Dysbarism: An Overview of an Unusual Medical Emergency.

Dysbarism is a general term which includes the signs and symptoms that can manifest when the body is subject to an increase or a decrease in the atmospheric pressure which occurs either at a rate or duration exceeding the capacity of the body to adapt safely. In the following review, we take dysbarisms into account for our analysis. Starting from the underlying physical laws, we will deal with the pathologies that can develop in the most frequently affected areas of the body, as the atmospheric pressure varies when acclimatization fails. Manifestations of dysbarism range from itching and minor

Persistent (patent) foramen ovale (PFO): implications for safe diving.

Diving medicine is a peculiar specialty. There are physicians and scientists from a wide variety of disciplines with an interest in diving and who all practice 'diving medicine': the study of the complex whole-body physiological changes and interactions upon immersion and emersion. To understand these, the science of physics and molecular gas and fluid movements comes into play. The ultimate goal of practicing diving medicine is to preserve the diver's health, both during and after the dive. Good medicine starts with prevention. For most divers, underwater excursions are not a professional nec

Neurology and diving.

Diving exposes a person to the combined effects of increased ambient pressure and immersion. The reduction in pressure when surfacing can precipitate decompression sickness (DCS), caused by bubble formation within tissues due to inert gas supersaturation. Arterial gas embolism (AGE) can also occur due to pulmonary barotrauma as a result of breath holding during ascent or gas trapping due to disease, causing lung hyperexpansion, rupture and direct entry of alveolar gas into the blood. Bubble disease due to either DCS or AGE is collectively known as decompression illness. Tissue and intravascula

[Physiopathologic consequences of underwater diving and medical management of divers].

Underwater diving is a very closely medically managed activity. Performing it, the human organism is under the physical laws of pressure and following consequences. The expiratory flows are significatively reduced, enhancing the risk of alveolar hypoventilation at exertion, the central nervous system is the privileged target during inopportune tissue degassing related accidents (leaving 20% of sequellae), barotraumatic injuries threaten middle and inner ear or lung (pulmonary barotrauma is the most severe accident), the toxicity of gas under pressure (i.e. oxygen, nitrogen) exposes to specific

Implications of a patent foramen ovale for environmental physiology and pathophysiology: do we know the 'hole' story?

The foramen ovale is an essential component of the fetal circulation contributing to oxygenation and carbon dioxide elimination that remains patent under certain circumstances in ∼30% of the healthy adult population, without major negative sequelae in most. Adults with a patent foramen ovale (PFO) have a greater tendency to develop symptoms of acute mountain sickness and high-altitude pulmonary oedema upon ascent to high altitude, and PFO presence is associated with worse cardiopulmonary function in chronic mountain sickness. This increase in altitude illness prevalence may be related to dysre

Microparticles initiate decompression-induced neutrophil activation and subsequent vascular injuries.

Progressive elevations in circulating annexin V-coated microparticles (MPs) derived from leukocytes, erythrocytes, platelets, and endothelial cells are found in mice subjected to increasing decompression stresses. Individual MPs exhibit surface markers from multiple cells. MPs expressing platelet surface markers, in particular, interact with circulating neutrophils, causing them to degranulate and leading to further MP production. MPs can be lysed by incubation with polyethylene glycol (PEG) telomere B surfactant, and the number of circulating MPs is reduced by infusion of mice with PEG or ant

Internal carotid artery dissection in stroke from SCUBA diving: a case report.

Although diving with compressed air is generally safe, neurological problems resulting from infarction in SCUBA diving are well known, including arterial gas embolism and decompression sickness (caisson's disease, bends) involving the brain and spinal cord. While air gas embolism forms the overwhelming majority of causes for stroke in divers, internal carotid artery (ICA) dissection is another potential mechanism for central nervous system infarction in the setting of SCUBA diving. A 38 year-old female, who presented with complaints of headache, nausea, vomiting, and left sided hemiparesis aft

Infraorbital hypesthesia after maxillary sinus barotrauma.

