Subdural empyema and epidural abscess: recent experience in a community hospital.

We treated 31 cases of localized central nervous system infection over a seven-year period in our community hospital. The causes included brain abscess in 18 cases (58%); cranial subdural empyema (CSE) in six cases (20%); spinal epidural abscess (SEA) in four cases (13%); cranial epidural abscess (CEA) in two cases (6%); and spinal subdural empyema (SSE) in one case (3%). Both CSE and CEA were often caused by sinusitis and manifested by fever, headache, altered sensorium, and focal neurologic signs. Treatment consisted of drainage by burr holes or craniotomy followed by long-term administratio

[Spinal epidural abscess in an 11-year-old boy (author's transl)].

Spinal epidural abscesses can occur in childhood. Neurological defects are late manifestations in the course of this disease. Recognition of the early symptoms followed by immediate neurosurgical intervention can prevent these defects. This report describes the course of an epidural abscess in an 11 year old boy. The painful febrile spinal syndrome of the cervical column was the outpointing initial symptom. Lateron disturbances of the autonomic nerve function appeared, such as acute abdomen. The differential diagnosis of the early symptoms is discussed.

Epidural Abscess Complicating Tunneled Caudal Epidural Catheter in an Infant for Postoperative Pain Management of Open Abdominal Surgery.

Regional anesthesia is being used more frequently in pediatric anesthesia practice, including the perioperative care of neonates and infants. Adverse effects may be encountered during epidural needle placement, with catheter advancement, or subsequently during infusion of local anesthetic agents. Despite applying standard practice of care regarding placement of epidural catheter, epidural catheter-related infections may still occur. We present the rare occurrence of an epidural abscess in a 4-month-old infant after placement and subsequent use of a tunneled caudal epidural catheter for postope

How to Avoid Misdiagnosing Primary Spinal Epidural Abscess as Spinal Epidural Hematoma: A Case Report and Literature Review.

Spinal epidural abscess (SEA) is an easily misdiagnosed disease. We report a case of primary SEA (PSEA) that was misdiagnosed as spinal epidural hematoma (SEH) and analyze the reasons. The misdiagnosis was confirmed intraoperatively. Comprehensive analysis is crucial for the diagnosis of PSEA and its differential diagnosis from SEH.

Tubercular spondylitis with prevertebral and epidural abscess masquerading as a mediastinal mass.

A 10-year old girl presenting with an insidious-onset low-grade fever for one month was found to have mediastinal widening with bilateral perihilar opacities. MRI scanning further revealed dorsal spondylitis with collapse of T2-4 vertebral bodies with prevertebral and epidural tubercular collections of 3 × 6.5 × 8 cm. This case emphasises the importance of considering TB among differential diagnoses of a mediastinal mass, especially among children from endemic countries.

A case of spinal epidural abscess concealed by delirium in a young man.

Spinal epidural abscess is a rare but serious condition with poor outcomes. It's classic triad of new back pain, neurological deficit and fever is only present in 15% of cases at presentation and is initially misdiagnosed in 75-89%.6,7 Delaying treatment is associated with worse outcomes. Delirium is itself a risk factor for mortality but the disturbance in cognition and memory can also complicate clinical assessment.1-5 We present a case of delirium caused by, and obscuring, a spinal epidural abscess. This case highlights the difficulties in diagnosing spinal epidural abscesses, the need for

Nocardial epidural abscess of the thoracic spinal cord and review of the literature.

Nocardia is a potential cause of opportunistic infection in organ-transplant recipients. We report an epidural abscess causing acute paraparesis in a patient with a rejected renal transplant and chronic liver disease. Early drainage and treatment with imipenem and trimethoprim-sulfamethoxasole improved the patient's neurologic status.

Catheter-related epidural abscesses -- don't wait for neurological deficits.

Epidural abscess is a rare but serious complication of epidural anesthesia for peri- and postoperative analgesia. It is feared because of possible persistent neurological deficits. Epidural abscess presents mostly with a classic triad of symptoms: back pain, fever and variable neurological signs and symptoms. When neurologic signs or symptoms develop, MRI scanning is the diagnostic procedure of choice. The therapy of choice is intravenous antibiotics for more than 4 weeks with or without a laminectomy or drainage. In the present paper we describe three patients with epidural abscesses presente

The silent epidural abscess as a complication of acute otitis media in children.

