Cutaneous small vessel vasculitis (CSVV) or leukocytoclastic vasculitis (LCV) is a group of immune complex mediated vasculitides that affect dermal capillaries or post-capillary venules and classically presents as lower extremity palpable purpura. CSVV can be subdivided by antibody type and clinical features. In patients presenting with signs of LCV and ANCA positivity, clinicopathologic correlation is important in characterizing the type of vasculitis. We report an uncommon case of IgA vasculitis with concurrent ANCA-positivity attributed to bacterial endocarditis.

OBJECTIVE: The Hospice Comfort Questionnaire-Patients (HCQ-P) is widely used to assess patient's comfort levels in hospice care. This research aimed to culturally adapt the HCQ-P for the Chinese context and validate its psychometric properties to ensure its applicability and effectiveness in China.
METHODS: This research was conducted in two main phases: (1) translation and cross-cultural adaptation of the HCQ-P into Chinese, (2) evaluation of the psychometric properties through expert consultations and a cross-sectional survey among 360 hospice care patients. The evaluation included determin

BACKGROUND: The clinical profile of endocarditis has changed over the past four decades with studies showing trends towards increasing age, more nosocomial and prosthetic valve infection and increasing rates of Staphylococcus aureus infection. However, these studies have been biased by referral patterns.
METHODS: We reviewed data collected at three hospitals in the Barwon-South-West region in Victoria, Australia. All cases identified between 1994 and 1999 were reviewed according to the Duke criteria.
RESULTS: During this period, 58 patients were diagnosed as having endocarditis. The incidenc

We have encountered 2 patients in whom the first manifestations of bacterial endocarditis were arthritis (in 1 case septic arthritis and in the other nonseptic arthritis) and bacterial meningitis. These presentations were followed by acute heart failure due to aortic valve destruction, although the patients showed no significant cardiovascular manifestations on admission. Aortic valve replacement was performed in each case and the patients' postoperative course was comfortable. We would like to emphasize the following points. (1) Arthritis and meningitis are uncommon in patients with bacterial

In a population-based study of 41 children with bacterial endocarditis (BE), diagnosed in the period 1970 through 1989 in eastern Denmark, we analyzed trends in the diagnosis of BE and in mortality, and searched for possible prognostic factors. During this period the delay in diagnosis from first symptom to treatment did not change, but the delay from admission to treatment was significantly prolonged from 0 to 3 days, despite the introduction of echocardiography (ECHO). There was a significant improvement in the prognosis, the mortality rate having decreased from 40 to 0% [95% confidence limi

The authors describe the case of a 24-year-old woman with valve disease. After a bout of respiratory tract infection, she was diagnosed to have bacterial endocarditis associated with mitral valve disease. The causative agents isolated included Eikenella corrodens, Streptococcus intermedius, Bacteroides oralis and Bacteroides bivius. At the same time, the patient was found to have developed IgA immunodeficiency. A complication accompanying the cardiac disease was spontaneous pneumothorax. Since antibiotic therapy had failed, the mitral valve was replaced by a prosthetic one. After the procedure

We report on two observations of bacterial endocarditis complicating mitral valve prolapse. A review of the literature finds that endocarditis has complicated about 4 p. 100 of the cases of mitral valve prolapse that have been followed-up for 4 to 9 years. In one out of 4 such cases, a diagnostic delay has been held responsible for severe complications: arterial embolism, and cardiac failure with or without rupture of the chordae tendinae. Echocardiography, especially bidimensional, might lessen the diagnostic delay. Our analysis of the available reports indicates that young men with a late sy

The features of 70 cases of bacterial endocarditis are reported. Streptococcus viridans was the cause in 45 per cent, staphylococci in 27 per cent, and enterococci in 7 per cent. Rheumatic heart disease was the predisposing factor in less than one quarter of patients and in almost half there was no obvious pre-existing cardiac disease. Apart from cardiac murmurs and pyrexia, the classical features of infective endocarditis were uncommon, haematuria being observed in less than one-third. Most patients were over the age of 30, one-third more than 60 years of age and the peak incidence occurred i

