Performance of an artificial intelligence system versus endoscopists for interrogation of scars after piecemeal endoscopic mucosal resection.

BACKGROUND AND STUDY AIMS: Detection and management of recurrence after piecemeal endoscopic mucosal resection (p-EMR) of large colorectal polyps is crucial for preventing post-colonoscopy colorectal cancer. Colonoscopy assisted with artificial intelligence systems for lesion detection (CADe) has shown efficacy in identifying polyps, particularly small lesions (< 10 mm). Our aim was to evaluate sensitivity and negative predictive value of CADe compared with endoscopists in detecting recurrence after p-EMR. METHODS: Sixty-six pseudo anonymized high-quality videos (25 with recurrence and 15 wit

Octreotide for prevention of delayed bleeding after endoscopic mucosal resection for large superficial non-ampullary duodenal tumors.

BACKGROUND AND STUDY AIMS: Effective strategies to prevent delayed bleeding (DB) following endoscopic mucosal resection (EMR) for large superficial non-ampullary duodenal tumors (SNADTs) remain limited. Octreotide exerts pharmacological effects that may influence hemostasis and thereby reduce bleeding risk. This exploratory study aimed to evaluate the association between postprocedural octreotide use and DB after duodenal EMR for large SNADTs. PATIENTS AND METHODS: This single-center, exploratory, retrospective study included all hot EMRs for large SNADTs ≥ 10 mm (2015-2024). Routine post-pro

Extended surveillance after piecemeal endoscopic mucosal resection: a safe approach to initial surveillance in low-risk patients.

BACKGROUND AND AIMS: Piecemeal endoscopic mucosal resection (EMR) is the standard of care for large, nonpedunculated colon polyps but is associated with recurrence rates of 9% to 31%. Current guidelines recommend 6-month surveillance for all patients, although this may not be necessary for lower-risk cases. METHODS: We retrospectively reviewed patients who underwent piecemeal EMR of ≥20-mm colon polyps between 2018 and 2021. Patients were stratified into 6- or 12-month surveillance groups based on polyp features. Recurrence rates and associated factors were compared. RESULTS: Among 193 patie

Reconstruction of a skull base defect after endoscopic endonasal resection of a pituitary adenoma: Sphenoid mucosal flaps.

This report describes a bilateral sphenoid sinus mucosal flap for the repair of a sellar floor defect and CSF leak following endoscopic endonasal skull base surgery. The key advantage of this technique is enabling the sphenoid mucosal flaps to remain vascularized, which reduces postoperative complications including CSF leakage, recurrent sinusitis, meningitis, encephalitis and pneumocephalus. The use of this technique is a viable and possibly favorable alternative to free grafts in the reconstruction of small to medium sized sellar defects with low flow or absent CSF leaks base surgery.

Recurrent oesophageal intramucosal squamous carcinoma treated by endoscopic mucosal resection and subsequent radiofrequency ablation using HALO system.

The method of radiofrequency ablation (RFA) is currently used for the treatment of high-grade dysplasia in Barrett's oesophagus. It has theoretical potential also for the use in squamous epithelial neoplasias. The authors present a case report of an early diagnosis of squamous cancer in a high-risk patient, its endoscopic treatment and follow-up, and successful RFA of recurrent neoplasia. RFA can expand our therapeutic possibilities for the management of recurrent neoplastic lesions after endoscopic treatment of squamous oesophageal cancer.

Where has the tumor gone? The characteristics of cases of negative pathologic diagnosis after endoscopic mucosal resection.

BACKGROUND AND STUDY AIMS: Discrepancies can occur between the histopathological findings from forceps biopsy and endoscopic mucosal resection (EMR), and occasionally in embarrassing cases tumorous tissue is not found at EMR. The aim of the present study was to evaluate the clinical, endoscopic, and histological features of gastric tumors in patients with pathololgically negative findings at EMR. PATIENTS AND METHODS: We retrospectively reviewed data from all patients with gastric tumor treated with EMR or endoscopic submucosal dissection (ESD) between August 1999 and April 2007 at our instit

[Non-healing iatrogenic gastric ulcers after endoscopic mucosal resection for gastric epithelial neoplasia: report of two cases].

Endoscopic mucosal resection (EMR) is widely accepted as a standard treatment for early gastric cancer or gastric adenoma. However, EMR inevitably results in the formation of large iatrogenic ulcer at the resected area. Although the characteristics of EMR-induced ulceration are not fully understood, this type of ulcer is thought to heal faster and to recur less often than non-iatrogenic gastric ulcer. Current available evidences have suggested that EMR-induced ulcers heal within 2-3 months. Herein, we report two cases of non-healing persistent gastric ulcers after EMR. One is a case of gastric

Helicobacter pylori eradication therapy may facilitate gastric ulcer healing after endoscopic mucosal resection: a prospective randomized study.

BACKGROUND AND AIM: It remains unclear whether Helicobacter pylori eradication therapy affects the healing rate of iatrogenic ulcers following endoscopic mucosal resection (EMR) for gastric tumors. The aim of our study was to prospectively evaluate the effect of H. pylori eradication therapy on gastric ulcer healing after EMR. METHODS: After EMR, patients were randomly assigned to either the H. pylori eradication group (Hp group) (lansoprazole 30 mg, amoxicillin 1000 mg, and clarithromycin 500 mg, twice a day for 7 days) or the noneradication group (proton pump inhibitor, PPI group) (lansopra

Superselective transcatheter embolization for acute lower gastrointestinal hemorrhage after endoscopic mucosal resection: a report of 3 cases.

