Efficient Mining Template of Predictive Temporal Clinical Event Patterns From Patient Electronic Medical Records.

Exploring the temporal relationship among events in patient electronic medical records (EMR) is an important problem in biomedical informatics and the results can reveal patients' impending disease conditions. In this paper, we investigate the problem of mining patterns from a sequence of point events, i.e., we only have the information on when the event happens but no duration or numerical value available. We propose a whole pipeline, including event preprocessing, pattern mining, and outcome analysis to mine the patterns and evaluate their effectiveness and discriminative power. Finally, we

Medical Emergencies Related to Ethanol and Illicit Drugs at an Annual, Nocturnal, Indoor, Electronic Dance Music Event.

UNLABELLED: Introduction Medical problems are frequently encountered during electronic dance music (EDM) events. Problem There are uncertainties about the frequencies and severity of intoxications with different types of recreational drugs: ethanol, "classical" illicit party drugs, and new psychoactive substances (NPS). METHODS: Statistical data on the medical problems encountered during two editions of an indoor electronic dance event with around 30,000 attendants were retrieved from the Belgian Red Cross (Mechelen, Belgium) database. Data on drug use were prospectively collected from the pa

Feasibility of Using Electronic Medical Record Data for Tracking Quality Indicators in Adults with Congenital Heart Disease.

BACKGROUND: In order to determine the feasibility of tracking quality of care in adults with congenital heart disease (ACHD), we aimed to estimate the availability of relevant data in electronic medical records (EMR) used in North American ACHD centers. METHODS: Previously proposed quality indicators (QIs) were reviewed to consider what types of data would be required for each. ACHD program directors were surveyed about the nature of electronic data in existing EMRs. From the survey, the availability of data types needed for the denominator and numerator of each QI were estimated, and an over

Electronically monitored dosing histories can be used to develop a medication-taking habit and manage patient adherence.

Nonadherence to prescribed medications can lead to medical complications, disease progression, hospitalizations, overestimated dosing requirements, impaired quality of life, and death, as well as incurring substantive costs for the healthcare system from suboptimal dosing during ambulatory pharmacotherapy. Adherence can be improved by helping patients build habits of taking prescribed medications, impacting day-to-day implementation of and persistence with rationally prescribed drug dosing regimens. Accurate, easily understood and personally relevant feedback is clearly relevant to many patien

Physician assessment of disease activity in JIA subtypes. Analysis of data extracted from electronic medical records.

OBJECTIVE: Although electronic medical records (EMRs) have facilitated care for children with juvenile idiopathic arthritis (JIA), analyses of treatment outcomes have required paper based or manually re-entered data. We have started EMR discrete data entry for JIA patient visits, including joint examination and global assessment, by physician and patient. In this preliminary study, we extracted data from the EMR to Xenobase™ (TransMed Systems, Inc., Cupertino, CA), an application permitting cohort analyses of the relationship between global assessment to joint examination and subtype. METHODS

An analytical approach to characterize morbidity profile dissimilarity between distinct cohorts using electronic medical records.

We describe a two-stage analytical approach for characterizing morbidity profile dissimilarity among patient cohorts using electronic medical records. We capture morbidities using the International Statistical Classification of Diseases and Related Health Problems (ICD-9) codes. In the first stage of the approach separate logistic regression analyses for ICD-9 sections (e.g., "hypertensive disease" or "appendicitis") are conducted, and the odds ratios that describe adjusted differences in prevalence between two cohorts are displayed graphically. In the second stage, the results from ICD-9 sect

Improving diagnostic testing and reducing overuse of antibiotics for children with pharyngitis: a useful role for the electronic medical record.

BACKGROUND: Because of rising resistance to antibiotics, appropriate use of antibiotics is an important measure of quality of care. The purpose of this study was to use an electronic medical record (EMR) to assess use of diagnostic testing and of antibiotics for pharyngitis in a pediatric outpatient setting and to target areas for improvement. METHODS: Using data retrieved from the EMR, we analyzed visits from March 1, 2001 to February 28, 2002 for children 3 to 18 years old diagnosed with pharyngitis. We determined the proportion of episodes with a diagnostic test for group A streptococci, t

A Sentence Classification-Based Medical Status Extraction Pipeline for Electronic Health Records: Institutional Case Study.

BACKGROUND: Clinical data warehouses store large volumes of unstructured text containing valuable information about patients' medical status. Traditional extraction systems based on named entity recognition (NER) identify medical terms but often fail to capture the contextual cues needed for accurate interpretation. Existing approaches to context-aware extraction differ in their reliance on expert annotation, computational power, and lexical resources, leading to uneven feasibility across institutions. Combined with heterogeneity in documentation practices and data-sharing restrictions, these

Evaluating Electronic Health Record Intervention Efficacy in Enhancing Inpatient Initiation of Guideline-Directed Medical Therapy.

BACKGROUND: Heart failure (HF) is a leading cause of hospitalization, morbidity, and mortality. Despite evidence supporting guideline-directed medical therapy (GDMT), prescribing remains suboptimal, particularly at hospital discharge. PROJECT RATIONALE: Hospitalization provides an opportunity to initiate or optimize GDMT, but workflow barriers and practice variation limit implementation. To address this gap, our system implemented a system-wide HF quality improvement initiative featuring evidence-based order sets and an electronic health record smart phrase ("HFNOTE") to centralize and standa

Proactive identification of rare diseases: lessons from hereditary angioedema diagnosis using electronic medical records.

BACKGROUND: Diagnosing rare diseases traditionally requires patients to endure lengthy and challenging journeys to find specialists familiar with their conditions. This study advocates a paradigm shift in rare disease diagnosis, moving from patients seeking physicians to physicians actively identifying patients. Using hereditary angioedema (HAE) as an example, we demonstrate how this approach, supported by electronic medical records (EMRs), enables proactive care for patients with rare diseases. Our EMR system incorporates a free-text search engine to screen for patients with potential HAE bas
