A 42-yr-old man who exsanguinated from an acute upper gastrointestinal bleed was found to have a primary aortoduodenal fistula on postmortem examination. The fistula arose in an aortic pseudoaneurysm associated with cystic medial necrosis. Although there was no suggestion of Marfan's syndrome on physical examination, there was cystic medial necrosis of not only the involved aorta, but also other systemic arteries. Vasa previa was suspected when sudden bleeding occurred in a twin pregnancy during an attempt to attach a scalp electrode. Shortly thereafter, we observed sustained fetal bradycardia using a portable ultrasound. Delivery was accomplished immediately with cesarean section, and an exsanguinated twin A and a normal twin B were delivered. After appropriate resuscitation and transfusion, twin A recovered, and both neonates appear normal at the age of 6 months. Congenital syndactylia of the fingers are discussed and the differential diagnosis is presented. While correction may be required for functional or easthetic purposes, the age of 4-6 years is recommended as the optimum time for correction. Early surgical treatment may be needed in cases of Apert's syndrome and in the case of congenital annular scars (amniotic bands). The operative technique is carefully described and general anaesthesia and exsanguination is recommended. Post-operative management is of equal importance and the fingers are dressed with vaseline gauze and a compression bandage.

We report a method designed to improve the specificity of tumour uptake after intravenous injection of an anti-tumour monoclonal antibody (MAb). It consists in increasing the blood clearance of the MAb injected in order to diminish its tissue activity, without altering tumour binding. BACKGROUND: Junctional hemorrhage control device use on the battlefield might be lifesaving, but little experience is reported. The purpose of the present case report is to detail prehospital use of the Combat Ready Clamp (called the CRoC by its users, Combat Medical Systems, Fayetteville, NC; Instructions for Use, 2010) in casualty care in order to increase awareness of junctional hemorrhage control.