One of the most common complications related to endoscopic mucosal resection is hemorrhage; in almost every case, the bleeding is endoscopically managed, but some cases are unresponsive to the treatment. We have encountered three cases of endoscopically uncontrollable lower gastrointestinal hemorrhage after endoscopic mucosal resection in the colon which we successfully treated by urgent superselective transcatheter embolization. In our three cases, massive hemorrhage occurred immediately after endoscopic mucosal resection of a sessile polyp 25-40 mm in diameter, two cases in the ascending col

Long-term outcomes of intragastric endoscopic mucosal resection using a modified buess technique for early gastric cancer.

BACKGROUND/AIMS: The aim of this study was to clarify long-term outcomes of endoscopic mucosal resection (EMR) using a modified Buess technique for mucosal gastric cancer. METHODS: The procedure included en bloc mucosal resection using a Buess-type rectoscope into the gastric lumen via a temporary gastrostomy under video camera guidance. We reviewed 5 patients who underwent this procedure between 1995 and 1997 with a single mucosal cancer of the stomach that was unsuitable for en bloc excision by endoscopic snare diathermy. RESULTS: All tumors were superficial elevated types (type 0-IIa) loc

[A case of multiple early gastric cancer showing complete response to UFT-E after endoscopic mucosal resection].

A 78-year-old male underwent endoscopic mucosal resection for early gastric cancer (type II a) in the anterior wall of antrum. Follow-up endoscopy showed another early gastric cancer (type II c) in the posterior wall of cardia. He refused an operation. UFT-E 300 mg/day has been administered daily since then. After 6 months, a complete tumor response was observed. The patient has been in good health without any sign of relapse for 14 months after achievement of the complete response.

Endoscopic mucosal resection (EMR) versus endoscopic submucosal dissection (ESD) for resection of large distal non-pedunculated colorectal adenomas (MATILDA-trial): rationale and design of a multicenter randomized clinical trial.

BACKGROUND: Endoscopic mucosal resection (EMR) is currently the most used technique for resection of large distal colorectal polyps. However, in large lesions EMR can often only be performed in a piecemeal fashion resulting in relatively low radical (R0)-resection rates and high recurrence rates. Endoscopic submucosal dissection (ESD) is a newer procedure that is more difficult resulting in a longer procedural time, but is promising due to the high en-bloc resection rates and the very low recurrence rates. We aim to evaluate the (cost-)effectiveness of ESD against EMR on both short (i.e. 6 mon

Comparison between ligation-assisted and precutting endoscopic mucosal resection for small rectal neuroendocrine tumors: a CHASID study.

BACKGROUND/AIMS: Few studies have compared modified endoscopic mucosal resection (EMR) techniques for small rectal neuroendocrine tumors (NETs). This study evaluated the clinical outcomes of ligation-assisted EMR (EMR-L) and precutting EMR (EMR-P) for the treatment of small rectal NETs. METHODS: We retrospectively reviewed patients who underwent EMR-L or EMR-P for rectal NETs ≤ 10 mm at four university hospitals between January 2018 and December 2022. Primary outcomes were en bloc resection and complete resection rates; secondary outcomes included procedural time and complications. RESULTS:

Clinical outcomes of endoscopic mucosal resection for large superficial nonampullary duodenal epithelial tumor: a single-center study.

BACKGROUND AND AIM: Endoscopic mucosal resection (EMR) of superficial nonampullary duodenal epithelial tumors (SNADETs) is challenging, and to date, only a few studies assessed the clinical outcomes of EMR in the duodenum. The aim of this study was to evaluate the efficacy and safety of EMR for the treatment of SNADETs >10 mm. PATIENTS AND METHODS: This is a single-center retrospective study reporting data from a cohort of consecutive patients undergoing EMR of large (>1 cm) SNADETs between January 2017 and December 2021. RESULTS: A total of 81 patients with 83 lesions underwent EMR (70 conv

Through-the-scope suture closure of nonampullary duodenal endoscopic mucosal resection defects: a retrospective multicenter cohort study.

BACKGROUND: Delayed bleeding is among the most common adverse events associated with endoscopic mucosal resection (EMR) of nonampullary duodenal polyps. We evaluated the rate of delayed bleeding and complete defect closure using a novel through-the-scope (TTS) suturing system for the closure of duodenal EMR defects. METHODS: We reviewed the electronic medical records of patients who underwent EMR for nonampullary duodenal polyps of ≥ 10 mm and prophylactic defect closure with TTS suturing between March 2021 and May 2022 at centers in the USA. We evaluated the rates of delayed bleeding and com

Follow-up outcomes of mucosal defect closures after endoscopic resection using a helix tacking system and endoclips.

BACKGROUND AND AIMS: The X-Tack endoscopic HeliX tacking system (Apollo Endosurgery, Austin, Tex, USA) has recently been approved by the Food and Drug Administration and is slowly gaining popularity for the closure of large tissue defects. Despite its increasing use, outcome data of using the X-Tack system for mucosal defect closure after endoscopic resection (ER) are limited. Here, we report the follow-up outcomes of a series of cases that underwent ER and mucosal closure aided by the HeliX tacking system. METHODS: We identified a total of 5 cases in which the X-Tack system and endoclips wer
