Vessel wall MR imaging for the detection of intracranial inflammatory vasculopathies.

Intracranial vasculopathies are routinely investigated by lumen-based modalities such as magnetic resonance angiography (MRA), computed tomography angiography (CTA), and digital subtraction angiography (DSA). These techniques are useful to analyze the vessel lumen, allowing to detect vessel stenosis or occlusion. However, the primum movins of the disease, i.e., an abnormal thickening of the vessel wall, remains within the arterial wall. The vasculopathy can moreover be present without always narrowing the lumen or modifying its regularity. Hence, there is a need to detect directly and analyze

Carotid artery dissections: thrombosis of the false lumen.

Carotid artery dissections are the second leading cause of stroke in young adults. The hemostatic response to a dissection involves exposure of the subendothelium to the intravascular environment. Platelet activation/aggregation superimposed by secondary coagulation cascade activity attempts to heal the injury. Failure of the hemostatic response to heal the injury may lead to further rupture of the intimal and medial layers, which allows for the blood to penetrate these layers to create a false lumen. Continued hemorrhaging into the false lumen may result in dissection progression or obstructi

Safety of Physical Activity After Cervical Artery Dissection.

BACKGROUND: Cervical artery dissection (CeAD) is a major cause for stroke in young adults. A timely association with minor-to-moderate unimposing cervical trauma, which is often sports-related, is common in CeAD. Our goal was to assess whether physical activity puts patients at risk post-CeAD. METHODS: Pooled data from two prospective observational CeAD cohorts with in-person follow-up of at least 1-year post-CeAD were assessed. Changes in physical activity were recorded using patient-reported assessment of change in activity compared to pre-CeAD. Baecke score-derived sports index was applied

The current false lumen endograft and clinical experience for false lumen occlusion in chronic aortic dissection.

False lumen management of chronic aortic dissections has evolved during the past decade. Thoracic endovascular aortic repair (TEVAR) continues to be the mainstay of endovascular dissection treatment and relies on the adequate sealing of the proximal entry tear. However, TEVAR alone often fails to achieve aortic remodeling due to persistent distal retrograde perfusion of the false lumen with continuous aneurysmatic degeneration. Endovascular occlusion of this retrograde false lumen flow using dedicated false lumen endografts (FLEs), has therefore gained popularity. Similar to other endografts,

Repair of Arch Aneurysm With Frozen Elephant Trunk for DeBakey Type I Dissection Previously Treated With a Hybrid Arch Bare-Metal Stent.

The AMDS hybrid prosthesis is a novel device for the treatment of acute DeBakey I aortic dissection in patients without an intimal tear in the arch. Composed of an uncovered nitinol braided stent with a proximal felt cuff, it is deployed antegrade during circulatory arrest. Whereas early results have been promising, here we present 4 cases of negative aortic remodeling that were rescued by total arch frozen elephant trunk technique. These cases highlight the importance of patient selection in considering the AMDS device and illustrate that rescue of negative aortic remodeling is possible.

Staged repair for infected aortic arch graft and branched endograft post type A dissection repair.

A quinquagenarian underwent zone 2 arch repair for acute type A dissection followed by endovascular repair utilizing a branch endoprosthesis and covered stents. He developed a fever and positive blood culture results 3 weeks after the thoracic endovascular repair. A preoperative left carotid to subclavian artery bypass was performed. First stage: A right axillary artery was re-cannulated. The innominate vein was divided to facilitate the exposure. At a bladder temperature of 20℃, selective antegrade cerebral perfusion was established. A bifurcated graft was anastomosed to the left common carot

The frozen elephant trunk: seeking a more definitive treatment for acute type A aortic dissection.

OBJECTIVES: Conventional treatment for type A aortic dissection includes replacement of the ascending aorta with an open distal anastomosis in the hemiarch position. The frozen elephant trunk (FET) is a hybrid technique that extends the repair to the descending thoracic aorta. The goal is to improve resolution of malperfusion syndrome and to induce positive aortic remodelling and reduce the need for reintervention on the downstream aorta. We aim to summarize the data on the short and long-term outcomes of this technique. METHODS: A thorough search of the literature was conducted isolating all

The Frozen Elephant Trunk Procedure Using New Russian Hybrid Prosthesis "Soft Elephant Trunk": the Early Experience.

Aim      To present the first experience of performing the Frozen Elephant Trunk (FET) surgery in patients with thoracic aorta disease using a new Russian hybrid stent graft "Soft Elephant Trunk".Material and methods  Between 2014 and 2021, 170 patients with thoracic aortic disease underwent complete aortic arch replacement using the FET technique. In 70 of these cases (since June 2019), a hybrid graft "Soft Elephant Trunk" was used. A specific feature of this graft is the conical reduction of the radial force of nitinol crowns and the soft distal end without radial force. The study endpoints

Bio-Bentall and hybrid arch frozen elephant trunk repair for acute type A aortic dissection with malperfusion.

The extent of repair in patients with acute type A aortic dissection is often determined by factors such as entry tear location, aortic anatomy, malperfusion and team expertise. The hybrid arch frozen elephant trunk, which has become an established technique to extend the distal acute type A aortic dissection repair, is particularly useful in malperfusion; however, it remains technically challenging and is associated with increased duration of circulatory arrest and risks of spinal cord ischaemia. Proximal dissection flap extension often determines repairability versus replacement of the aorti

Novel repair of acute Stanford type B aortic dissection using combined endovascular graft and transfemoral replacement of vascular plug.

We present successful treatment of dissected thoracoabdominal aorta using combined thoracic endovascular aortic repair and transfemoral replacement of Amplatzer™ vascular plug in a 38-year-old patient. Computed tomography angiography revealed a false lumen from the left subclavian artery to the left common iliac trunk, with re-entries connecting it to the true lumen of the aorta. We replaced the prosthetic endovascular graft just below the left subclavian artery to the top of the superior mesenteric artery. Amplatzer™ vascular plug was replaced below the diaphragm in a closed transfemoral proc

Frozen elephant trunk for blunt aortic arch injury.

Total arch replacement with the frozen elephant trunk technique is expanding its indications to treat a wide range of aortic arch and descending aortic conditions. Although blunt thoracic aortic injury commonly occurs at the aortic isthmus or descending thoracic aorta, we encountered a 36-year-old male patient with Grade III blunt aortic injury located in the mid-arch. An endovascular approach was not feasible and thus the patient underwent total arch replacement using the frozen elephant trunk technique.

Thoracic EndoVascular Aortic/Aneurysm Stent-Graft Repair (TEVAR) primer.

Cardiac surgeons have variable exposure to thoracic aortic EndoVascular repair (TEVAR) as the primary surgeon. As paradigms evolve and EndoVascular structural heart interventions expand, TEVAR utilization by cardiac surgeons is of particular interest. In order to definitively manage thoracic aortic pathologies in the modern era, cardiac surgeons must incorporate TEVAR into their armamentarium during surgical training or risk a prolonged learning curve.

Retrograde type A dissection in the Vascular Quality Initiative thoracic endovascular aortic repair for dissection postapproval project.

BACKGROUND: Retrograde dissection (RD) can be a serious complication after thoracic endovascular aortic repair (TEVAR), with retrograde type A dissection (RTAD) particularly life-threatening. Prior studies have suggested that treatment timing, anatomic characteristics, device selection, and procedural conduct of TEVAR performed for type B aortic dissection could mitigate the occurrence of RD. The Vascular Quality Initiative TEVAR for Dissection Registry is an ongoing project meant to satisfy Food and Drug Administration requirements for postmarket approval surveillance of the Gore conformable