We report a case of a diver who suffered an episode of maxillary sinus barotrauma that presented with decreased sensation over the cutaneous distribution of the infraorbital nerve after an ascent which produced facial pain and crepitus. This case illustrates a potential confusion between a decompression sickness etiology and a barotraumatic etiology for the observed sensory deficit. The clinical features of this case were most consistent with a barotraumatic etiology for the findings noted. The anatomy of the trigeminal nerve and previous reports of cranial nerve deficits following barotrauma

Hyperbaric therapy: report of its utilization at the Toronto General Hospital.

Hyperbaric oxygen therapy offers definitive treatment for decompression sickness, air embolism and carbon monoxide intoxication. It appears to be a very useful adjunct in the management of gas gangrene and may be of value in certain infections that have defied conventional therapy. Intermittent OHP seems to help maintain the viability of ischemic tissues where oxygen requirements are low and where the potential for circulatory restoration is present. Extended utilization in association with radiotherapy and cardiac surgery requires more specialized facilities or still awaits a sounder foundati

Diver with decompression injury, elevation of serum transaminase levels, and rhabdomyolysis.

A 43-year-old female recreational scuba diver presented to the emergency department 1 hour after a rapid, uncontrolled ascent. Her presentation included progressing confusion, slow and slurred speech, and complaints of headache and hypesthesia over her forearms and anterior thighs bilaterally. Differential diagnosis included arterial gas embolism and decompression sickness. She underwent recompression therapy with US Navy Table 6 within 120 minutes of her ascent. After recompression therapy, the patient had signs and symptoms consistent with severe rhabdomyolysis, including creatine kinase lev

Adjunctive hyperbaric oxygen therapy in the treatment of thermal burns: a review.

Hyperbaric oxygen therapy provided in specially designed pressure chambers is currently the treatment of choice for decompression sickness, arterial gas embolism, and serious carbon monoxide poisoning. It is an important adjunct in the treatment of gas gangrene, chronic osteomyelitis, radiation injury, and indolent wounds. The potential benefit in the treatment of thermal burns is not well appreciated. Hyperbaric oxygen therapy may significantly reduce morbidity, mortality, and cost of care. The application of this technology based on the demonstrated beneficial effects on the pathophysiology

Ostwald Ripening for Gas Bubbles and Decompression in Diving.

INTRODUCTION: The Ostwald ripening phenomenon for gas bubbles in a liquid consists mainly in gas transfer from smaller to larger bubbles. While the phenomenon itself is well known, the consequence of the temporal evolution of bubbles in the bloodstream in human physiology lacks deeper investigation. METHODS: In this work, a series of direct experiments was carried out in which the Ostwald ripening for air bubbles in a viscous fluid was reproduced. There, the time evolution of the bubbles' mean radius, number of bubbles, and radius size distribution was measured, where the initial bubble distr

Electric shock leading to acute lung injury in a scuba diver.

INTRODUCTION: Electrical injuries are a rarely reported complication of scuba diving. CASE REPORT: A 33-year-old woman wore a 12-volt heated shirt designed for motorcycling, powered by a canister light battery, while scuba diving. A leak in her drysuit allowed water to make contact with an electrified connector from the heated shirt, and she experienced painful electrical shocks. She was able to disconnect the power source and finish the dive, but she developed progressive fevers and dyspnoea several hours later. She was diagnosed with acute lung injury and treated with bronchodilators. Her s

Mitigating Risks of Altitude Chamber Training.

INTRODUCTION: Altitude chambers are used for training aircrews in a hypobaric hypoxic environment to better prepare them for pressurization and oxygen malfunction incidents during flights. However, adverse effects may occur during training sessions, with decompression sickness (DCS) being a major concern. The aim of this study was to examine the risks of different adverse effects during altitude chamber trainings (ACT) in the Israeli Air Force (IAF) facility and to compare them to other training facilities.METHODS: We retrospectively reviewed the records of 1627 individuals in the IAF who were

Dysbaric osteonecrosis in technical divers: The new 'at-risk' group?

INTRODUCTION: Dysbaric osteonecrosis (DON) in people working under increased atmospheric pressure is well documented. It is generally less common in military and commercial divers than in caisson workers, except in some high-risk groups, such as in many indigenous diving industries where workers have little or no understanding of decompression principles. With the increasing popularity within the recreational diving community of deep air and mixed-gas decompression diving ('technical diving'), it is likely that diving physicians may see an increase in the prevalence of DON in this group in the
