma is a neuroendocrine tumor that accounts for less than 0.1% of all bladder tumors. Symptoms caused by catecholamine release such as hypertension, palpitation, syncope, and macroscopic hematuria are the most common findings. Treatment modalities include transurethral resection, and partial or total cystectomy. CASE PRESENTATION: A 38-year-old Turkish female patient was examined for hematuria that had been persisting for 6 months. Among the clinical findings, only hematuria was present. Absence of adrenergic symptoms such as hypertension, palpitations, and synco

Laparoscopic partial cystectomy through extraperitoneal space for primary bladder schwannoma.

BACKGROUND: Schwannomas can occur in the body where nerve sheaths are present. Genitourinary schwannomas are very rare, especially primary bladder schwannomas. They account for only 0.1% of bladder tumours. The literature reports that simple surgical resection has a good effect and prognosis. CASE PRESENTATION: A 39-year-old man had no significant improvement in symptoms due to frequent urination and urgency for 1 month following the treatment of prostatitis for 2 weeks. Ultrasound and computed tomography (CT) showed a mass in the left side wall of the bladder (size approximately 2.0 × 1.9 cm
