Early mobilization of operatively fixed ankle fractures: a systematic review.

BACKGROUND: It is commonly believed that early motion after joint fixation is advantageous, especially in the upper limb. In the ankle joint this is much less clear. No previous systematic review of the evidence for this could be found in the literature. MATERIALS AND METHODS: Nine randomized control trials were identified which met the inclusion criteria and compared early motion of the ankle joint to immobilization in a cast for 6 weeks. These varied in quality and numbers. All treated patients equally in all other respects including weight bearing. Where outcome measures were similar, some

The effect of knee immobilization on degree of hip flexion: a clinical correlation with posterior wall acetabular fractures.

The purpose of this investigation was to determine whether maintaining the knee in extension substantially limits hip flexion. The dominant lower extremity in 22 male subjects was goniometrically evaluated to determine hip flexion both when the knee was allowed to flex and when the knee was held in extension. Pelvic rotation values were subtracted from thigh rotation values to show true flexion of the femur in relation to the pelvis. Hip flexion averaged 49.25 degrees +/- 9.08 degrees in the straight-leg raise test and 94.14 degrees +/- 4.94 degrees in the flexed-knee raise test. This study sh

The Swedish experience with Salter's innominate osteotomy in the treatment of congenital subluxation and dislocation of the hip.

The results of 83 Salter's innominate osteotomies (i.o.) performed to correct congenital subluxation and dislocation of the hip joint in 76 patients were reviewed. The overall radiographic results were classified as excellent or good in 34 hips (41%) and as fair or failure in 49 hips (59%). The best results were obtained in subluxated hips, previously not treated or treated only with closed reduction, prior to i.o. in patients operated on before the age of 5 years. In that group of hips, excellent or good results were obtained in 18 of 20 hips (90%). The poorest results were obtained in hips w

Modification of the Wagner lengthener to permit dynamic axial loading.

The Wagner lengthener is mechanically excellent, but it lacks a function allowing dynamic axial loading (dynamization), which stimulates bone consolidation at the lengthened section. The authors devised a simple metal insert with which they modified the Wagner lengthener to permit dynamization. The modified appliance was successfully used in two patients.

Early mobilisation after primary flexor tendon repair in 152 fingers (excluding zone II) and in 60 thumbs. "France Ouest Main Service".

The results of classical treatment (suture plus immobilisation) and of Kleinert's technique (suture under magnification with early semipassive movement), were compared in 290 primary flexor tendon repairs. The site of injury and the nature of associated injuries were the major prognostic factors. Kleinert's rehabilitation program was of particular value when a large range of movement was needed in simple lesions of zones III and IV. When tendon rupture must be avoided and preservation of strength is more important than achieving a full range of movement, immobilisation after an atraumatic sutu

Temporal release pattern of copeptin and troponin T in patients with suspected acute coronary syndrome and spontaneous acute myocardial infarction.

BACKGROUND: The release pattern of copeptin during the initial 36 h of spontaneous acute myocardial infarction (AMI) has received relatively little investigation but may provide important information on optimal timing of diagnostic measurements. METHODS: We investigated the release pattern of copeptin and cardiac troponin T in patients with suspected acute coronary syndrome (ACS). Blood samples were collected in the ambulance, at admission, and after 2, 4, 6, and 12-36 h. Copeptin and high-sensitivity cardiac troponin T (hs-cTnT) were measured in heparin plasma samples. RESULTS: Of 93 patien

Physical therapy on the wards after early physical activity and mobility in the intensive care unit.

BACKGROUND: Weakness and debilitation are common following critical illness. Studies that assess whether early physical activity initiated in the intensive care unit (ICU) continues after a patient is transferred to a ward are lacking. OBJECTIVE: The purpose of this study was to assess whether physical activity and mobility initiated during ICU treatment were maintained after patients were discharged from a single ICU to a ward. DESIGN: This was a cohort study. METHODS: Consecutive patients who were diagnosed with respiratory failure and admitted to the respiratory ICU (RICU) at LDS Hospita

[Traumatologic problems in old age (author's transl)].

In all fractures of the extremities survival has priority to local problems. The first thing to take care of is restitution of joint mobility, especially at the upper extremity (shoulder-and elbow joint). Early ambulation is facilitated by hanging cast and correctly indicated surgery, especially concerning humerus and elbow. At the lower extremities painless stability is more important than full mobilisation of the joints. Frequently a walking plaster cast is suited best for early ambulation and self support. Special problems are connected with fractures of the upper end of the femur. The incr

Feasibility, safety and outcomes of ambulation within 2 h postoperatively in patients with lung cancer undergoing thoracoscopic surgery.

