Research on shared decision-making (SDM) has increased, making it an essential clinical practice. Its application in surgery is a reality, despite the difficulties encountered in daily clinical practice, such as lack of time, training, and tools. Indeed, the use of patient decision aid tools (PtDA) improves communication, anxiety, decisional conflict, and patient satisfaction, in other words, quality of healthcare outcomes. Most PtDA tools have been developed in breast surgery and other surgical subspecialities such as colorectal cancer. Their customisation, through simple designs, accessible

Advanced chronic diseases, or multicomplexity in older adults presents unique challenges. Transthyretin cardiac amyloidosis (ATTR-CA) is one such scenario where heart failure is a common presentation, and management remains challenging. We describe the challenges involved in the diagnosis of ATTR-CA in older adults, which has implications on treatment options. We discuss the application of comprehensive geriatric assessment (CGA) and shared decision making (SDM) in the context of these challenges. Based on geriatric medicine principles, an innovative framework for applying SDM in ATTR-CA patie

Parents and doctors face difficult decisions when a baby is born at the borderline of viability. Somehow, they must voice their values, shared concerns, and disagreements in a way that allows an initial decision about life-sustaining treatment to be made quickly, under conditions of profound uncertainty and emotional stress. In the United States, the favored approach to such decisions has been labeled "shared decision making" (SDM).This article argues that prevailing models of SDM rely on several underexamined assumptions: that the facts relevant to decision-making-such as survival probabiliti

OBJECTIVES: Shared decision-making (SDM) is increasingly recommended for managing chronic musculoskeletal pain, yet its routine use remains limited. This scoping review aimed to identify and synthesise the barriers and facilitators to both implementing and using SDM across healthcare settings and levels of care.
METHODS: A comprehensive literature search was conducted in April 2025 across seven databases. Eligible peer-reviewed studies focused on factors influencing SDM in the context of chronic musculoskeletal pain. Two reviewers independently screened and extracted data. A deductive-inducti

This expert elicitation involving nine key opinion leaders from Malaysia, the Philippines, Singapore, and Thailand aimed to describe current gaps in post-operative pain (POP) and low back pain (LBP) management in Asia-Pacific (APAC) countries and provide recommendations for the adoption of shared decision-making in acute pain management. Gaps in acute pain management due to patient or healthcare professional (HCP) challenges were identified. Patient challenges include inadequate understanding of the consequences of poorly managed acute pain, the goals of pain management and how to manage acute

INTRODUCTION: Treatment of stable juvenile osteochondritis dissecans (OCD) of the knee in adolescents is controversial. Traditionally, initial management has been nonoperative. However, early subchondral drilling is also a consideration to potentially reduce the recovery time because ∼50% of stable OCD lesions eventually require surgery after a period of failed nonoperative care. This study uses choice-based conjoint (CBC) analysis to explore patient and family preferences regarding initial nonoperative treatment versus early drilling.
METHODS: This study used a CBC survey using Sawtooth Soft

OBJECTIVE: Shared decision-making (SDM) between parents facing extremely preterm delivery and the medical team is recommended to develop the best course of action for neonatal care. We aimed to describe the creation and testing of a literature-based checklist to assess SDM practices for consultation with parents facing extremely preterm delivery.
STUDY DESIGN: The checklist of SDM counseling behaviors was created after literature review and with expert consensus. Mock consultations with a standardized patient facing extremely preterm delivery were performed, video-recorded, and scored using t

BACKGROUND: "Shared decision-making" (SDM) is a cornerstone of prostate cancer (PCa) screening guidelines due to tradeoffs between clinical benefits and concerns for over-diagnosis and over-treatment. SDM requires effort by primary-care-providers (PCP) in an often busy clinical setting to understand patient preferences with the backdrop of patient risk factors. We hypothesized that SDM for PCa screening, given its prominence in guidelines and practical challenges, may be associated with quality preventative healthcare in terms of other appropriate cancer screening and encouragement of other pr

