Morel-Lavallée Lesions: A Diagnostic and Clinical Dilemma.

Morel-Lavallée lesions (MLL) occur following closed degloving injuries in trauma and appear as subcutaneous fluid collections. These lesions provide a diagnostic dilemma due to their resemblance to other subcutaneous lesions, including post-traumatic or post-procedural hematomas, and malignant entities such as soft tissue sarcomas. Magnetic resonance imaging (MRI) remains the diagnostic test of choice to diagnose MLL. Here we present a single case of MLL in a 40-year-old female who sustained an injury to her left thigh months earlier after falling through a pier as well as a review of literatu

Extensor tendon injuries. Presentation of a series of ninety-nine cases.

A series of 99 cases of injuries of the extensor system, followed in the 3 Traumatology and Orthopedics Ward (Dijon, France) is presented. Results were analyzed according to Verdan's classification. In most cases, a Jenning "barb-wire" type repair, introduced in France by Y. Allieu, was made. Early mobilization and skin tension problems are emphasized; two cases of degloving on the dorsum of the hand are presented with details. As well, a special place concerning the results of repair in the proximal interphalangeal joint must be reserved, because of the very frequent association of articular

Complex pelvic ring injuries associated with floating knee in a poly-trauma patient: A case report.

RATIONALE: Complex pelvic ring fracture associated with floating knee is comparatively rare which usually results from high-energy trauma including vehicle-related accidence, falls from height, and earthquake-related injury. To our knowledge, few literatures have documented such injuries in the individual patient. Management of both injuries present challenges for surgical management and postoperative care. The purpose of this study is to prove the feasibility and benefits of damage control orthopedics (DCO). PATIENT CONCERN: Our case involved a 45-year-old lady who was hit by a dilapidated b

A rare case of steering wheel injury causing coronal-plane pancreatic lacerations.

The incidence of pancreatic injury after blunt abdominal trauma is extremely low. A timely and accurate diagnosis is important, as a delay could be life-threatening. In this case, a 70-year-old driver crashed a car into a concrete wall at low speed. He was transported to the emergency hospital but died about 9.5h later with the cause of death unknown. An autopsy revealed that his pancreas was lacerated in the coronal plane and there was mesenteric contusion. Cause of death was determined to be blood loss resulting from pancreatic and mesenteric contusion. The mechanism of the injury was consid

From Oversight to Outcome: Addressing the Challenges of Foreign Body Management in Temporal Soft-Tissue Injuries.

This case report details the importance of thorough pre-operative assessment and management of soft tissue degloving injuries to prevent complications. A 55-year-old female experienced pain and bleeding in the right temporal region following a road traffic accident. After the initial suturing, she developed increasing pain, swelling, and purulent discharge, prompting a reassessment. Initially diagnosed with a soft tissue injury and no visible foreign bodies, a CT scan later revealed foreign objects in the temporal region as symptoms persisted. Emergency surgery was performed to remove the fore

Split-thickness skin excision in severe open fractures.

Split-thickness skin excision can be used as a one-stage procedure for the accurate diagnosis of flap viability and the immediate treatment of friction-avulsion injuries in severe open fractures. After cleaning the wound, the avulsed flap is temporarily sutured back to its original bed and a split thickness graft is taken from it and meshed to a 1:3 ratio. Surface dermal capillary bleeding then serves as an indicator of viability, clearly displaying a line for the excision of devascularized skin and correlating well with a concomitant fluorescein test. The wounds are re-opened and, after fixat

Morel-LAvallee lesion initially diagnosed as quadriceps contusion: ultrasound, MRI, and importance of early intervention.

Morel-Lavallee lesions (MLL) are rare, closed degloving injuries caused by trauma that delivers a shearing force to the soft tissue most commonly of the hip. If not treated in the acute and subacute setting these lesions are often complicated by re-accumulation of fluid, infection, or chronic pain. We present a unique case of a recurrent, massive medial knee/thigh MLL in which proper treatment was delayed due to initial diagnosis of a quadriceps contusion. We describe the ultrasound and magnetic resonance imaging findings of this patient and based on a review of recent literature propose that

Posttraumatic hip bulging mimicking an abscess in a patient with hip prosthesis: the role of ultrasonography.