AIMS: The aims of this study were to evaluate the safety, feasibility and outcomes of ambulation within 2 h after thoracoscopic surgery in patients with lung cancer. BACKGROUND: There are no consensus guidelines on the ideal time for early ambulation following thoracic surgery, although enhanced recovery programmes have been proposed since years. METHODS: This non-randomized, concurrent-control study was conducted on patients who underwent thoracoscopic surgery between October 2020 and February 2021. Participants were assigned to either the observation group (ambulation within 2 h of extubat

Functional Outcomes of Service Members With Bilateral Transfemoral and Knee Disarticulation Amputations Resulting From Trauma.

As longitudinal studies for those with bilateral transfemoral amputation (BTFA) or knee disarticulation (KD) are lacking, it is important to quantify performance measures during rehabilitation in an effort to determine reasonable expectations and trends that may influence the rehabilitation process. At initial evaluation (date of first independent ambulation) and follow up (median 135 [range = 47-300] days later), 10 participants with BTFA/KD completed 6 minute walk testing and Activity Specific Balance Confidence and Lower Extremity Functional Scale questionnaires. Of these, six participants

Knee surveillance for ambulant children with cerebral palsy.

PURPOSE: A majority of ambulant children with cerebral palsy (CP) develop progressive musculoskeletal pathology (MSP) during growth. Fixed flexion deformity at the knee joint (FFDKn) contributes to flexed knee gait and is prone to relapse after index multi-level surgery. This perspective introduces the concept of "knee surveillance" (KS), defined as a repeated systematic assessment of gait and knee range of motion until skeletal maturity. KS aims to detect early FFDKn, allowing for early intervention with minimally invasive techniques such as anterior distal femur hemiepiphysiodesis (ADFH), an

Early motion after quadriceps and patellar tendon repairs: outcomes with single-suture augmentation.

BACKGROUND: Complications of immobilization after quadriceps and patellar tendon repairs include decreased patellar mobility, limited flexion, persistent pain, muscle weakness, and patella baja. In contrast, early motion limits muscle atrophy, accelerates tendon healing, and prevents joint stiffness. HYPOTHESIS: Quadriceps and patellar tendon repairs protected with a "relaxing suture" are strong enough to safely permit early motion, full weightbearing, and brace-free ambulation. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Twenty quadriceps and 30 patellar tendon ruptures were

Recent trends in lower extremity amputation. Surgery and rehabilitation.

With the use of modern prosthetic appliances and the initiation of early ambulation, satisfactory function can be expected in the vast majority of below-knee amputees. The rehabilitation of the above-knee amputee is more difficult and the performance of these patients does not compare favorably with that of the below-the-knee amputee. Immediate postsurgical fitting appears to have resulted in a higher percentage of successful below-knee amputations in patients with peripheral vascular disease. The rigid dressing provided by the temporary prosthesis applied to the stump immediately after the am

Strategies to minimize postoperative complications in microsurgical free tissue transfer: The role of anticoagulation, antiplatelets, and ambulation.

OBJECTIVE: Microsurgical free tissue transfer (FTT) is essential in reconstructive surgery, but thromboembolic events are a leading cause of flap failure. This study evaluates how perioperative anticoagulation, antiplatelet therapy, and ambulation affect postoperative complications in FTT. STUDY DESIGN: A retrospective cohort study of 497 free flap procedures assessed associations between antiplatelet and anticoagulation therapy, ambulation timing, and postoperative outcomes. The cohort included pediatric and adult patients who underwent a free tissue transfer from 2012 to 2021. Multivariable

Prediction of independent ambulation at hospital discharge in patients with proximal femoral fractures based on preoperative clinical information: A retrospective study using extreme gradient boosting and SHapley additive explanations.

BACKGROUND: Independent ambulation at hospital discharge is a critical determinant of discharge destination and caregiving burden in older adults with proximal femoral fractures. Preoperative prediction of walking independence may support discharge planning and early intervention. METHODS: This retrospective observational study included 350 patients who underwent surgery for proximal femoral fractures between April 2018-April 2023. Independent ambulation was defined as Functional Ambulation Category (FAC) = 5. Preoperative variables included age, Body Mass Index (BMI), cognitive function (HDS
