Developing a non-pharmacological pain management toolkit in nursing homes using a structured evidence-generation approach.

INTRODUCTION: Pain is a prevalent issue in nursing home residents. Despite the importance of non-pharmacological interventions, they are underused compared to pharmacological treatments, and the respective evidence-based guidelines have hardly been implemented in daily practice. This study was carried out to collaboratively develop a practical, user-friendly toolkit with evidence-based non-pharmacological pain management interventions for improving care for nursing home residents in pain. METHODS: The toolkit was developed following a systematic approach and including nursing staff from the o

Central Venous Catheter Flushing Recommendations: A Systematic Evidence-Based Practice Review.

BACKGROUND: Treatment for many children with blood disorders or cancer includes the use of central venous catheters (CVCs). Few prospective studies have been conducted to address flushing guidelines in pediatric hematology oncology patients. Eighteen pediatric hematology oncology units were surveyed regarding current CVC flushing policies and procedures. Results reported extreme variations in CVC flush procedures, which instigated this systematic review. AIMS: The purpose of this project was to critically review current literature and expert opinion regarding CVC flushing practice in the hope

A policy change strategy for head lice management.

The purpose of this project was to formulate an effective change strategy for head lice management in a group of five separate school districts within one county. Despite a desire to use evidence to support their practice, school nurses often encounter educational system barriers that prevent independent management of health conditions. The use of collaborative community relationships, identification of underlying hierarchy structures in school policy development, and targeted system education were the catalysts for changing beliefs and ultimately policy within these school districts. The focu

Integration of Medication-Assisted Treatment Information for Opioid Use Disorders Into a Graduate Nursing Curriculum.

PURPOSE: The United States is in the midst of an unprecedented public health crisis, with annual morbidity and mortality data related to opioid use consistently increasing and appearing more worrisome. To mitigate such consequences, it is critical that those with opioid use disorders are provided with and have access to evidence-based treatment modalities. METHODS: The project utilized a course scaffolding approach to integrate a comprehensive substance use framework into an advanced practice nursing curriculum, with an emphasis on medication-assisted treatment as part of an advanced pharmaco

Basic newborn resuscitation guidelines for healthcare providers in Maragua District Hospital: a best practice implementation project.

OBJECTIVE: The overall aim of this project was to assess compliance with evidence-based criteria regarding newborn resuscitation among healthcare workers in the maternity units in Maragua District Hospital (a busy district hospital in Kenya). INTRODUCTION: Globally, the majority of deaths under five years are attributable to neonatal causes. Perinatal asphyxia accounts for a significant portion of these deaths, especially in developing countries. These deaths are largely preventable, and interventions geared towards assisting newborns to initiate their first breath within the first minute of

OBJECTIVE: The current implementation project aimed to promote evidence-based practice with prechemotherapy nursing assessment among adult cancer patients in a large university cancer center in Shanghai, China, over a 6-month timeframe. INTRODUCTION: Prechemotherapy nursing assessment is an essential element of cancer patient care, aiming to prevent or minimize potential problems from chemotherapy treatment. Regular comprehensive prechemotherapy assessment is not part of routine care currently in many clinical settings within China. METHODS: The project utilized the JBI approach to implement

Clinical Opioid Withdrawal Scale (COWS): Implementation and Outcomes.

The opioid crisis has shown evidence worldwide and locally. There was no protocol to detect opioid withdrawal at this organization. The purpose of this project was to develop a nursing-driven opioid withdrawal management tool using the validated Clinical Opioid Withdrawal Scale (COWS) to address an increase in opioid-addicted patients. Evidence-based practice change was executed using the San Diego 8A's method for adult trauma step-down patients experiencing opioid withdrawal with the COWS tool affecting frequency of nursing attempts at managing opioid withdrawal over an 8-week period. Evidence-Based Education for Perianesthesia RNs to Increase Knowledge and Change Attitudes of Electroconvulsive Therapy.

PURPOSE: To increase perianesthesia registered nurses (RNs') knowledge and change attitudes of electroconvulsive therapy (ECT). DESIGN: A pretest/post-test design was used. METHODS: An interprofessional team collaborated in the development of a 1-hour teaching session, which included PowerPoint presentation, case studies, and online resources. The education was offered four times from December 2017 to April 2018. Data were collected on self-reported learning outcomes and the Questionnaire on Attitudes and Knowledge of ECT. FINDINGS: After the education, increased knowledge was reported. RNs

Development of clinical practice guidelines for patient management of blood pressure instability in multiple system atrophy, Parkinson's disease, and other neurological disorders.

Multiple system atrophy (MSA) is a progressive, neurodegenerative disorder. Among its range of multisystem dysfunctions, manifestation of highly unstable blood pressure (BP) is often a primary clinical concern for practitioners and patients. The ability to manage BP instability has important implications ranging from patient comfort, safety, and choice of treatments to the incidental demands placed on a strained health care system. Many conditions require that patients monitor their BP, yet no guidelines are available for patients with MSA who have similar needs. Utilization of a self-care pro

Effectiveness of a perinatal and pediatric End-of-Life Nursing Education Consortium (ELNEC) curricula integration.

BACKGROUND: Educational practices and national guidelines for best practices of providing palliative care to children and their families have been developed and are gaining support; however, the dissemination of those practices lags behind expectations. Incorporating education for pediatric palliative care into nursing pre-licensure programs will provide guidelines for best practices with opportunities to enact them prior to graduation. OBJECTIVE: To evaluate the effect of an integrated curriculum for palliative care on nursing students' knowledge. DESIGN: Matched pretest-posttest. SETTING:

Demystifying pediculosis: school nurses taking the lead.

The treatment of Pediculosis capitis, or head lice, is fraught with misinformation, myths, and mismanagement. Common myths include the need to exclude children from school, the need to remove all visible nits ("no-nit" policies), the need for massive environmental cleaning, that head lice live for long periods of time, and that schools are a common location for lice transmission. Head lice are a common childhood nuisance, causing embarrassment and emotional trauma in both children and families. This article explores and challenges the commonly held beliefs about the identification, management,

Comparison of Neonatal Nursing Practices for Determining Feeding Tube Insertion Length and Verifying Gastric Placement With Current Best Evidence.

BACKGROUND: Oral-nasogastric feeding tubes (FTs) are often malpositioned, placing infants at risk for complications. Confusion exists regarding the accuracy of methods to determine FT insertion length and verify gastric FT placement, and it is unknown whether evidence-based methods are used by neonatal nurses. PURPOSE: To compare individual and unit-based neonatal nursing practices regarding methods used to determine FT insertion length and verify gastric FT placement. METHODS: Neonatal nurses were surveyed about individual and unit-based practices regarding methods used to determine FT inse

Dysphagia and aspiration pneumonia in older adults.

PURPOSE: To review the risks, diagnosis, management, and prevention of aspiration pneumonia in adults over age 65, and discuss the major role of dysphagia as a risk factor in this population. DATA SOURCES: A comprehensive search of the literature using PubMed (MedLine) and CINAHL. CONCLUSIONS: Aspiration pneumonia is a common diagnosis in older adults and is associated with significant morbidity and mortality. Oropharyngeal and esophageal dysphagia, often related to stroke, dementia, poor oral hygiene, or multiple chronic illnesses, increases the risk of aspiration. Nurse practitioners (NPs)
