Neonatal hypernatremia and dehydration in infants receiving inadequate breastfeeding.

INTRODUCTION: Neonatal hypernatermic dehydration (NHD) is a potentially very serious condition, which has been reported to occur in infants who have breast feeding problems in the first week of the life. This study looked at the incidence, risk factors, clinical symptoms and complications of NHD in healthy breastfed term neonates. METHODS: A prospective case-control study was conducted on 53 neonates with serum sodium concentrations of >or=150 mmol/L (as the case group) who were recruited between June 2006 and June 2007 from the Ghaem hospital (Mashhad, Iran) to investigate the relationship b

Dehydration and hypernatremia in breast-fed term healthy neonates.

OBJECTIVE: The aim of the study was to determine the incidence of significant weight loss, dehydration, hypernatremia and hyperbilirubinemia in exclusively breast-fed term healthy neonates and compare the incidence of these problems in the warm and cool months. METHODS: During the study period 496 neonates were recruited. RESULTS: 157 neonates (31.6%) had significant weight loss (> 10 % cumulative weight loss or per day weight loss > 5%). Clinical dehydration was present in 2.2% of neonates. Of these 157 neonates, 31.8% had hypernatremia and 28 % had hyperbilirubinemia. CONCLUSION: The inci

Diuretic-induced dehydration/hypovolemia inhibits thermal panting in rabbits.

Respiratory and thermoregulatory responses to hyperthermia during isosmotic dehydration/hypovolemia were studied in 17 anaesthetized adult rabbits divided into two groups: normovolemic group (NV; n=10) and hypovolemic group (HV; n=7). Hypovolemia/isosmotic dehydration (a decrease in plasma volume by 16.1+/-1.2%) was induced by furosemide (5 mg kg(-1) i.v.). During hyperthermia (the rise in body temperature to 42 degrees C by a gradual body surface heating), the HV rabbits had lower (P<0.05) respiratory frequency and higher (P<0.05) tidal volume than the NV animals. The panting was absent in th

Dehydration in black and white older adults using diuretics.

PURPOSE: To assess the association between dehydration and ethnicity in older adults; and to determine if diuretic use can help explain the disparity between blacks and whites in diagnosis of dehydration. METHODS: We conducted a case-control study in black and white older adults in a pharmaceutical assistance program who were hospitalized during 1997. Cases were all those diagnosed with dehydration (N=9186). Randomly selected controls were 4:1 frequency matched to cases by sex and age group (N(Total)=45585). RESULTS: Patients taking loop, potassium-sparing, thiazide or combination diuretics

[Digitalis intoxication during the neonatal period: role of dehydration].

BACKGROUND: Despite the great progress which has been made in the treatment of acute digitalis intoxication by digoxin-immune Fab, it still remains a severe complication of cardiotonic therapy. CASE REPORT: A neonate with ventricular septal defect and large left-to-right shunt was treated with digitalis and diuretics at the usual starting doses. An intensive phototherapy was also required because of a hyperbilirubinemia due to glucose-6-phosphate dehydrogenase deficiency. Toxic digoxin accumulation (plasma level 14 ng/mL) was diagnosed three days after the initiation of treatment by the prese

Diaphoresis and dehydration during neuroleptic malignant syndrome: preliminary findings.

Serum osmolality and its relationship to diaphoresis and polydipsic behavior were examined in a series of 38 episodes of neuroleptic malignant syndrome (NMS) in 29 patients. Clinical variables were associated with significantly higher serum osmolality, and diaphoresis tended to emerge earlier than polydipsia in the course of NMS. The findings of this study are consistent with the hypothesis that, at least in some NMS patients, diaphoresis leads to dehydration followed by physiologically appropriate thirst and increased oral water intake. It appears that intravenous hydration is often necessary

Early severe dehydration in young breast-fed newborn infants.

Six breast-fed infants living in a dry desert climate area presented at ages 4-11 days with severe dehydration. In all cases, dehydration was associated with inadequate breast milk production by the mothers. In contrast to earlier reports on dehydration in breast-fed infants, five of the mothers were from a low socioeconomic background and three were multiparas, including two mothers who had previous experience with breast-feeding. In two infants severe bacterial infections were documented. Two mothers had small retracted nipples. As in earlier reported instances, sodium concentration in breas

Opioid modulation of thermal dehydration-induced thirst in rats.