A 48-yr old female patient, with stage 4 rheumatoid arthritis, who had undergone multiple joint prostheses including four arthroplasties of the left hip, presented for a sudden-onset large bulge on the left thigh, after a minor local injury. Orthopedic examination and radiography excluded fractures. Ultrasonography revealed a large mixed hypo- and hyperechoic collection, with no Doppler signal, but with comet tail artifacts. Repeated cultures from the collection were negative. Fluid analysis revealed increased quantities of titanium and cobalt. The sudden-onset deformity and fluid collection we

Outcomes of Coverage of Soft Tissue Defects in the Thumb with a "Kite Flap".

Deep soft tissue injuries and defects in the digits such as pulp amputation and degloving injuries are relatively frequent in hand surgical practice. Injuries around the thumb constitute a specific subtype, because their fitting requires the use of techniques that are different from standard. Many different surgical options have been described for the treatment of these serious lesions, depending on site and size of the defect, as well as on bone exposition. The "kite flap" is one of the most popular and frequently used. Results of the treatment of 14 of the 22 patients whose defects in the th

Dermal Matrices: Game Changers in Leg and Foot Soft Tissue Reconstruction? A Case Series.

The present article aims to present the use of dermal matrices in severe degloving, avulsion, and necrotizing injuries of the leg and foot in 3 patients. Conventional reconstruction would require the use of free flaps, since exposure of vessels, nerves, joints, and tendons rendered the mere resurfacing with skin grafts insufficient, and extensive cutaneous detachment precluded the use of local fasciocutaneous flaps. All injuries underwent thorough and repeated surgical debridements and wash outs, followed by negative pressure wound therapy (NPWT). Once negative tissue cultures were obtained, t

Toe transfer to the vessels of the reversed forearm flap.

A 23-year-old man had a severe degloving injury of his hand, amputation of multiple fingers, and devascularization of the thumb. Primary care consisted of successful revascularization of the thumb and skin grafting of the hand. Secondary soft tissue reconstruction of the hand was done with a reverse forearm flap. This was followed by second-toe transfer to the hand, with the recipient vessels, the radial artery, and the venae comitantes of the reverse forearm flap.

Traumatic abdominal aortic injury treated by endovascular stent placement.

Traumatic injury of the abdominal aorta is rare and potentially lethal. The authors present the case of a restrained passenger who was involved in a high-speed, head-on motor vehicle accident. On arrival in the emergency department, the patient complained of abdominal pain, was tachycardic and had a large ecchymoses on his right flank and lower abdominal wall. Computed tomography (CT) angiography (CTA) of the abdomen revealed a degloving injury to the abdominal wall, small bowel injury, and a distal abdominal aortic injury with a small amount of retroperitoneal hemorrhage. The patient underwen

Recurrent Post-traumatic Morel-Lavallée Lesion of the Hip and Thigh Treated With Open Capsulectomy and Povidone-Iodine Sclerotherapy: A Case Report.

Morel-Lavallée lesions (MLLs) are closed internal degloving injuries that may lead to persistent post-traumatic fluid collections. We report the case of a 65-year-old male patient with a recurrent MLL over the left hip and thigh following a fall. Initial management with aspiration and corticosteroid injection was unsuccessful, and open capsulectomy also failed to prevent recurrence. Due to the unavailability of conventional sclerosing agents, ultrasound-guided sclerotherapy using 10% povidone-iodine was performed. After two sessions, there was a marked reduction in the size of the lesion witho

Three-flap cover for total hand degloving.

Re-surfacing after a total hand degloving injury is one of the most difficult management problems in hand surgery. We present one such case that was managed using three flaps for cover. The radial forearm flap is very thin and pliable, resulting in satisfactory coverage of first web space and the thumb remnant, and facilitates a mobile thumb. The groin-hypogastric flap covered the remnant fingers admirably. The patient had a good first web and could use the hand to lift objects and hold a pen to write legibly within 2 months of the injury.
