The hypothetical role of congenital hypotonia in the development of early coronoid hyperplasia.

BACKGROUND: Coronoid hyperplasia (CH) is an abnormal bony elongation of a histologically normal coronoid process. Its definitive cause remains unknown. OBJECTIVES: To analyze the possible implication of congenital hypotonia in the pathogenesis of early coronoid overgrowth. PATIENTS AND METHODS: Two infants with congenital hypotonia were evaluated for limited mouth aperture. Bilateral CH was diagnosed. Transoral coronoidectomy was followed by an early dynamic physiotherapy program. RESULTS: Significant improvement of maximum interincisal opening was achieved. The review of the scientific lit

Swallowing function after occipitocervical arthrodesis for cervical deformity in patients with rheumatoid arthritis.

BACKGROUND: Some patients develop dysphagia after OC arthrodesis with RA. A previous report has indicated that establishing appropriate occipito-C2 is important for avoiding these side effects. However, a more recent report has demonstrated that the O-C2 angle did not have a significant effect on the incidence of postoperative dysphagia. OBJECTIVE: To investigate the swallowing function of patients with rheumatoid arthritis (RA) before and after they underwent occipitocervical (OC) fusion. METHODS: The study was performed in collaboration with the Departments of Orthopaedic, Otorhinolaryngol

[A device for tracheostomized children in home care].

Tracheomized children with deglutition disturbance require frequent suction--a procedure that may present problems for parents and other home-caregivers. We introduced a suction pump to drain the thoracic cavity continuously. This reduced the number of times of suction was required and raised no problems.

Individual design of the anterolateral thigh flap for functional reconstruction after hemiglossectomy: experience with 238 patients.

The aim of this study was to evaluate tongue function in patients with oral cancer treated surgically and reconstructed with anterolateral thigh free flaps (ALTFs). Patients (N=238) underwent primary reconstruction after hemiglossectomy between September 2012 and October 2014. Patients were divided into two groups according to the flap design: 'individual design' (ABC flap) and 'common design'. Patients were followed postoperatively and assessed after 6 months for the following functional outcomes: speech, deglutition, tongue mobility, and donor site morbidity. Intelligibility and deglutition

[Voluntary barium poisoning].

A 45-year-old man attempted to commit suicide by ingesting a large amount of barium. In some hours, he experienced generalized muscle weakness with hypokalaemia, treated by large dose of potassium (440 mmol in the first day). This weakness resulted in difficulties in swallowing and respiratory failure requiring mechanical ventilation. An anuric renal insufficiency started early, requiring haemodialysis for three weeks. It was induced probably by renal toxicity of barium and recovered completely. Later, the patient experienced an extrapyramidal syndrome initiated by tremor and myoclonia. Hypert

Foregut duplication cyst of the floor of mouth in a neonate: case report.

Congenital oral masses may interfere with vital functions such as respiration and deglutition in neonates. We report a congenital heterotopic oral gastrointestinal cyst that caused failure to thrive. A working knowledge of the differential diagnosis and pathophysiology of this can guide the clinician in its evaluation and management.

Primary tracheoesophageal shunt operation for postlaryngectomy voice restoration--a sphincter mechanism against aspiration.

Out of a total of 113 consecutive tracheoesophageal (TE) shunt operations for postlaryngectomy voice restoration in the past 8 years performed at the Department of Otorhinolaryngology, 92 patients (81%) succeeded in the postoperative TE speech. The essential part of this surgery consists of the construction of the TE shunt using the membraneous part of the trachea obtained at surgery. In the course of 8 years, important changes have been employed for the prevention of aspiration. We attempted to combine primary cancer surgery with the creation of intelligible voice, but without aspiration. For

Deformities of the uvula in the oral cavity- a case series.

Disorders of the palatal uvula is one of the least highlighted areas of medical literature, inspite the fact that uvula is a key organ in functions like speech, deglutition and mastication. The aim of this paper is to present a series of cases of wide range of uvular deformities ranging from bifid uvula to absence of uvula.

Response to multiple rapid swallows shows impaired inhibitory pathways in distal esophageal spasm patients with and without concomitant esophagogastric junction outflow obstruction.

Distal esophageal spasm (DES) is a motility disorder characterized by premature contraction of the esophageal body during single swallows. It is thought to be due to impairment of esophageal inhibitory pathways, but studies to support this are limited. The normal response to multiple rapid swallows (MRS) is deglutitive inhibition of the esophageal body during the MRS sequence. Our aim was to compare the response to MRS in DES patients and healthy control subjects. Response to MRS during HRM was evaluated in 19 DES patients (8 with and 11 without concomitant esophagogastric junction outflow obs

Sequence variability during hypopharyngeal bolus transit.

The pharyngeal phase of deglutition is considered to occur in a reflexive, preprogrammed fashion. Previous studies have determined a general sequence of events based on the mean timing of bolus transit and swallowing gestures. Individual variability has not been studied, however. The purpose of this study was to determine the amount of sequence variability that normally occurs during the hypopharyngeal phase of deglutition. Dynamic swallow studies from 60 normal volunteers were evaluated and event sequence variability was determined for 12 two-event sequences during swallowing of three bolus s

Electroacupuncture treatment for post-stroke oropharyngeal dysphagia: a randomized controlled trial.

INTRODUCTION: Dysphagia is a common complication after stroke and can negatively affect patients' quality of life. Despite electroacupuncture (EA) being widely used to treat dysphagia, its effectiveness and underlying mechanism of action have not been thoroughly examined. The aim of this study was to gather preliminary data on the efficacy of EA when used as an adjunct to a standardized but individually adjusted rehabilitation program for patients with post-stroke oropharyngeal dysphagia (PSOD) and to compare EA with adjunctive neuromuscular electrical stimulation (NMES). METHODS: Forty-five

Modified transcervical lipofilling of the base of the tongue under local anaesthesia: Case series.

Laryngeal carcinoma represents one-third of all head and neck cancers and is burdened by significant morbidity and mortality for advanced-stage disease. Surgical treatments, including Open Partial Horizontal Laryngectomy (OPHL), are often followed by long-lasting swallowing rehabilitation and more rarely, persistent dysphagia. Some authors reported single cases, successfully treated with fat injection of the base of tongue, but nowadays, a standardized technique has never been described so far. We provide a step-by-step technique description of the trans-cervical lipofilling of the base of the

[Esophageal transit and esophageal motility disorders in patients with nontoxic goiter and recurrent dysphagia].

Esophageal motor function was studied in 14 patients affected with diffuse and (multi)nodular nontoxic goiter, with dysphagia and neck discomfort, and in 10 age-matched controls without thyroid and/or gastroesophageal diseases. Esophageal manometry was employed to evaluate upper esophageal sphincter pressure (UESP) and lower esophageal sphincter pressure (LESP), amplitude, duration and propagation velocity of peristaltic contractions and the presence of simultaneous deglutitive pressure waves. Esophageal transit was evaluated by radioisotopical method, at different times, in proximal, middle a

Effects of intraluminal acidification on oesophageal motor activity.

This study assessed the effect of prolonged intraluminal acidification on the motor activity of the entire oesophageal body (under controlled conditions). Intraoesophageal pressures were recorded in 13 endoscopy negative subjects with gastro-oesophageal reflux disease in whom saline, HC1 0.1 N, and saline solutions were infused (1.5 ml/min) blindly in the oesophageal body, 6 cm distal to the upper oesophageal sphincter for three consecutive periods of 45 minutes each. These findings were compared with those of a control group. Intraoesophageal acidification caused an increase in the deglutitio
