Simultaneous infection of abandoned leads and newly implanted leadless cardiac pacemaker: Why did this occur?

An 80-year-old man with a history of dilated hypertrophic cardiomyopathy received a dual-chamber pacemaker for sick sinus syndrome and atrioventricular block in February 2010. On May 30, 2019, he developed pocket erosion, with streaks of pus exuding from the pocket. The pacemaker generator was removed, although both capping leads were left buried under the skin, and a leadless pacemaker was implanted into the right ventricular (RV) apex the next day. Blood and pus cultures on July 15, 2019 indicated methicillin-resistant Staphylococcus aureus (MRSA). The patient was transferred to our hospital

Indications and outcomes of elective open chest lead extractions.

BACKGROUND: Complications associated with cardiovascular implantable electronic devices may necessitate device and lead removal. An open approach to removal may be electively chosen in cases with high risk of complications or those requiring additional concomitant cardiac surgery. This study aimed to investigate outcomes of patients who underwent elective open lead extractions (OLE) at two large tertiary care centers. METHODS: The records of 29 patients undergoing elective OLE were analyzed through retrospective chart review. RESULTS: 69 total leads were extracted from 29 patients (77% compl

Implantable cardioverter defibrillator lead-related methicillin resistant Staphylococcus aureus endocarditis: Importance of heightened awareness.

Methicillin resistant Staphylococcus aureus (MRSA) septicemia is associated with high morbidity and mortality especially in patients with immunosuppression, diabetes, renal disease and endocarditis. There has been an increase in implantation of cardiac implantable electronic devices (CIED) with more cases of device-lead associated endocarditis been seen. A high index of suspicion is required to ensure patient outcomes are optimized. The excimer laser has been very efficient in helping to ensure successful lead extractions in patients with CIED infections. We present an unusual case report and

Contribution of biomaterials analysis to the study of extractables from neonatal central venous catheters.

The study of extractable chemicals is used to evaluate whether a medical device contains harmful substances susceptible to migrate and contaminate the patients they are used in. The analysis of the biomaterial itself after extraction is often left aside. The aim of this study is to characterise and investigate whether any changes occur during the extraction process. Two biomaterials (thermoplastic polyurethane (TPU) and silicone from intravenous catheters) were studied after extraction, using acetone and hexane. To study the surface, we used scanning electron microscopy (morphological aspect),

Impact of patent foramen ovale on transvenous lead extraction characteristics and outcomes.

BACKGROUND: Transvenous lead extraction (TLE) may be indicated for cardiac implantable electronic device infection, malfunction, or upgrade. TLE can increase right heart pressures and mobilize debris, resulting in a right-to-left embolism and stroke via a patent foramen ovale (PFO). OBJECTIVE: This study aimed to identify the prevalence of PFO in patients undergoing TLE and compare the incidence of adverse events. METHODS: Intraoperative transesophageal echocardiogram is routinely performed during TLE at the University of California, San Diego, to identify PFO. Between June 2020 and October

One-Year Follow-Up of Patients Undergoing Transvenous Extraction of Pacemaker and Defibrillator Leads.

INTRODUCTION: The number of pacemaker and ICD implantations has increased substantially in the recent years. Therefore, complications are also observed in a greater number. In many cases, transvenous extraction of the previously implanted device (pacemaker or ICD) is the only solution. One may find in the literature information about the efficacy and safety of that procedure, but data concerning the results of long-term follow up are still limited. AIM: The aim of the study was to assess the one-year mortality in the cohort of patients undergoing transvenous lead extraction procedures in our

A Patient-Specific 3D Printed Lingual Nerve Protector in Third Molar Extraction.

We present a novel, patient-specific 3D-printed lingual nerve protector designed to minimize the risk of nerve injury during mandibular third molar extraction, a common cause of lingual nerve damage. Lingual nerve injuries, often resulting from trauma, dental procedures, or maxillofacial surgeries, lead to significant functional impairments. Using computed tomography (CT) data, the custom protector is fabricated through 3D printing with a self-retaining structure to shield the lingual mucosa and nerve. The device effectively separates the lingual nerve from the surgical field without requiring

Intra-aortic balloon pump rupture and entrapment.

Intra-aortic balloon pump is used frequently to support a failing myocardium in cardiac patients. Due to the invasive nature of this device, usage is accompanied by consistent risk of complications. Balloon rupture, although it occurs rarely, may lead to entrapment if diagnosis delays. A 78-year male who underwent cardiac surgery experienced balloon rupture and entrapment in the right femoral artery during the postoperative follow-up. Surgical extraction under local anesthesia was performed and the patient had an uneventful course. Fast and gentle solution of the problem is necessary to preven

Investigation of Sr-doped ZnO electron transport layers for perovskite solar cells: a combined DFT and SCAPS-1D study.

