Emphysematous pyelonephritis-related delayed abdominal wall necrotizing fasciitis: A case report.

RATIONALE: Emphysematous pyelonephritis (EPN) is a severe, gas-forming renal infection typically associated with diabetes mellitus. Necrotizing fasciitis (NF) is a rapidly progressive soft tissue infection with significant morbidity. Although concurrent EPN and NF have been described, delayed NF arising years after previously treated EPN is exceedingly rare. This report highlights a unique case of abdominal wall NF secondary to a chronic infectious pathway originating from prior EPN. PATIENT CONCERNS: A 55-year-old woman with poorly controlled type II diabetes mellitus initially presented wit

Emphysematous Pyelonephritis with IgA-Dominant Infection-Related Glomerulonephritis: An Unusual Picture.

OBJECTIVE: The aim of this case report is to illustrate a very rare case of emphysematous pyelonephritis complicated by IgAdominant postinfectious glomerulonephritis. CASE REPORT: We report the case of a 53-year-old woman with emphysematous pyelonephritis who initially presented with unintentional weight loss for 3 months and subnephrotic range proteinuria without fever. Urinalysis revealed proteinuria, microscopic hematuria, and pyuria. A kidney biopsy was performed for suspected glomerulonephritis. The patient's right kidney biopsy was consistent with immunoglobulin A (IgA)-dominant infecti

[Fulminant Emphysematous Pyelonephritis Secondary to Urothelial Neoplasia with Reno-Colic Fistula].

The emphysematous pyelonephritis is a uncommon and potentially fatal entity. The recognition of this condition enables the early correct management of patients. Diabetes and ureteral obstruction are the main risk factors identified. We present the case of a patient with no relevant risk factors or prior medical history, in which the reason of admission to the emergency department was back pain, eventually evolving into septic shock in the early hours. The imaging tests identified the presence of peri-renal and subcutaneous gas which guided the medical team to the right diagnosis (emphysematous

Emphysematous pyelonephritis successfully treated by early intervention using a renoureteral catheter.

A 49-year-old woman with poorly controlled type 2 diabetes mellitus was admitted to hospital complaining of fever, vomiting, and lower abdominal pain. Laboratory investigation revealed leukocytopenia, high blood sugar, and pyuria. Pyelonephritis was then diagnosed. Escherichia coli was isolated from blood and urinary cultures. In spite of antimicrobial therapy, the patient's condition deteriorated. A computed tomography scan of the abdomen on the second day of hospitalization revealed the presence of air in the collecting system of the left kidney. Emphysematous pyelonephritis was diagnosed, a

First report of concurrent mixed germs induced destructive emphysematous pyelonephritis and emphysematous endophthalmitis in the patient with COVID-19.

A 35-year-old diabetic woman was referred to the emergency department with fever, left flank pain, pneumaturia, and impaired vision of the left eye from 4 days ago. Fever, tachycardia, tachypnea, low blood pressure, metabolic acidosis, and azotemia were the first findings. The diagnosis was a coincidence of emphysematous pyelonephritis and emphysematous endophthalmitis due to computerized tomography of the patient. Immediate fluid and electrolytes resuscitation, intravenous antibiotic administration, and nephrectomy save the patient. Urine, blood, and vitreous cultures revealed mixed germ infe

A Rare Case of Emphysematous Pyelonephritis in a Non-diabetic Patient With Staghorn Calculus.

Emphysematous pyelonephritis (EPN) represents a severe and acute infection localized in the renal parenchyma and surrounding perirenal area, typically observed in individuals with predisposing factors such as urinary tract obstruction, diabetes mellitus, or compromised immune function. Here, we present a unique case involving a 23-year-old female patient presenting to the emergency department with complaints of discomfort localized to the right side of her abdomen. Despite the absence of diabetes mellitus, the patient was diagnosed with EPN based on clinical presentation and imaging findings.

Emphysematous Pyelonephritis at 29 Weeks of Gestation: A Case Report and Review of the Literature.

Emphysematous pyelonephritis (EPN) is a urinary tract infection progression characterized by gas retention in the renal tissues and a high mortality rate, but few cases have been reported. In this study, we present a 32-year-old primigravida with type 2 diabetes mellitus and a history of pyelonephritis who developed pyelonephritis at 29 weeks. Antimicrobial therapy was initiated; however, her clinical symptoms worsened. Ultrasonography and magnetic resonance imaging (MRI) led to a diagnosis of EPN. Fortunately, the patient improved with antibiotic therapy and delivered at 39 weeks.  Treatment

Predictors of mortality and nephrectomy in emphysematous pyelonephritis: a tertiary care centre study.

INTRODUCTION: Emphysematous pyelonephritis (EPN) is a deadly disease due to its associated morbidity and mortality. Attempts have been made to identify predictors of severity, mortality and need for nephrectomy in EPN with little success. METHODS: We conducted an ambispective study of EPN patients between March 2014 and September 2019. Retrospective data were collected which included age, sex, comorbidities, symptoms, signs, laboratory investigations including imaging, need for dialysis, management and any complications. All patients were then followed prospectively for renal dynamic scan, st

Necrotizing Fasciitis of Thoracic and Abdominal Wall with Emphysematous Pyelonephritis and Retroperitoneal Abscess.

Emphysematous pyelonephritis is a life-threatening severe form of pyelonephritis usually occurring in patients with diabetes mellitus with or without obstructive uropathies in whom there is necrotizing infection leading to the gas production of an unclear mechanism involving the renal parenchyma and the collecting system. Necrotizing fasciitis is characterized by progressive necrosis of fat and fascia due to deep-seated infection of subcutaneous tissue. It has a fulminant course with considerable mortality. Diabetes Mellitus is a common predisposing factor. The combined occurrence of emphysema

Extensive emphysematous pyelonephritis in a renal allograft: case report and review of literature.

Emphysematous pyelonephritis (EPN) is an acute, severe necrotizing infection of the renal parenchyma and perirenal tissue, which results in the presence of gas within the renal parenchyma, collecting system, or perinephric tissue. EPN of renal allograft is rare, with only 23 cases reported in Western literature. Here, we report a patient treated successfully with surgery. We also review the literature, focusing on old and new suggested classification systems for EPN.

Bilateral emphysematous pyelonephritis cured by antibiotics alone: a case and literature review.

Emphysematous pyelonephritis (EPN) is a rare but severe infectious disease. This disease sometimes presents bilaterally, making it difficult to cure. Diabetes mellitus is a common cofactor in this disease. Drainage or nephrectomy, often combined with antibiotics, is ordinarily used for treatment. To our knowledge, only 8 cases of bilateral EPN cured by antibiotics alone have been reported. We report the case of an 86-year-old woman with bilateral EPN cured by antibiotic therapy alone, thus avoiding surgery or drainage in a frail elderly patient.
