Dermatitis artefacta is a psychosomatic disorder characterized by cutaneous lesions that are self-inflicted by the patient, apparently due to itchy sensation. Usually, the patient denies any direct involvement with this condition and is rarely conscious of the underlying psychosomatic etiology. Allergic contact dermatitis (ACD) is a common occupational disease. Hairdressers and beauticians are at increased risk of occupational chronic hand eczema. We present a case of mixed occupational, non-occupational and iatrogenic ACD in a hairdresser which illustrates that delayed diagnosis can result in high morbidity, and unnecessary treatment and cost. A hairdresser with chronic hand and facial eczema failed medical management with topical steroids and dupilumab. Patch testing revealed contact allergy to multiple occupational exposures, home exposures and topical medicaments.

The importance of occupational environments in relation to the course of an atopic skin disease was investigated. A series of 1008 adult patients, who had had an atopic disease since their teenage years, were studied. A comparison of the occupational dermatitis occurring in the same aircraft factory during 2 separate decades, 1955-1965 and 1981-1990, is presented. Subungual pulpitis is highly specific to this industry, because of the handling of resins and sealing agents. The number of cases dropped from 122 to 40, in accordance with progress in preventive medicine and technological changes in the factory. Irritant contact dermatitis nevertheless remained appreciable, while allergic contact dermatitis greatly decreased.

A 42-year-old woman had recurrent dermatitis on the hands, arms, face and trunk for 5 years. She was allergic to chrysanthemum, with positive patch tests. She also had contact urticaria to chrysanthemum, with a wheal-and-flare reaction 15 min after application of chrysanthemum juice to a scratch test site. This was not seen in control subjects. The co-existence of allergic contact dermatitis and contact urticaria from chrysanthemum might play a rôle in the development of dermatitis, especially severe chronic dermatitis of her hands.