This study investigates strontium (Sr)-doped zinc oxide (ZnO) as an electron transport layer (ETL) for lead-free perovskite solar cells (PSCs) through a combined density functional theory (DFT) and SCAPS-1D simulation approach. The opto-electronics properties of pristine and Sr-doped ZnO were analysed using the mGGA functional implemented in CASTEP. The results revealed that Sr doping tunes the electronic structures of ZnO by reducing the band gap from 3.37 eV to 2.91 eV, while improving carrier mobility and optical absorption. Density of states (DOS) analysis confirmed that Sr doping enhances

A wandering defibrillator lead.

A 53-year-old woman was admitted because of sudden onset of severe chest pain and palpitations. Her medical history revealed an out-of-hospital cardiac arrest due to ventricular tachycardia caused by arrhythmogenic right ventricular cardiomyopathy for which an implantable cardioverter-defibrillator was implanted with epicardial patches. On 2D echocardiography, a mobile piece of lead was seen in the right atrium and right ventricle, loosely attached to the free wall of the right ventricule; 3D reconstruction confirmed this suggestion. Remarkably, the lead disappeared while performing the echoca

Transvenous Lead Extraction Outcomes in Hemodialysis Patients: Markers for Increased Mortality.

BACKGROUND: The increasing use of cardiac implantable electronic devices (CIEDs) has led to a progressive increase in transvenous lead extraction (TLE) procedures. Due to immune compromise and systemic dysfunction, hemodialysis (HD) patients face higher risks of device-related infection and need for TLE. OBJECTIVES: This study evaluates TLE outcomes in dialysis patients and identifies predictors of 30-day all-cause mortality (ACM). Subgroup analysis focuses on dialysis patients undergoing TLE for infectious indications to determine predictors of adverse outcomes. METHODS: A retrospective rev

Association of frailty with outcomes of patients undergoing transvenous lead removal for cardiac implantable electronic device infections in the United States.

BACKGROUND: Frailty, marked by reduced physiological resilience, is associated with adverse outcomes in patients undergoing various cardiovascular procedures. The rising incidence of cardiac implantable electronic device (CIED) infections necessitate transvenous lead removal (TLR), yet the impact of frailty on TLR outcomes remains unclear. OBJECTIVE: The purpose of this study was to evaluate the association between frailty and outcomes in patients undergoing TLR for CIED infections. METHODS: This retrospective study analyzed the National Readmissions Database (2016-2021) for patients hospita

Vacuum-Assisted Percutaneous Management of Cardiac Implantable Electronic Device Lead Endocarditis.

Background and Clinical Significance: Infective endocarditis is a disease of the endocardial surface of the heart, most often affecting heart valves (native or prosthetic) or intracardiac device. Although relatively rare, it carries high embolic risk of complications and mortality. Complete device extraction is recommended; however, conventional surgery may be prohibitive in frail patients with multiple comorbidities. Case presentation: We present a case of a 74-year-old male with implantable cardioverter-defibrillator (ICD)-related infective endocarditis and large lead-associated vegetation m

Incidence and Clinical Predictors of Early and Late Complications of Implantable Cardioverter-Defibrillators in Adults With Congenital Heart Disease.

BACKGROUND: The implantable cardioverter-defibrillator (ICD) has been proven to improve survival in adults with congenital heart disease (ACHD), but it is associated with a high rate of complications. We aimed to quantify the incidence of early (≤ 3 months; ECs) and late (> 3 months; LCs) complications in ACHD patients implanted with an ICD and to identify their clinical predictors. METHODS: We retrospectively reviewed 207 patients who had ICD follow-up at Toronto General Hospital from 1996 to 2019. RESULTS: The most common diagnoses were tetralogy of Fallot (32.4%), dextro-transposition of

Post-cardiac Implantable Electronic Devices: Inflammation of the Pocket. Should We Be More Aggressive?

BACKGROUND: Pacemaker pocket early post-implantation inflammation (EPII) is defined as clinical signs of local erythema without any systemic signs of infection occurring early after implantation. Data on the best treatment regimen for apparent superficial EPII is scarce. OBJECTIVES: To investigate the prognostic value of medical treatment, rather than extraction surgery, in patients with EPII. METHODS: Data from 6013 consecutive patients who underwent cardiac implantable electronic device (CIED) implantation or replacement from 2007-2015 were retrospectively analyzed; 40 (0.7%) presented wit

Monitoring excimer laser-guided cardiac lead extractions by uniportal video-assisted thoracoscopy: A single center experience.

BACKGROUND: Though laser guided extractions of cardiac implantable electronic devices leads have become a routine procedure, the severe complications are associated with a high mortality. Here, we report our single center experience using uniportal video-assisted thoracoscopy for laser lead extraction and compare it to stand-alone laser lead extraction. METHODS: The intraoperative data and postoperative clinical outcomes of patients undergoing laser lead extraction with concomitant thoracoscopy (N = 28) or without (N = 43) in our institution were analyzed retrospectively.