OBJECTIVE: The Management of Myelomeningocele Study demonstrated that fetal surgery, as compared to postnatal repair, decreases the rate of hydrocephalus and improves expected motor function. However, fetal surgery is associated with significant maternal and neonatal risks including uterine wall dehiscence, prematurity, and fetal or neonatal death. The goal of this study was to provide information about counseling expectant mothers regarding myelomeningocele in the era of fetal surgery.
METHODS: The authors conducted an extensive review of topics pertinent to counseling in the setting of myel

There is a growing body of literature that recognizes the importance of shared decision-making (SDM) in the care for children with chronic conditions and/or disabilities. Although participation in SDM can be more or less active, the tuning between parents and professionals about the way they want to participate in SDM is often an implicit process, limiting parents' optimal involvement. Role definitions may support both partners in the process of SDM. We conducted a scoping review to investigate the available knowledge on the interpretation and variability of different roles of parents and prof

RUNX1 familial platelet disorder (RUNX1-FPD) is associated with a 35-50% lifetime risk of hematologic malignancy (HM). Like all germline HM predisposition syndromes, RUNX1-FPD can only be cured with allogeneic hematopoietic stem cell transplantation (HSCT). Current genetic screening techniques allow for early detection of germline predisposition and, consequently, the opportunity for HSCT before overt development of HM (i.e., preemptive HSCT). However, there is as yet no consensus on the use of preemptive HSCT for RUNX1-FPD. Described here is the case of an individual with RUNX1-FPD and a fami

Lecanemab, the first anti-amyloid therapy approved by the European Medicines Agency, has demonstrated a statistically and clinically significant but moderate slowing of decline in early Alzheimer's disease (AD). In contrast, its long-term impact on the disease course is not formally established. Any benefit, therefore, needs to be carefully weighed against its adverse effects and practical constraints, which must be discussed with patients and caregivers within a shared decision-making process (SMD). However, applying SDM in this context is challenging due to treatment complexity, cognitive im

INTRODUCTION: Serious mental illnesses, such as schizophrenia, schizoaffective or bipolar disorder, often starts in late teens or twenties. Adherence to antipsychotic treatment for serious mental illness is often poor. Involving service users in decisions about their treatment leads to improved clinical outcomes and adherence. Shared decision-making is bilateral information sharing between clinician and service user where decision-making is shared between both parties. This study aimed to evaluate the lived experiences of young adults on how healthcare professionals involve them in decisions a

BACKGROUND: Patient and public involvement (PPI) is increasingly recognised as a valuable component of health research, yet patients are rarely included in data analysis within shared decision making (SDM) studies. We aimed to assess the feasibility and value of involving patient partner appraisers of SDM using the OPTION5 instrument. Specific objectives were to examine interrater reliability between patient partners and researchers, and to explore patient partners' perspectives on observed differences in interrater reliability.
METHODS: An explanatory sequential mixed-methods design was used

Shared treatment decision-making refers to an approach to care in which patients and their clinicians work in partnership to choose between treatment options, or delay/forgo treatment, concordant with patient values and considering the best available evidence. Research in various diseases, including cancer, has shown that patients who feel informed in treatment decisions have greater satisfaction with their medical care, better adherence to treatment, and better quality-of-life outcomes. The treatment landscape for advanced bladder cancer has evolved in recent years and several treatment optio

PURPOSE: Examine rural-urban disparities in prostate-specific antigen (PSA) testing and participation in shared decision-making (SDM) following the 2018 USPSTF guidelines.
METHODS: We analyzed data from the 2019, 2021, and 2023 cycles of the National Health Interview Survey including men ≥55 years, excluding those with a history of prostate cancer or missing data. We tested three outcomes: (1) ever received a PSA test; (2) past 12-month PSA testing; and (3) participation in SDM (assessed in 2019 only). The main exposure was rural-urban classification of respondents' residence.
RESULTS: There

BACKGROUND: African American men face significant disparities in prostate cancer outcomes, influenced by low trust in health care providers and disparities in treatment quality. Shared decision making (SDM) is recommended to improve engagement and outcomes within this population.
METHODS: Semistructured interviews were conducted with 15 African American men diagnosed with early-stage prostate cancer within the Veterans Administration health system in Louisiana. The Theoretical Domains Framework guided the interview guide development and qualitative data analysis.
RESULTS: Participants had a
