Vital Pulp Therapy: Managing Deep Caries in the Permanent Dentition.

The adoption of vital pulp therapy (VPT) is a significant advancement in preserving the longevity of vital mature pulp. VPT represents a potential alternative approach to nonsurgical root canal therapy in which compromised pulp is treated such that it maintains its vitality and function. With the introduction of novel bioceramic materials, including calcium silicate cements and mineral trioxide aggregate, the prognosis for VPT in mature permanent teeth has greatly increased, and as a result, adaptation of VPT in these teeth has garnered considerable support. This article reviews evidence-based

In Vitro Evaluation of the Effect of Light-Cured Pulp-Capping Materials on Intrapulpal Temperature and Recovery Time During Sequential Light-Curing.

OBJECTIVE: Thermal insult is one of the factors contributing to pulpal damage during dental procedures, particularly in deep cavities with minimal dentine thickness. This study aimed to evaluate the effect of different light-cured pulp-capping materials on intrapulpal temperature (IPT) changes and thermal recovery time during sequential light curing in deep cavities. METHODS: Deep Class 1 cavities were prepared for 56 extracted premolars, maintaining a dentine thickness of 0.5 mm. Teeth were divided into 4 groups (n=14) based on the pulp-capping materials: TheraCal LC, Biner LC, izy INO LC an

Sympathetic nerves inhibit dental pulp stem cell differentiation and tertiary dentine formation via α1B-adrenergic receptor.

AIM: The dental pulp is extensively innervated. Our previous study showed that the α1B-adrenergic receptor ADRA1B is the primary sympathetic neurotransmitter receptor in human dental pulp stem cells (hDPSCs) and its expression was enhanced in inflamed pulp tissues. The sympathetic nerve system (SNS) negatively impacts the proliferation and migration capacities of hDPSCs via ADRA1B. In this study, we aimed to explore the role of the SNS in odontoblast differentiation of hDPSCs and tertiary dentine formation during dentine-pulp injury repair. METHODOLOGY: We investigated in vitro function of AD

Which procedures and materials could be applied for full pulpotomy in permanent mature teeth? A systematic review.

Objective: Coronal pulpotomies were recently re-investigated as an alternative to root canal treatment in vital permanent teeth. General dentists may be interested in knowing how to perform full pulpotomy, in particular in face of difficult endodontic cases of vital teeth. Material and methods: A systematic review was undertaken on the PubMed and Cochrane databases in order to determine which procedure should be applied for pulp capping and coronal restoration in routine dental practice. Fifty-three publications were included and allocated to one of two methodological categories: histological

[Decisive factors in the indication for pulp and peridontal treatment].

The explanation of the goal of pulp-periodontium treatment is followed by guidelines concerning the indication for the methods of pulp preservation (indirect capping, direct capping, pulpitis treatment by means of anaesthetics), of partial pulp preservation (vital amputation) and for pulpectomy (vital and non-vital techniques). The different passages of the paper terminate with conclusions for practice.

Is hard tissue formation in the dental pulp after the death of the primary odontoblasts a regenerative or a reparative process?

OBJECTIVES: Conceptually, two types of tertiary dentine may be produced in response to caries and environmental irritations: "reactionary dentine" that is secreted by existing primary odontoblasts and "reparative dentine", formed after the death of the odontoblasts by proliferation and differentiation of progenitor cells into odontoblast-like cells. Because histologic evidence for tubular dentine generated by newly differentiated odontoblast-like cells is lacking in human teeth, the present study examined pulpal cellular changes associated with caries/restorations, in the presence or absence o

Pulp survival and postoperative treatment needs following selective vs. total caries removal in mature permanent teeth with reversible pulpitis: A randomized clinical trial.

OBJECTIVES: To compare pulpal survival and treatment needs following selective and total caries removal in mature permanent teeth. METHODS: The design was a parallel double blind, randomized clinical trial. Mature teeth with caries radiographically extending ≥ 2/3 of dentine and without spontaneous pulpitis were included. Teeth were allocated to either selective (SCR) or total caries removal (TCR) using block randomization technique. In the SCR group, caries removal to firm dentine was followed by placement of Biodentine and composite restoration. In TCR group caries removal was to hard denti

Relative effectiveness of direct and indirect pulp capping in the primary dentition.

INTRODUCTION: Carious or traumatised teeth with a normal pulp status or with reversible pulpitis need an indirect or direct pulp capping procedure to keep the pulp vital. AIM: To evaluate the clinical outcome of both interventions for treating vital primary teeth. STUDY DESIGN: Two reviewers on Pubmed and ISI Web of Science performed a comprehensive literature review of publications from 1966 until December 2017. Among PubMed abstracts, publications were selected according to the following criteria: prospective clinical study, correct indication for the performed treatment, and clear definit

Evaluation of Histological Response of Human Pulp to MTA as DPC Agent With LLLT-Split Mouth Randomised Controlled Trial.

This study examined the histological response of human dental pulp to Mineral Trioxide Aggregate (MTA) as a pulp-capping agent, with or without Low Level Laser Therapy (LLLT). Thirty-two premolars from 16 patients were treated in a split-mouth design: one side received MTA alone, the other MTA with diode laser-assisted haemostasis and decontamination. After 6 weeks, the teeth were extracted for histological analysis. Results showed that the group with LLLT had a higher incidence of complete dentine bridges (87.5% vs. 68.8%) and thicker bridges (> 0.25 mm: 81.3% vs. 68.8%), though differences w

Successful Healing of Periapical Pathology with Partial Pulpotomy in a Mature Permanent Molar: A Case Report.

BACKGROUND Vital pulp therapy approaches are preferred over non-surgical root canal treatment, giving the advantage of preserving the vitality of the dental pulp and thus maintaining its benefits. Such approaches can be performed in teeth having normal apical area; however, performing vital pulp therapy approaches in teeth associated with periapical pathology remains controversial. CASE REPORT We present a case of a mature mandibular right first permanent molar tooth in a medically fit 10-year-old female diagnosed as asymptomatic irreversible pulpitis with asymptomatic apical periodontitis wit

Buonocore Memorial Lecture. Dentin caries: progression and clinical management.

A fundamental issue of restorative treatment is assessing the different conditions of cases not only from tooth-to-tooth but also from the activity of each caries lesion and the size of the cavity. In addition, restorative treatment is sometimes carried out for prosthetic and cosmetic purposes and involves cutting sound, unaffected dentin. These aspects are outlined in classic textbooks and are based on the principles described by Black (1908). However, each factor and its relative importance has changed over the years. Significant time has been devoted to improving the important technical asp

Evaluation of Pulp Repair after BiodentineTM Full Pulpotomy in a Rat Molar Model of Pulpitis.

Dental pulp is a dynamic tissue able to heal after injury under moderate inflammatory conditions. Our study aimed to evaluate pulp repair under inflammatory conditions in rats. For this purpose, we developed a rat model of controlled pulpitis followed by pulpotomy with a tricalcium silicate-based cement. Fifty-four cavities were prepared on the occlusal face of the maxillary upper first molar of 27 eight-week-old male rats. E. coli lipopolysaccharides at 10 mg/mL or phosphate-buffered saline PBS was injected after pulp injury. Non-inflamed molars were used as controls. Levels of inflammation-r
