Impact of Preoperative Lacrimal Sac Morphology on Postoperative Rhinostomy Shape and Prognosis in Endoscopic Dacryocystorhinostomy.

PURPOSE: To determine the association between the preoperative shape of the lacrimal sac and the postoperative shape of a rhinostomy in patients undergoing endoscopic dacryocystorhinostomy (DCR) for primary acquired nasolacrimal duct obstruction. MATERIAL AND METHODS: This retrospective study involved 180 cases among 129 patients. Preoperative dacryocystography (DCG) classified lacrimal sacs into constricted or cylindrical types based on their width relative to the nasolacrimal duct. Postoperative rhinostomy shapes were categorized into flat, ladle, and ice-scoop types according to the presen

DCR for nasolacrimal duct stenosis may be less effective than for complete obstruction.

OBJECTIVES: To ascertain the success of endo-DCR in nasolacrimal duct stenosis (NLDS) versus nasolacrimal duct obstruction (NLDO). METHODS: Consecutive adult patients with epiphora attending a tertiary lacrimal clinic from February 2012 to February 2021 were reviewed. NLDS was diagnosed by patent lacrimal syringing and combined dacryocystography (NLD stenosis) and dacryoscintigraphy (post-sac delay) findings in all eyes. Cases with evidence of canalicular stenosis or other identifiable causes of epiphora were excluded. The epiphora resolution and improvement rates following endo-DCR were comp

3D CT-DCG reconstruction-informed inferior turbinate dacryocystorhinostomy in post-traumatic chronic dacryocystitis with displaced lacrimal sac.

BACKGROUND: Conventional endoscopic dacryocystorhinostomy (DCR) relies on the middle turbinate as a landmark for localizing the lacrimal sac. However, in post-traumatic lacrimal sac displacement, the middle turbinate approach often fails, and blind exploration may cause excessive surgical trauma. OBJECTIVE: To evaluate the feasibility and effectiveness of a three-dimensional (3D) CT-dacryocystography (CT-DCG)-guided inferior turbinate approach, utilizing the distal nasolacrimal duct as an anatomical anchor for retrograde exploration of the displaced lacrimal sac in traumatic dacryocystitis.

Topographic evaluation of the medial canthus-alar groove line in terms of determining the boundaries of lateral osteotomies.

UNLABELLED: Nasolacrimal duct injury is a rare complication of rhinoplasty. Detailed regional anatomy knowledge is mandatory to avoid this complication. To display a reference line to protect the nasolacrimal duct from injury during lateral osteotomy, dacryocystography images of 10 patients who underwent rhinoplasty surgery at the authors' clinic were obtained. Lateral osteotomy lines and a line from the medial canthus to the alar groove were marked with radio-opaque wires before procedures. Horizontal and sagittal distances from these lines to the nasolacrimal duct were measured at the beginn

Beyond direct visualization: a comparative analysis of computed tomography dacryocystographic features in dacryolithiasis, lacrimal sac cysts, and primary acquired nasolacrimal duct obstruction.

BACKGROUND: Dacryoliths occur in approximately 5.8-18% of patients undergoing dacryocystorhinostomy (DCR), but conventional diagnostic methods often fail to distinguish them from other causes of nasolacrimal duct obstruction (NLDO). This study aimed to analyze the characteristic features of dacryoliths on computed tomography dacryocystography (CT-DCG) and to establish imaging criteria for their differential diagnosis from lacrimal sac cysts and primary acquired nasolacrimal duct obstruction (PANDO). METHODS: In this retrospective case-control study, CT-DCG images from 54 patients (18 with sur

Anatomical and Functional Alterations in Nasolacrimal Duct Obstruction: A Comprehensive Review.

The lacrimal drainage system, or LDS, is an intricate anatomical and functional system that mobilizes and drains tears from the ocular surface into the nasal cavity. The drainage system first begins at the puncta, small openings located at the medial eyelids which actively funnel tears into the canaliculi. The canaliculi drain into the lacrimal sac (LS), with the LS residing in the lacrimal fossa, which collects the tear fluid and subsequently drains the fluid into the nasolacrimal duct (NLD), which empties into the inferior meatus of the nasal cavity. Variations in canalicular length and duct

Artificial Intelligence for Lacrimal Drainage Disorders: Advances in Diagnosis, Management, and Clinical Translation.

PURPOSE: Lacrimal drainage disorders (LDDs) are common in oculoplastic practice, but their diagnostic evaluation continues to depend on tests that are variably subjective, operator-dependent, and at times invasive. In recent years, artificial intelligence (AI), spanning conventional machine learning (ML), deep learning (DL), radiomics, and large language models (LLMs), has shown increasing potential to enhance screening, diagnostic classification, lesion localization, and decision support in LDDs. This narrative review summarizes the current evidence on AI applications in the lacrimal field, p