Male Sprague-Dawley rats were utilized to study the effects of the opioid receptor antagonists, naloxone and naltrexone, on thirst induced by thermal dehydration. In an initial experiment, the depressant effect of naloxone (1.0 mg/kg, IP) on the water intake of rats deprived of water for 24 h was confirmed. In subsequent experiments, rats were thermally dehydrated by exposing them without water to a 40 degrees C environment for 1-4 h. Following heat exposure, rats were injected with either naloxone or naltrexone either IP or ICV. Fifteen minutes later, rats were provided with water and water i

Cellular dehydration and immunoglobulin binding in senescent neonatal erythrocytes.

The life span of neonatal erythrocytes (60-80 days) is shorter than that of adult erythrocytes (120 days). We studied neonatal red blood cells separated on stractan density gradients to further characterize the aging process and to explore the possibility that senescence antigens play a role in the destruction of neonatal erythrocytes. Quantitation of membrane proteins 4.1a and 4.1b served as a marker for cell age and confirmed an enrichment for senescent red cells in the most dense layers of the gradients. Despite the shorter life span of neonatal erythrocytes, cord blood contained a larger p

Disappearance of the cartilage interlacunar network during histological dehydration.

The interlacunar network connects adjacent chondrocytes in rapidly growing cartilage and with anionic dyes stains more intensely than the surrounding matrix. The network is seen in fresh-frozen sections and sections treated with a variety of fixatives, dehydration agents and embedding media. The network is not seen after using such common dehydrating fluids as ethanol and acetone. Polyanionic glycosaminoglycans (GAG) are abundant in the cartilage matrix and it has been suggested that the network is an artifact caused by a rearrangement of the matrix GAG during histological processing. [Epidemiologic comments on hypertonic dehydration in Madrid (author's transl)].

The epidemiologic factors are studied in 584 cases of Hypertonic Dehydration (H.D.) admitted to the Children's Clinic "La Paz" over a period of 5 years. The overall monthly mean (MMT) resulted in 9.75 children a month with a standard deviation of +/- 2.1. The overall monthly evolution is analysed according to the sex, the seasonal incidence, as well as the more relevant etiologic factors: Concentrated diet, diarrhoea, vomiting, respiratory pathology and fever. The epidemiologic peculiarities are discussed according to age.

The circle of Willis revisited: Forebrain dehydration sensing facilitated by the anterior communicating artery: How hemodynamic properties facilitate more efficient dehydration sensing in amniotes.

We hypothesize that threat of dehydration provided selection pressure for the evolutionary emergence and persistence of the anterior communicating artery (ACoA - the inter-arterial connection that completes the Circle of Willis) in early amniotes. The ACoA is a hemodynamically insignificant artery, but, as we argue in this paper, its privileged position outside the blood-brain barrier gives it a crucial sensing function for the osmolarity of the blood against the background of the rest of the brain, which efficiently protects itself from dehydration. Till now, the questions of why the ACoA evo

Temporal dynamics of dehydration differentially regulate components of innate immunity.

Hydration state is an important regulator of vertebrate physiology, yet the temporal dynamics of hydration-associated immune responses remain poorly understood. We investigated how progressive dehydration and repeated dehydration-rehydration cycles influence innate immunity in Antaresia childreni (Children's pythons). Snakes were assigned to either a hydrated control group or a dehydration-treatment group, and plasma osmolality, agglutination, lytic ability, and bacterial killing ability were measured throughout two complementary experiments. Progressive dehydration produced stepwise increases

The relationship between oropharyngeal dysphagia and dehydration in older adults.

BACKGROUND: Relationship between dysphagia and dehydration has not been studied widely. The aim of this study is to determine the frequency of dysphagia and dehydration in geriatric outpatient clinic, to evaluate the relationship between these two conditions. METHODS: The cross-sectional study included 1345 patients. Plasma osmolarity (Posm) was calculated using the following formula: [1.86 x (Na + K) + 1.15 x glucose + urea + 14]. Overt dehydration was defined as a calculated Posm of > 300 mmol/L. Eating Assessment Tool (EAT-10) score of ≥ 3 was accepted as dysphagia. Associations between de

Prevalence of dehydration in older hospitalized patients with oropharyngeal dysphagia.

The diagnosis of dehydration among older patients with oropharyngeal dysphagia (OD) remains a clinical challenge. We aimed to assess the prevalence of dehydration in hospitalized older adults with OD and its relationship with other comorbidities. This is a cross-sectional study involving hospitalized older patients (≥ 70 years) with OD, assessed for OD using the Volume-Viscosity Swallowing Test. Treatment for impaired safety of swallow was based on xanthan gum thickened fluids with appropriate bolus viscosity (250 mPa·s or 800 mPa·s). Hydration status was evaluated using three different method
