Extracorporeal membrane oxygenation in acute massive pulmonary embolism: a systematic review.

Massive pulmonary embolism (PE) can present with extreme physiological dysfunction, characterised by acute right ventricular failure, hypoxaemia unresponsive to conventional therapy and cardiac arrest. Consensus regarding the management of patients with persistent shock following thrombolysis is lacking. Our primary objective was to describe the application of extracorporeal membrane oxygenation (ECMO) in the treatment of acute massive PE. We were unable to identify any randomised controlled trials (RCTs) comparing ECMO with other support systems in the setting of massive PE. We reviewed case

Massive pulmonary embolism in late pregnancy.

PURPOSE: To describe the management problems presented by a case of acute massive pulmonary embolism in a labouring woman. CLINICAL FEATURES: A case of massive pulmonary embolism is described in a woman who presented in early labour at thirty-eight weeks gestation. Immediate management involved the administration of oxygen and intravenous heparin, and transfer to the regional cardiothoracic centre. Pulmonary angiography confirmed the diagnosis of massive pulmonary embolism, but attempts at percutaneous catheter disruption of the clot were of only temporary benefit. The patient subsequently un

[Peripheral calcific embolism after percutaneous mitral valvuloplasty].

The authors report a case of calcific embolization after percutaneous mitral valvuloplasty. The nature of the embolism was confirmed by anatomopathological examination after popliteal embolectomy. This complication would appear to be extremely rare but represents an additional risk in patients with calcific mitral stenosis.

[Acute occlusion of the renal artery caused by an embolism of cardiac origin. Treatment by in situ thrombolysis].

An intra-arterial infusion of streptokinase successfully dissolved a large thrombus of the renal artery in a 54-year old male patient with old-standing mitral stenosis. This case illustrates the effectiveness of fibrinolysis in situ. This method provides complete arterial recanalization and preserves the renal function, thus enabling surgical embolectomy to be performed.

Percutaneous aspiration thromboembolectomy (PAT): an alternative to surgical balloon techniques for clot retrieval.

Percutaneous aspiration thromboembolectomy (PAT) is an angiographic technique that can be used to remove thromboembolic debris from the distal lower extremity circulation. This procedure employs a specially designed catheter-sheath system, which can be used alone or in combination with balloon angioplasty or thrombolytic drugs (streptokinase 10,000 U/hr or urokinase 100,000 U/hr for 6 hours) to remove thromboembolic material. PAT is best suited for treating iatrogenic emboli resulting from intra-arterial catheterization or balloon angioplasty but can be used as a supplement to Fogarty embolect

Catheterization of branchial sinus tracts. A new method.

The complete surgical extirpation of branchial cleft abnormalities is essential in avoiding recurrent disease. The most technically difficult maneuver is often the localization and excision of associated sinus tracts and fistulae. The authors communicate a simple yet effective technique for dealing with this difficulty utilizing the Fogarty embolectomy catheter.

Characteristics and Outcomes of Patients Transferred for Treatment of Acute Limb Ischemia.

BACKGROUND: Patients requiring emergent vascular surgery often undergo transfer from one facility to another for definitive surgical care. In this study, we analyzed morbidity and mortality in patients presenting for emergent lower extremity thrombectomy and embolectomy in the transferred and nontransferred populations. METHODS: A retrospective analysis of prospectively collected data was performed utilizing the National Surgical Quality Improvement Program (NSQIP) database for all nonelective, emergent lower extremity embolectomy or thrombectomy (Current Procedural Terminology [CPT] 34,201 a

Controlled reperfusion using a simplified perfusion system preserves function after acute and persistent limb ischemia: a preliminary study.

OBJECTIVE: Reperfusion of the limb after acute and persistent ischemia is associated with high rates of morbidity and mortality despite complete revascularization. Although reperfusion is a prerequisite for maintaining limb function, it may in itself cause further injury. There is experimental evidence that modification of the initial reperfusion modalities can minimize this reperfusion injury. We hypothesized that controlled reperfusion using a simple blood bag perfusion system reduces reperfusion injury and facilitates the return of normal function. METHODS: Fifteen consecutive patients (me

Hybrid Surgical Approach for Acute Limb Ischemia: A Case Report and Management Algorithm.

Acute limb ischemia (ALI) is a time-critical vascular emergency associated with high morbidity and amputation risk if treatment is delayed. Popliteal artery aneurysms (PAAs), though less common, represent an important and often underrecognized cause. We report a 67-year-old man who presented with sudden-onset severe leg pain, pallor, sensory loss, and motor deficit, consistent with Rutherford IIb ALI. Computed tomography angiography revealed a nearly thrombosed PAA with absent distal run-off. The patient underwent hybrid revascularization combining open thrombo-embolectomy with prosthetic inte

Review of Interventional Options and Surgical Techniques in the Management of Acute Pulmonary Embolism.

INTRODUCTION: Acute massive pulmonary embolism (PE) represents a potentially lethal condition causing acute right ventricular heart dysfunction and cardiogenic shock. We aim to provide an overview of interventional and surgical options for PE management, with a focus on surgical techniques and outcomes. METHODS: This was a narrative review so a systematic literature search was not performed. RESULTS: Catheter-directed thrombolysis and catheter-directed mechanical thrombectomy have demonstrated improved outcomes in several recent trials compared to medical management alone for higher risk PE.

An Unusual Case of Cannula Obstruction Resulting from Venous Thromboembolism.

Cerebral strokes of unknown origin frequently present with a patent foramen ovale (PFO), a common atrial septal defect occurring in approximately 25% of the adult population. Deep vein thrombosis (DVT) or pulmonary embolism (PE) in the presence of a PFO can produce paradoxical systemic embolization subsequent to an increase in pulmonary pressure, permitting entry of thrombi into the arterial circulation. Diagnosis of an impending paradoxical embolism (IPDE) involves the detection of DVT or PE in the presence of an abnormal communication between left and right circulations and may include a rig

Modern treatment of acute mesenteric ischaemia.

BACKGROUND: Diagnosis of acute mesenteric ischaemia in the early stages is now possible with modern computed tomography (CT), using intravenous contrast enhancement and imaging in the arterial and/or portal venous phase. The availability of CT around the clock means that more patients with acute mesenteric ischaemia may be treated with urgent intestinal revascularization. METHODS: This was a review of modern treatment strategies for acute mesenteric ischaemia. RESULTS: Endovascular therapy has become an important alternative, especially in patients with acute thrombotic superior mesenteric a

Acute and Chronic Pulmonary Emboli.

Under most circumstances, the goal of treatment of pulmonary embolism is the prevention of recurrent embolic events, achieved through conventional anticoagulant therapy with unfractionated heparin or a low molecular weight heparin, followed by warfarin therapy for a minimum of 6 months. When acute pulmonary embolism is associated with significant right ventricular dysfunction or systemic hypotension, more aggressive intervention may be warranted. Under these circumstances, potential interventions include thrombolytic therapy (either systemic or catheter-directed), placement of an inferior vena
