The impact of sedation depth on the occurrence of delirium and prognosis in intensive care unit patients: a meta-analysis.

BACKGROUND: Delirium and poor clinical prognoses are prevalent among ICU patients, particularly those requiring mechanical ventilation. While sedation is a cornerstone of intensive care, the impact of its depth remains controversial; deep sedation is often linked to adverse outcomes, whereas light sedation may improve recovery trajectories. We conducted a systematic review and meta-analysis to clarify the association between sedation depth and delirium incidence, mortality, and ICU length of stay (LOS). METHODS: Following PRISMA 2020 guidelines and PROSPERO registration (CRD420251054377), we

Does level of sedation impact detection of advanced neoplasia?

BACKGROUND AND AIMS: Two of the foremost issues in screening colonoscopy involve delivering quality and maximizing adenoma detection rates (ADR). Little is known about the impact of deep sedation on ADR. This study aims to compare the detection of advanced lesions during screening colonoscopy performed with moderate conscious sedation (MCS) versus deep sedation (DS). METHODS: A retrospective cohort study was performed using the Clinical Outcomes Research Initiative database. Average risk screening colonoscopies performed January 2000 to December 2005 were examined for practice setting, patien

A retrospective analysis of the effects of moderate sedation on the degree of cardia exposure in overweight elderly patients.

BACKGROUND: Gastric cancer is a type of malignant gastrointestinal tumor that poses a serious threat to human life and ranks as the second leading cause of cancer-related mortality worldwide. Although cardia-related diseases are rarely reported in the literature on gastrointestinal disorders, cardia lesions are not uncommon in clinical practice. The development of esophageal cancer may be associated with risk factors for esophageal adenocarcinoma, such as reflux esophagitis, high-fat diet, advanced age, and obesity. During gastroscopy, it is important to note that inadequate visualization of t

Association of nurse-led targeted sedation-analgesia management with ventilation duration and sedation quality in mechanically ventilated patients.

PURPOSE: Variability in sedation-analgesia assessment and titration is common in mechanically ventilated ICU patients and may contribute to unnecessary deep sedation and delayed ventilator liberation. This study examined the association between a nurse-led targeted sedation-analgesia management workflow and clinical outcomes in an Intensive Care Unit (ICU). METHODS: This single-center retrospective observational cohort study was conducted and reported in accordance with the STROBE statement. Adult patients receiving invasive mechanical ventilation for ≥24 hours in the ICU from January 1, 2024

Comparison of Continuous Sedatives in the Burn ICU on Delirium and Coma.

Lighter sedation targets over the past decade have resulted in improved outcomes for critically ill populations. Although guidelines exist for the general ICU population, these recommendations often exclude the burn population. The purpose of this study is to assess the impact of the initial continuous sedative on coma- and delirium-free days in critically ill patients with burns. This retrospective cohort study evaluated adult patients admitted to a burn intensive care unit at an academic medical center between January 2010 and September 2019. Patients were enrolled into 3 groups based on the

Correlation Between Body Movements and Salivary Secretion During Sedation.

During dental sedation, control of the cough reflex is crucial for a safe and smooth procedure. Accumulated saliva is one of the predisposing factors for coughing. Body movements during dental sedation appear to enhance salivation. Therefore, the aim of this study was to investigate the difference in salivary secretion between the with-movements state and the without-movements state during sedation. Salivary weight for 1 min was measured 3 times in 27 patients with intellectual disability during dental treatment under deep sedation with midazolam and propofol. The observed variables were body

Incidence of tolerance in children undergoing repeated administration of propofol for proton radiation therapy: a retrospective study.

BACKGROUND: Propofol is an excellent hypnotic drug for use in repeated radiation procedures in young children. To date, tolerance to propofol generally does not develop in pediatric patients undergoing radiation therapy. However, several studies have suggested that there may be potential for development of tolerance to propofol. The aim of this study was to evaluate the development of a tolerance to propofol used for repeated deep sedation in children undergoing proton radiation therapy (PRT). METHODS: All children undergoing PRT at our institution between December 2015 and January 2018 were

A study of the Bispectral Index Monitor during procedural sedation and analgesia in the emergency department.

STUDY OBJECTIVE: The Bispectral Index Monitor has been validated as an objective measure of sedation depth in the operating room; however, its value for states other than general anesthesia remains unclear. We hypothesized that bispectral index monitoring would reliably correlate with traditional definitions of sedation depth in emergency department patients undergoing procedural sedation and analgesia. METHODS: In this prospective observational study we measured the Bispectral Index score and a modified Ramsay Sedation Scale score every 5 minutes in a convenience sample of adult ED patients

A New Method for Comprehensive Analysis of Benzodiazepine, Opioid, and Propofol Interactions and Dose Selection Rationales in Gastrointestinal Endoscopy Sedation.

BACKGROUND: The aim of this study was to explore a new method for determining optimal dosing regimens for combinations of propofol, midazolam, and an opioid to achieve rapid on- and off-set of deep sedation. METHODS: We simulated 16 published dosing regimens using a well-validated pharmacodynamic model. The study was divided into 2 parts. First, the regimen that best provided deep sedation and rapid recovery was selected. A deep sedation-time area-under-the-curve (AUC) method was used to compare published dosing regimens; a higher AUC indicated better sedation and faster recovery. Second, sub

Correlation of Comfort Score and Narcotrend Index during Procedural Sedation with Midazolam and Propofol in Children.

Background/Objectives: Precise assessment of hypnotic depth in children during procedural sedation with preserved spontaneous breathing is challenging. The Narcotrendindex (NI) offers uninterrupted information by continuous electrocortical monitoring without the need to apply a stimulus with the risk of assessment-induced arousal. This study aimed to explore the correlation between NI and the Comfort Scale (CS) during procedural sedation with midazolam and propofol and to identify an NI target range for deep sedation. Methods: A prospective observational study was conducted on 176 children (6

The arousal level of consciousness required for working memory performance: An anaesthesia study.

Regarding the stage of arousal level required for working memory to function properly, limited studies have been conducted on changes in working memory performance when the arousal level of consciousness decreases. This study aimed to experimentally clarify the stages of consciousness necessary for optimal working memory function. In this experiment, the sedation levels were changed step-by-step using anaesthesia, and the performance accuracy during the execution of working memory was assessed using a dual-task paradigm. Participants were required to categorize and remember words in a specific
