Lumbar spinal stenosis: surgical considerations.

Spinal stenosis is an acquired or congenital narrowing of the spinal or nerve-root canals. Surgical treatment is often effective. Acquired spinal stenosis most commonly occurs in those with degenerative disk disease and arthritic facets. If the degenerative process stabilizes and there is adequate room to accommodate the neural contents, symptomatic patients become asymptomatic. Residual stability after decompression must be assessed in patients having multilevel decompression. Fusion maybe indicated. In women with osteoporosis coexisting with degenerative scoliosis and spinal stenosis, decomp

Fracture of L7 vertebral articular facets and pedicles following dorsal laminectomy in a dog.

A 6.5-year-old German Shepherd Dog underwent decompressive surgery for cauda equina syndrome. On the basis of intraoperative findings, the dorsal laminectomy was extended from L6 to S2. Partial dorsal annulectomies were also performed on the L6-7 and L7-S1 intervertebral disks. Two weeks following discharge, the dog had an acute onset of signs of pain and worsening of the neurologic status. Radiography and surgical exploration revealed bilateral fractures of the vertebral pedicles and caudal articular facets of L7. Vertebral stabilization was achieved with intramedullary pins and methylmethacr

Safety of chemonucleolysis. Adverse effects reported in the United States, 1982-1991.

This survey covers 121 "serious" and "unexpected" adverse events after treatment with chymodiactin (chymopapain for injection) among approximately 135,000 patients in the United States. They were reported to the Food and Drug Administration (FDA) within 15 days of notification of the manufacturer between 1982 and the end of 1991. They included fatal anaphylaxis (seven cases), infections (24 cases), hemorrhage (32 cases), and neurologic (32 cases) and miscellaneous (15 cases) events, with a mortality rate of 0.019%. Anaphylactic reactions reported in a postmarketing survey can be attributed to

Percutaneous transforaminal endoscopic discectomy in a nine-year-old patient with sciatica: case report, technical note and overview of the literature.

Percutaneous transforaminal endoscopic discectomy (PTED) is an alternative procedure to open microdiscectomy (OM) to treat sciatica caused by lumbar disk herniation. Even though robust evidence comparing PTED with OM is lacking, PTED is becoming increasingly popular to treat spinal disorders. In this technical report, the surgical technique and outcomes of PTED in a 9-year-old patient are described. Furthermore, an overview of the literature on full-endoscopic techniques to treat sciatica is given, showing that PTED is feasible, safe and effective to treat lumbar disk herniation in the pediatr

Report of a controlled clinical trial comparing automated percutaneous lumbar discectomy and microdiscectomy in the treatment of contained lumbar disc herniation.

STUDY DESIGN: The results of a randomized controlled trial comparing automated percutaneous lumbar discectomy (APLD) with lumbar microdiscectomy for the treatment of small contained lumbar discal herniations are reported. All patients gave full informed consent and were assessed by an independent observer. Seventy-one patients with radiologically confirmed small contained lumbar disc herniations were randomly assigned to undergo either APLD or lumbar microdiscectomy. All patients were formally assessed by the independent assessor using the Macnab outcome classification at 3 weeks, 2 months, an

Cervical disk arthroplasty: The new gold standard?

Cervical spondylosis is a broad term that describes pathological degeneration of the cervical spine, leading to axial neck pain, cervical myelopathy, and radiculopathy. Surgical treatments have been developed to address cervical radiculopathy and myelopathy, with the gold standard being anterior cervical diskectomy and fusion (ACDF). However, cervical fusion is associated with many well-known complications, such as pseudarthrosis and adjacent segment disease (ASD). As a result, cervical disk arthroplasty (CDA) was developed as an alternative to fusion for addressing cervical pathology without

Combined intra-extradural approach for posterolateral thoracic disk herniations. Preliminary study and technical note.