Acute otitis media with complications is a persistent problem in the modern antibiotic era with a relatively high mortality rate. Acute mastoiditis is a serious complication of acute otitis media with potentially grave consequences and the epidural abscess constitutes the commonest of all intracranial complications, arising from middle ear infections. We report two cases of children with acute mastoiditis without evidence of intracranial complication or subperiosteal abscess, in whom the early evaluation with computed tomography (CT) disclosed an unsuspected epidural abscess. Therefore, we str

Proteus penneri isolated from the pus of a patient with epidural abscess.

P. mirabilis and P. vulgaris are the two wellknown species in the genus Proteus. P. myxofaciens and P. penneri are recent additions to the genus. We isolated P. penneri from the pus of a patient with suppurative otitis media and an epidural abscess. The characteristics of the organism, including morphology, staining, physiology and biochemistry, were studied. Clinical microbiological laboratories should suspect P. penneri in the case of as Proteus strain that is negative for indole, salicin and esculin, but otherwise resembles P. vulgaris. Update on spinal epidural abscess: 35 cases and review of the literature.

Thirty-five cases of spinal epidural abscess were evaluated retrospectively and compared with 153 cases reported in the literature. As in other series, Staphylococcus aureus was the major pathogen. Patients with acute abscesses had fever, leukocytosis, and purulence at surgery. Patients with chronic abscesses had less fever and leukocytosis, more granulation tissue at surgery, and a greater delay in diagnosis but retained the potential for rapid neurologic deterioration. In contrast to other studies, abscesses in both the lumbar and anterior spinal compartments and patients with specific sourc

An uncommon presentation of multiple spinal epidural abscesses: A case report.

INTRODUCTION: Spinal epidural abscess (SEA) is a rare but severe infection occurring in the epidural space of the spinal cord, increasingly associated with invasive spinal procedures and intravenous drug use. Recognizing and accurately diagnosing SEA is crucial due to its potential neurological impact. PRESENTATION OF CASE: A 51-year-old male presented with urinary and bowel obstruction, right-sided hemiplegia, fever, and jaundice. Neurological examination revealed pronounced deficits, while imaging and cerebrospinal fluid analysis confirmed multiple epidural abscesses. Cultures identified St

[An enormous epidural abscess secondary to cranial metastasis of a lung tumor].

Epidural abscess most often develops as a result of otitis media, sinusitis, and mastoiditis. In these cases, most of the time, pathogens enter the epidural space through the venous and arterial system, while during open fractures or neurosurgery, bacteria can also enter the skull bone and dura directly. Our case report can be considered a literary rarity, since the destruction of the cranium – caused by the metastasis of a lung tumor – played a role in the development of the enormous epidural abscess, which opened the non-sterile sinuses and mastoid system in the direction of the epidural spa

Not All Back Pain Is Muscle Strain: A Case of Epidural Abscess.

Epidural abscesses are rare suppurative abscesses of the central nervous system that can expand and lead to severe neurologic complications and even death. Here we describe the case of a 68-year-old female who developed a spinal epidural abscess one month following cervical spinal decompression and fusion. The patient presented with decreased grip strength, flaccid paralysis of the lower extremities with hyporeflexia, urinary incontinence, and decreased sensation in the bilateral lower extremities. A cervical spine MRI revealed a large cervical spinal epidural abscess causing multilevel spinal

Aspergillus spinal epidural abscess: A case report and review of the literature.

Aspergillus spinal epidural abscess (ASEA) is a rare entity that may mimic Pott's paraplegia as it commonly affects immunocompromised patients. We present one institutional case of ASEA with concomitant review of the literature. A 58-year-old female presented with intermittent low back pain for 10 years recently aggravated and with concurrent spastic paraparesis, fever, and weight loss. Emergent magnetic resonance imaging (MRI) showed T11-T12 epidural abscess with discitis and osteomyelitis. After empirical treatment with antibiotics, computed tomography-guided, percutaneous biopsy with draina
