Infective endocarditis (IE) has been increasingly diagnosed in patients without previously detected predisposing heart disease, but its clinical features have yet to be fully determined. A recent single-center study including echocardiographic images and surgical findings investigated the incidence of undiagnosed, clinically silent valvular or congenital heart diseases and healthcare-associated infective endocarditis (HAIE). The study confirmed that a large proportion of patients with IE have no previous history of heart disease. Analysis of underlying disease in these patients showed that und

In 2008, National Institute for Health and Care Excellence (NICE) guidelines recommended against the use of antibiotic prophylaxis (AP) before invasive dental procedures (IDPs) to prevent infective endocarditis (IE). They did so because of lack of AP efficacy evidence and adverse reaction concerns. Consequently, NICE concluded AP was not cost-effective and should not be recommended. In 2015, NICE reviewed its guidance and continued to recommend against AP. However, it subsequently changed its wording to 'antibiotic prophylaxis against infective endocarditis is not routinely recommended'. The l

A 9-year-old child, with a background of repaired pulmonary atresia and Ebstein's anomaly, presented with fever, night sweats and lethargy. Blood cultures grew Granulicatella elegans, a nutritionally variant Streptococcus and known cause of infective endocarditis (IE). Echocardiogram revealed no clear vegetation, but increased stenosis of the right ventricle to pulmonary artery conduit. The child was successfully managed with high-dose benzylpenicillin, completing 2 weeks in the hospital, and was discharged to complete the final 4 weeks of therapy with ceftriaxone in the community, as per Europ

Moraxella catarrhalis is a gram-negative, catalase and oxidase positive diplococcus. While it causes otitis media, sinusitis, bronchitis and conjunctivitis in children and adults, it has a tendency to cause lower respiratory tract infections in older ages. More severe clinical pictures with the range of sepsis to endocarditis are also seen in immunocompromised patients. In this report, a case of M. catarrhalis endocarditis in an immunocompetent host who needed valve replacement has been presented. Forty three years old female patient was admitted to our hospital with the complaints of fever, n

From January 1981 through December 1991, 1120 consecutive aortic valves were surgically explanted and their gross anatomy and histology studied at our university. Rheumatic disease (65%), dystrophic calcific valvular disease (23%), noninflammatory disease of the aortic root and/or floppy aortic valve (6.3%), and endocarditis (5.4%) were the causes of valve dysfunction. Among the total population the male sex predominated. The male to female ratio was 2.4 in the group with dystrophic calcific valvular disease and 1.6 in the group with bacterial endocarditis. The mean age was 37 ± 7.5 years in t

BACKGROUND: Infective endocarditis (IE) is associated with high morbidity and mortality. However, the impact of geriatric syndromes on clinical outcomes in elderly patients with IE remains insufficiently characterized. The aim was to assess the prevalence of geriatric syndromes; focusing on frailty, functional dependence, comorbidity and malnutrition; and their association with in-hospital mortality in advanced-age patients with infective endocarditis.
METHODS: This prospective multicenter observational registry included patients aged ≥ 75 years with possible or definitive IE. Comprehensive g

Concomitant right- and left-sided infective endocarditis (IE) is an uncommon clinical entity, particularly when involving the Eustachian valve (EV), a vestigial remnant of the inferior vena cava. While right-sided IE is typically associated with intravenous drug use and indwelling cardiac devices, it can occur in non-traditional populations if anatomical niches are colonized. A 78-year-old female presented with heart failure and a "mixed" embolic signature, including pulmonary, renal, and cerebral infarcts, following a dental extraction. Imaging confirmed aortic valve endocarditis and a 1.5 cm

Kingella kingae, a rare member of the Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella (HACEK) group, is an uncommon cause of infective endocarditis (IE) in adults. Advances in molecular diagnostics, including polymerase chain reaction (PCR) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS), have significantly improved its identification. We present the case of a 78-year-old immunocompromised woman who developed watershed territory cerebral infarctions in the setting of K. kingae endocarditis. The patient initially presented w