Native valve infective endocarditis (IE) refers to infection resulting from the seeding of bacteria onto the heart valves, usually in patients with one or more risk factors such as intravenous drug use, poor dentition or recent dental surgery, prior structural or valvular heart disease, or indwelling cardiac devices. Although viridans group streptococci (VGS) are a common cause of aortic valve endocarditis, it is uncommon for the infection to result from bacterial translocation across the intestinal mucosa during gastroenteritis. This case involves a 64-year-old female who presented with an ac

Non-bacterial thrombotic endocarditis (NBTE) involves the deposition of fibrin and platelets on heart valves, frequently leading to systemic embolism. The association between NBTE and cancer demands thorough investigation in cases lacking an evident cause. This case report elucidates the clinical course of a nonsmoking woman in her sixties with NBTE linked to pulmonary adenocarcinoma. The patient, who had a history of multiple sclerosis (MS) and was receiving dimethyl fumarate treatment, presented to the emergency department with stroke-like symptoms. Diagnostic challenges arose due to preexis

A 66-year-old man with a large secundum atrial septal defect (ASD) that had been repaired percutaneously 30 months previously with an Amplatzer ASD occluder (ASO) presented with fevers and Staphylococcus aureus bacteremia. Transesophageal echocardiography revealed a 1.5 x 1.5 cm mobile mass on the left atrial side of his ASO consistent with a vegetation. When the ASO was explanted, the left atrial side of the device was poorly endothelialized. In conclusion, the present report is the first description of late infective endocarditis in an adult with an ASO.

We report a case of multiple signal loss lesions detected by T2* weighted image associated with bacterial endocarditis. A 23 year-old woman, who had undergone surgical closure of ventricular septal defect at age 2, suffered headache and general fatigue after dental treatment. She was admitted to another institution, and examinations revealed bacterial endcarditis. She was transferred to our hospital, and underwent aortic valve replacement. After the valve replacement, CT revealed parenchymal hematoma in the left frontal lobe. CT angiography (CTA) and cerebral angiography (CAG) demonstrated an

We report an illustrative case of a 60-year-old man with Streptococcus viridans subacute bacterial endocarditis (SBE) and positive antineutrophil cytoplasmic autoantibodies (c-ANCA). C-ANCA positivity has been associated with a variety of rheumatic and infectious disease areas, but has been rarely associated with SBE. The patient had mitral valve prolapse with mitral regurgitation, and S viridans SBE developed after a dental procedure. Laboratory abnormalities included anemia, elevated erythrocyte sedimentation rate, positive rheumatoid factor, positive anticardiolipin antibody, positive lupus

The clinical features of an infective embolic choroidopathy are described, from its early onset to late complications in a 45-year-old man with acute Staphylococcus aureus endocarditis of the aortic valve. Initial fundus examination revealed, in addition to fresh choroidal lesions, stigmata of a previous embolic episode secondary to endocarditis from Actinobacillus actinomycetemcomitans. The choroidal lesions were extremely asymmetrical, with a clear preference for localization in the left eye. Similar ocular findings were seen in a 78-year-old female with mitral valve prolapse and acute S. au

We investigated the association between mitral valve prolapse (MVP) and bacterial endocarditis (BE) in a case-control study of 56 patients with BE and 168 age- and sex-matched controls who had had echocardiography. Cases and controls were selected from patients without other known cardiovascular risk factors for BE. Twenty percent of the BE cases (11 of 56) and 4% of the controls (7 of 168) had MVP; the odds ratio of 5.3 (95% confidence interval 2.0 to 14.4) indicated a significantly greater risk of BE in patients with MVP. This increased those who had preexisting systolic murmurs (9 of 11). E

The capacity of bacterial endocarditis to mimic other systemic illnesses is well known. This report describes a patient with blood culture-negative bacterial endocarditis who presented with features suggestive of rheumatoid arthritis. Despite sterile synovial fluid, synovial biopsy culture resulted in identification of the causative organism and led to specific antibiotic therapy and cure. This is the first report of such a case.
