Helicopter rescue and prehospital care for drowning children: two summer season case studies.

BACKGROUND: Drowning is the second leading cause of unintentional injury-related death in children <14 years of age and is one of the most important causes of accidental injury between the ages of 1 and 4 years. In this study, the characteristics of non-fatal unintentional drownings in a small series of pediatric victims were examined. METHODS: We retrospectively analyzed data collected by the rescue team from May to October in two consecutive years (2006, 2007). RESULTS: Nine accidents occurred in public waters, while 5 occurred in lakes and rivers. The submersion time reported ranged from

[Trends in research on the history of medicine in Korea before the modern era].

Research on the history of medicine in Korea in the form of modern scholarship began with the publication in 1930 of Yi Neunghwa's "A History of the Development of Medicine in Korea." The purpose of the present study lies in surveying studies on the history of medicine in Korea in the past 80 years since the publication of Yi's paper. In terms of periodization, research on the history of medicine in Korea is bifurcated by the publication of two comprehensive histories-i. e., Miki Sakae's A History of Medicine and Disease in Korea (1963) and Kim Du-jong's The Complete History of Medicine in Kor

Small Bowel Obstruction Due to Abdominal Cocoon Syndrome in Post-COVID-19 Patients.

BACKGROUND: Abdominal cocoon syndrome, also recognized as encapsulating peritoneal sclerosis, is an exceedingly rare medical condition characterized by an anomalous membranous envelopment of the small bowel. Despite its clinical rarity, the precise etiology and pathogenic mechanisms of this syndrome remain elusive. METHODS: This comprehensive discussion presents a case series encompassing six patients who sought medical attention in the Emergency Room, all sharing a common presentation: severe abdominal distension and persistent constipation. Intriguingly, these symptoms manifested following

Large discrepancy between prehospital visitation to mobile emergency care unit and discharge diagnosis.

INTRODUCTION: In Copenhagen, Denmark, patients in need of prehospital emergency assistance dial 112 and may then receive evaluation and treatment by physicians (from the Mobile Emergency Care Unit (MECU)). ST-elevation myocardial infarction (STEMI) is a severe condition leaving only a limited time frame to deliver optimal care in the form of percutaneous transluminal coronary angioplasty. In theory, all patients with chest pain could have STEMI. The aim of this study was to study which of the patients suspected of having acute cardiac disease based on the 112 calls and met by the MECU were giv

Characteristics of pediatric drowning out-of-hospital cardiac arrests in the United States.

AIMS: Drowning is a leading cause of pediatric death in the United States, yet data characterizing these arrests is limited. Our study aimed to describe demographic and emergency medical services (EMS) response characteristics for drowning-related pediatric out-of-hospital cardiac arrests (pOHCAs). METHODS: Using the 2023 National Emergency Medical Services Information System dataset, we conducted a cross-sectional study of EMS-attended 9-1-1 responses involving children 1 day-17 years old who experienced a drowning-related pOHCA. We used descriptive statistics and multivariable logistic regr

Cognitive strategies in the Emergency Medical Services: proposal for a conceptual integrated framework to explicate clinical reasoning in EMS.

BACKGROUND: Clinical reasoning is fundamental to safe and effective Emergency Medical Services (EMS) care. EMS providers operate in dynamic, time-pressured environments characterised by diagnostic uncertainty, limited information, and high-stakes decisions. These conditions necessitate rapid yet adaptable reasoning strategies. The SPART model was developed through focused ethnography and content validation to describe observable diagnostic and therapeutic activities in EMS practice. It provides a structured representation of the care process from dispatch to transfer of care, serving as a basis

Patients discharged to police custody after paramedic evaluation: an observational cohort study.

BACKGROUND: In Finland, more than a third of the patients attended by the emergency medical services (EMS) are discharged on scene without ambulance transport, with reported recontact of <10% and 24-hour to 48-hour mortality of 0.3%-3.5%. In cases of disturbing or violent behaviour or suspected criminal activity, patients can also be discharged directly to police custody, with or without consultation with a prehospital physician. This study reports the incidence of patient deterioration in these situations. METHODS: All EMS calls between 2013 and 2021 with patient discharge to police custody

Early identification and delay to treatment in myocardial infarction and stroke: differences and similarities.

BACKGROUND: The two major complications of atherosclerosis are acute myocardial infarction (AMI) and acute ischemic stroke. Both are life-threatening conditions characterised by the abrupt cessation of blood flow to respective organs, resulting in an infarction. Depending on the extent of the infarction, loss of organ function varies considerably. In both conditions, it is possible to limit the extent of infarction with early intervention. In both conditions, minutes count. This article aims to describe differences and similarities with regard to the way patients, bystanders and health care pr

Evaluation of the injury profile of personnel in a busy urban EMS system.