Non-bacterial thrombotic endocarditis (NBTE) is characterized by presence of sterile vegetations that develop from fibrin and platelets on heart valves. The main conditions predisposing to NBTE are malignancy, autoimmune diseases and other hypercoagulable states. The authors describe a case of a 25-year-old male, in whom NBTE was diagnosed on the bicuspid aortic valve. The presence of significant aortic regurgitation and dental caries were initially suggestive of infective endocarditis; although, serial blood culture were negative and procalcytonin concentration was within normal ranges. BACKGROUND: Libman-Sacks endocarditis is a cardiac manifestation of systemic lupus erythematosus (SLE) and is characterized by non-bacterial verrucous vegetations, causing valvular stenosis and/or regurgitation. The effectiveness of immunosuppressive therapy for valve dysfunction due to Libman-Sacks endocarditis has not been reported.
CASE SUMMARY: A 67-year-old woman with a history of chronic atrial fibrillation was emergently admitted with acute decompensated heart failure. Transoesophageal echocardiogram revealed severe mitral regurgitation (MR) due to oedematous thickening and poor coapta

Behcet's disease is a systemic immune-mediated vasculitis predominantly affecting males. Cardiac involvement, termed cardiac Behcet's disease, is rare but potentially fatal, often mimicking infective endocarditis. This case presented with syncope as the initial symptom, leading to the identification of a non-coronary aortic sinus nodule, suspected as the cause of intermittent complete atrioventricular block. After pacemaker implantation, no recurrent syncope occurred, and immunosuppressive therapy reduced nodule size. This case underscores the importance of maintaining a high suspicion for car

Granulicatella species are rare, nutritionally variant streptococci that cause infective endocarditis. Their clinical and microbiological characteristics remain unknown. We reviewed five years of Granulicatella cases in our hospital database (Jan 2017-Jun 2022), finding 6 Granulicatella adiacens cases and 1 Granulicatella elegans case. Clinical backgrounds and bacteremia sources were diverse; 3 cases developed polymicrobial bacteremia. Antimicrobial testing showed non-susceptibility to penicillin G in 4 of 7 cases (57.1%), and high susceptibility to carbapenems and vancomycin in all cases. Det

Abiotrophia defectiva is a nutritional variant streptococcus, with affinity for endovascular structures that cannot be cultured on non-supplemented media, leading to a delay in diagnosis. This case describes a 36-year-old woman with a previously known bicuspid aortic valve that presented with asthenia, myalgias and anorexia with 2-month onset. On admission, we documented fever and a systolic murmur. Transthoracic echocardiography revealed an oscillating mass attached to the aortic valve. The patient was admitted with a possible diagnosis of infective endocarditis. Later, blood cultures were po

Staphylococcus caprae, a non-aureus staphylococcus primarily associated with goats, is typically regarded as a commensal organism. While its role in veterinary contexts is well established, its emergence as a human pathogen in healthcare settings has gained increasing recognition. It has been implicated in infections such as bacteremia, otitis externa, osteoarticular infections, and prosthetic device-related complications. However, infective endocarditis caused by S. caprae remains extremely rare, with only six cases reported in the literature since 1995. This report presents a case of native

Infective endocarditis is a significant healthcare burden due to the associated high mortality and complications. Endocarditis caused by both Candida albicans (C. albicans) and non-Candida albicans Candida (NCAC) species has been associated with a substantial rise in in-hospital morbidity and mortality. We used the Pubmed database to identify 47 out of 101 articles that had met our inclusion and exclusion criteria. We had put in place a broad inclusion criterion with no age or gender restrictions. These 47 articles included abstracts, 11 review articles, 13 case reports, 10 research articles,

BACKGROUND AND OBJECTIVE: Fungal infective endocarditis (FIE) is a rare but clinically important cause of endocarditis that poses unique challenges to diagnosis and treatment. As clinicians are faced with an aging population, increasing utilization of prosthetic valves and implantable cardiac devices, as well as increasing use of immunosuppressive therapies, which are known risk factors, familiarity with FIE is increasingly relevant. This narrative review presents the current state of knowledge concerning laboratory diagnosis, imaging tools, and treatment of FIE.
METHODS: A comprehensive lite

Infective endocarditis, a serious microbial infection of the cardiac endothelial surface, may involve any heart valve. However, right-sided endocarditis is uncommon in non-intravenous drug abusers without underlying heart disease, and the contextual involvement of the left-sided valve is exceptional. A 63-year-old man with no evidence of intravenous drug abuse or heart disease, presented with persistent fever, worsening of breath, and aphasia. His medical history was notable for mild arterial hypertension and serious lymphangitis with cutaneous erosions on the legs. Transesophageal echocardiog