UNLABELLED: Purpose Thoracic disk herniation is uncommon. It still represents a challenge for spinal surgeons. Multiple surgical strategies are available and, often, they are matter of debate. We describe a preliminary experience about a combined extra-intra dural approach for posterolateral disk herniation in patients affected by spinal cord compression. METHODS: We performed a combined extra-intra dural approach in two preliminary cases. We performed bilateral laminectomy of adjacent vertebrae and unilateral partial medial arthrectomy of the involved segment. After a lateral extradural disk

Myoclonus induced by the use of gabapentin.

Myoclonus is a rare side effect of gabapentin (GBP) and has been reported in patients with preexisting myoclonus, mental retardation, chronic static encephalopathy, diffuse brain damage, impaired renal function, or end stage renal disease. We report a case of myoclonus in a patient with normal renal function and no previous disorders. A 69-year-old female underwent diskectomy and foraminotomy at the left L4-L5 level. Postoperatively, she complained of paresthesia in her left leg, which was thought to be due to root manipulation during surgery. To relieve the paresthesia, she was given tramadol

Injuries during a massive tug-of-war game.

"Tug-of-war" may cause a variety of sports injuries, which has rarely been reported previously. This report described an uncommon case of a previously fit 64-year-old male who presented with abrupt onset of loss of consciousness after falling down in a game of massive tug of war including 1,500 participants as the rope snapped apart. Computed tomography (CT) scan of his abdomen revealed liver and spleen rupture. Spinal cord injury due to traumatic herniation of intervertebral disc at C5-6 level and bilateral brachial plexus injury were also noted after exploratory laparotomy with primary repai

Treatment of herniated lumbar disk syndrome with chymopapain.

One hundred twenty-four patients with herniated lumbar disks received intradiskal injections of chymopapain before the Food and Drug Administration's withdrawal of the drug. One year after injection, 90 patients (72.6%) experienced major improvement, 21 (16.9%) had slight improvement, and 13 (10.5%) had no improvement. Of those patients who had no previous back surgery and who were not compensation cases, 77 patients (81%) had marked improvement, 13 (13.7%) had slight improvement, and five (5.3%) had no improvement. Three- to six-year follow-ups were obtained by questionnaire. Of the 114 patie

Research advances in unilateral endoscopic spinal surgery for the treatment of lumbar disc herniation: a review.

Lumbar disc herniation is a common cause of back and leg pain. Treatment has evolved significantly from traditional open surgery towards minimally invasive spine surgery (MISS). Within MISS, unilateral endoscopic spinal surgery (UESS), such as percutaneous transforaminal endoscopic discectomy (PTED) and unilateral biportal endoscopy (UBE), has largely replaced older bilateral approaches. Recent innovations like one-hole split endoscopy (OSE) and arthroscopic-assisted uniportal surgery (AUSS) further expand UESS capabilities. This review provides the first comprehensive analysis of UESS evoluti

Should adjacent asymptomatic lumbar disc herniation be simultaneously rectified? A retrospective cohort study of 371 cases that received an open fusion or endoscopic discectomy only on symptomatic segments.

BACKGROUND CONTEXT: In the treatment of multiple disc herniations, the decision of whether to include the presumed asymptomatic lumbar disc herniation (asLDH) at adjacent segments remains uncertain. On the one hand, the untouched asLDH might soon become symptomatic and require treatment. On the other hand, additional surgery involving more segments will introduce greater risk, complications, and cost. PURPOSE: To investigate the prognosis of untreated asLDH after open fusion or percutaneous endoscopic lumbar discectomy (PELD) on symptomatic lumbar disc herniation (LDHs) in patients. STUDY DE

Surgical outcomes of percutaneous endoscopic lumbar discectomy in obese adolescents with lumbar disc herniation.

In recent years, with improved living standards, adolescent obesity has been increasingly studied. The incidence of lumbar disc herniation (LDH) in obese adolescents is increasing yearly. No clinical studies have reported the use of percutaneous endoscopic lumbar discectomy (PELD) in obese adolescent lumbar disc herniation (ALDH) patients. This study evaluated the preliminary surgical outcomes of PELD in obese ALDH patients. Fifty-one ALDH patients underwent single-level PELD surgery between January 2014 and January 2020. Patients were divided into an obese group and a normal group. Patient ch