The occupational injury profile of emergency medical technicians (EMTs) and paramedics is not well described. We retrospectively studied 254 injuries over a 3.5-year period in a busy urban EMS system. Low back strain was the most common injury (93/254, 36%), with EMTs suffering a significantly higher injury rate than paramedics (0.33 v 0.17 injuries/person-years at risk, P = .03). Lifting caused 58/93 (62.4%) of back injuries, and most occurred at the scene to which personnel were dispatched (58/93, 62.4%). The back injuries were recurrent in 31% of personnel. The data showed trends toward hig

A scoping review of support interventions for bystanders involved in out-of-hospital cardiac arrest.

BACKGROUND: Bystanders witnessing or performing cardiopulmonary resuscitation (CPR) during out-of-hospital cardiac arrest (OHCA) may experience significant psychological distress. Although the need to support bystanders after OHCA is increasingly recognised, existing post-event support interventions have not been previously described. OBJECTIVE: To identify and describe existing post-event support interventions for bystanders involved in OHCA. METHODS: A scoping review was conducted using Joanna Briggs Institute methodology and PRISMA-ScR reporting. Databases and grey literature were searche

Prehospital undertriage of older injured patients in western Switzerland: an observational cross-sectional study.

BACKGROUND: The ageing of the population is leading to an increase in the number of traumatic injuries and represents a major challenge for the future. Falls represent the leading cause of Emergency department admission in older people, with injuries ranging from minor to severe multiple injuries. Older injured patients are more likely to be undertriaged than younger patients. The aim of this study was to investigate the extent of undertriage in older patients with particular emphasis on access to trauma centres and resuscitation rooms. METHODS: Retrospective observational cross-sectional stu

Prehospital resuscitative endovascular balloon occlusion of the aorta during long-distance ground transport for traumatic liver injury: A case report.

BACKGROUND: Resuscitative endovascular balloon occlusion of the aorta (REBOA) is an established adjunct treatment of hemorrhagic shock caused by subdiaphragmatic bleeding. However, evidence for its use in prehospital settings and long-distance transportation in remote locations remains limited. Herein, we report a case of hemorrhagic shock secondary to traumatic liver injury, for which prehospital REBOA was successfully employed. CASE PRESENTATION: A 41-year-old man sustained multiple traumatic injuries after being run over by heavy machinery. The patient was diagnosed with hemorrhagic shock

The Impact of Prehospital and Hospital Care on Clinical Outcomes in Out-of-Hospital Cardiac Arrest.

Background: In recent years, several actions have been made to shorten the chain of survival in out-of-hospital cardiac arrest (OHCA). These include placing defibrillators in public places, training first responders, and providing dispatcher-assisted CPR (DA-CPR). In this work, we aimed to evaluate the impact of these changes on patients' outcomes, including achieving return of spontaneous circulation (ROSC), survival to discharge, and survival with favorable neurological function. Methods: We retrospectively retrieved data of all calls to the national emergency medical service in Ashdod city,

Drone-delivered automated external defibrillators in prehospital emergency care: implementation evidence, bottlenecks, and conceptual integration with artificial intelligence and next-generation communications.

Out-of-hospital cardiac arrest (OHCA) remains a major global health challenge. Survival depends on immediate cardiopulmonary resuscitation and early defibrillation. Static public-access defibrillator programs often fail to reach private, rural, or otherwise underserved locations. Drone-delivered automated external defibrillators (AEDs) may overcome ground-traffic and geographic barriers, but rapid delivery does not always lead to timely treatment. This Perspective synthesizes simulation, economic, and real-world implementation evidence, and it separates device arrival from successful defibrill

Beyond Clinical Acuity: Cardiac Comorbidity and Complication Profiles and the Prehospital Hospitalization/Transport Decision in Ischemic Heart Disease-A Five-Year Retrospective Emergency Medical Service Study in Astana, Kazakhstan.

Background/Objectives: Ischemic heart disease (IHD) is a major cause of cardiovascular mortality, and emergency medical service (EMS) crews often make the first decision on whether an IHD-coded patient should be transported to hospital or managed at the scene. Evidence from Central Asian EMS systems is sparse, and it remains unclear whether routinely coded comorbidity information adds decision-relevant information beyond acute presentation. We aimed to identify predictors of the EMS hospitalization/transport decision among IHD calls in Astana, Kazakhstan. Materials and Methods: We conducted a
