BACKGROUND: The public health costs associated with alcohol-related traffic crashes are a continuing problem for society. One harm reduction strategy has been to employ per se limits for blood alcohol concentrations (BACs) at which drivers can legally operate motor vehicles. This limit is currently 0.08% in all 50 US states. Recently, the National Transportation Safety Board proposed lowering the legal limit to 0.05% (NTSB, 2013). While research has well-validated the ability of alcohol to impair driving performance and heighten crash-risk at these BACs, relatively little is known about the de

We report here the case of a death following blunt trauma to the head in which the victim immediately became unconscious and remained unconscious until death. There were no associated intracranial bleeding or visible macroscopic/microscopic injuries to the brain. Additional injuries to other parts of the body included non-fatal contusions and abrasions. The victim was intoxicated, with a blood alcohol level of 168 mg/dl. Therefore, the conclusion was drawn that death in this case was due to the combination of the concussive effects of the head trauma and the central nervous system depressant e

RATIONALE: Chronic non-cancer pain (CNCP) is a major health problem. Patients are increasingly treated with chronic opioid therapy (COT). Several laboratory studies have demonstrated that long-term use of opioids does not generally impair driving related skills. But there is still a lack of studies investigating on-the-road driving performance in actual traffic.
OBJECTIVES: The present study assessed the impact of COT on road-tracking and car-following performance in CNCP patients.
METHODS: Twenty CNCP patients, long-term treated with stable doses of opioid analgesics, and 19 healthy control

INTRODUCTION: Driving following illicit drug consumption ('drug-driving') is a potential road safety risk. Roadside drug testing (RDT) is conducted across Australia with the dual aims of prosecuting drivers with drugs in their system and deterring drug-driving. We examined trends over time in self-reported past six-month drug-driving among sentinel samples of regular drug users and assessed the impact of experiences of RDT on drug-driving among these participants.
METHODS: Data from 1913 people who inject drugs (PWID) and 3140 regular psychostimulant users (RPU) who were first-time participan

Oral fluid is a viable alternative to traditional sample matrices used in driving under the influence of drugs (DUID) casework. Oral fluid offers several advantages including rapid collection relative to the time of incident, ease of collection, and the ability to detect all routinely encountered drugs of abuse. In July of 2024, the New York State Police (NYSP) implemented a statewide evidentiary oral fluid program following the successful completion of a nine-month pilot study. The pilot consisted of 201 paired oral fluid-blood samples which showed an overall correlation rate of 96%. Ninety-t

Sevoflurane, a volatile anesthetic routinely used in clinical settings, was investigated to determine the extent of its interference with in-house forensic blood ethanol analysis. This potential interference could have a significant impact on the analysis and subsequently the interpretation of ethanol in human performance antemortem forensic toxicology casework (e.g. Driving While Under the Influence (DWI) cases). Aqueous samples with ethanol concentrations spanning 0.02-0.40 g/100 mL were fortified with sevoflurane and analyzed using two different dual-column headspace--gas chromatography wit

BACKGROUND: Although driving under the influence of cannabis is increasingly common among young adults, little is known about residual effects on driver behavior. This study examined acute and residual effects of smoked cannabis on simulated driving performance of young cannabis users.
METHODS: In this double-blind, placebo-controlled, parallel-group randomized clinical trial, cannabis users (1-4 days/week) aged 19-25 years were randomized with a 2:1 allocation ratio to receive active (12.5% THC) or placebo (0.009% THC) cannabis in a single 750 mg cigarette. A median split (based on whole-blo

INTRODUCTION: Driving while impaired (DWI) increases the risk of a motor vehicle crash by impairing performance. Few studies have examined the prevalence and predictors of marijuana, alcohol, and drug-specific DWI among emerging adults.
METHODS: The data from wave 3 (W3, high school seniors, 2012, N=2407) and wave 4 (W4, one year after high school, N=2178) of the NEXT Generation Health Study with a nationally representative cohort. W4 DWI (≥1day of past 30days) was specified for alcohol-specific, marijuana-specific, alcohol/marijuana-combined, illicit drug-related DWI. Multinomial logistic re

OBJECTIVE: Using benzodiazepines and certain antidepressants is associated with an increased risk of motor vehicle crashes due to impaired driving skills. Hence, several countries prohibit people who use these drugs from driving. Traffic regulations for driving under the influence of these drugs are, however, largely based on single-dose studies with healthy participants. The effects of drugs on chronic users may be different because of potential development of tolerance or by adapting behavior. In this study, we test the effects of anti-depressants, hypnotics, or anxiolytics use on driving pe

Single-vehicle (SV) crashes are of major concerns because of their high fatality rates. To understand the proneness of high injury severity for vehicle operators brought about by SV crashes without the confounding influence of other road users, this study focuses on those SV crashes without colliding with pedestrians, which are defined as out-of-control SV crashes given the general consequence of involved vehicles. Moreover, to compare the influence of contributory factors (including driver-vehicle/rider-vehicle, roadway, and environmental characteristics) by vehicle types, the injury severity

The implementation of the points-based driving license helps to change the drivers' behavior and is related to a reduction of traffic accidents and fatalities. In Spain, when a driver loses all points, the driving license is revoked, so the driver must enroll on a Driver Awareness and Re-education (DARE) course. However, at the moment offenders are not submitted to any test to confirm absence of alcohol or drugs of abuse consumption, even when 9% of Spanish drivers lose their driving license for driving under the influence (DUI). The objective of this pilot study was the comparison of the usef

BACKGROUND: Alcohol- and cannabis-impaired driving remain major public health concerns, particularly among young adults. Although prior studies have identified numerous risk factors, most have focused on limited subsets of predictors, restricting a broader understanding of impaired driving. This study applied machine learning to identify salient predictors of alcohol- and cannabis-impaired driving from a wide range of candidate variables.
METHODS: Data came from annual cross-sectional surveys of 18- to 25-year-olds participating in the Washington Young Adult Health Survey (2015-2022). Analyse

INTRODUCTION: Crashes are a global public health concern, with psychoactive substance use and attention impairments potentially increasing risk and affecting male and female drivers differently. This study aims to identify factors related to psychoactive substance use and attention impairments that predict road crash involvement by sex, offering insights for targeted prevention effort.
METHODS: This observational study examined individuals assessed for driving license reissuance after driving under the influence of alcohol and/or drugs. Data collected included personal and sociodemographic de

BACKGROUND: Barriers to accessing opioid agonist treatment (OAT) extend beyond regulatory and clinical constraints, with logistical factors such as clinic location, limited prescriber availability, and transport challenges also playing a role. Despite this, transport-related barriers remain underexplored in the literature. This study aims to examine transport practices and driving histories among OAT clients.
METHODS: A cross-sectional survey across 15 public OAT clinics in New South Wales, Australia, was performed. Data were collected on participant demographics, sedating medication and illi

Underage drinking is a significant problem in the US. It is responsible for several thousand mortalities and fatalities each year, both among minors and other members of society. Additionally, underage alcohol consumption produces a severe economic burden in the US. Introduction to alcohol in youth poses serious long-term risks for adolescents, including occupational, educational, and psychosocial impairments, and increases the risk for developing alcohol abuse disorders in adulthood. In order to address and mitigate this problem, the US has set a minimum age drinking law of 21 in all 50 state

Gabapentin is an approved treatment for epilepsy and neuropathic pain but due to a relatively minimal side effect profile at therapeutic doses it is also prescribed for numerous off-label uses. Due to an increasing number of prescriptions, the misuse and abuse of gabapentin has also increased. This study reviewed driving while intoxicated (DWI) and postmortem cases positive for gabapentin submitted to the Dallas County Southwestern Institute of Forensic Sciences from January 2018 to December 2025. A total of 3189 cases were positive for gabapentin, of which 87 were DWI and 2973 were postmortem
