Modified Gundersen Flap Using Inferior Palpebral-Bulbar Conjunctiva.

PURPOSE: The purpose of this article was to describe a modification to the traditional Gundersen flap technique that expands eligible eyes to include those with severe conjunctival scarring and to report results from a clinical case. METHODS: A 68-year-old woman with a history of herpes simplex keratitis, multiple failed penetrating keratoplasties, lagophthalmos, persistent epithelial defect, and low visual potential in the left eye presented for the evaluation of a Gundersen flap. Severe superior bulbar conjunctival scarring precluded a traditional approach. A modified Gundersen flap techniq

Hypermucoviscous Klebsiella corneal abscess: Infection from inside out.

BackgroundCorneal ulcers are typically caused by external ("exogenous") factors such as trauma, contact lens use, or preexisting ocular surface diseases. Internal systemic ("endogenous") sources have not been reported as direct causes of corneal ulcers. Here, we report, to our knowledge, the first documented case of a corneoscleral abscess that resulted from an underlying bacteremia.MethodThis is a single case report.ResultA previously healthy 35-year-old male presented to the Emergency Department with left eye redness, pain, and blurred vision. Initial examination showed reduced visual acuity

Integrated versus non-integrated orbital implants for treating anophthalmic sockets.

BACKGROUND: Anophthalmia is the absence of one or both eyes, and it can be congenital (i.e. a birth defect) or acquired later in life. There are two main types of orbital implant: integrated, whereby the implant receives a blood supply from the body that allows for the integration of the prosthesis within the tissue; and non-integrated, where the implant remains separate. Despite the remarkable progress in anophthalmic socket reconstruction and in the development of various types of implants, there are still uncertainties about the real roles of integrated (hydroxyapatite (HA), porous polyethy

Decrease in levator function in the anophthalmic orbit.

BACKGROUND/AIMS: It has been argued that levator function in anophthalmic patients is reduced, but no detailed report on this topic has been issued. This study was undertaken to investigate measured levator function in anophthalmic patients, and to determine how measured values are related to prosthesis size. METHODS: Unilateral anophthalmic patients with no significant ptosis, and no history of trauma or surgery to orbit or eyelid were recruited. Levator function was measured with patients wearing their prosthesis, test prosthesis of different heights, and no prosthesis. RESULTS: Thirty-two

Pleomorphic adenocarcinoma of the ciliary epithelium: a clinicopathological, immunohistochemical, ultrastructural, DNA-ploidy and comparative genomic hybridization analysis of an unusual case.

PURPOSE: To describe detailed phenotypic and genotypic analysis of a pleomorphic adenocarcinoma of the ciliary epithelium (CE). CASE REPORT: An 86-year-old white woman developed an enlarging mass protruding from her previously eviscerated left eye 2 months postoperatively. Based on light and ultrastructural microscopy, the final diagnosis was a pleomorphic adenocarcinoma of the ciliary epithelium (CE). DISCUSSION: Cell proliferation indices confirmed the unusually rapid growth rate of this tumor; the peridiploid DNA content might explain the relatively low incidence of distant metastases. An

Trauma-related eye removal is associated with increased psychiatric morbidity compared to other surgical indications: a retrospective cohort study.

BACKGROUND/AIMS: To evaluate psychiatric outcomes following enucleation or evisceration (EE) and determine whether these outcomes differ by the underlying aetiology related to surgery, including trauma, blind painful eye, infection and malignancy. METHODS: This retrospective cohort study utilised the TriNetX Research Network, analysing deidentified records of 20 109 patients who underwent EE. Psychiatric outcomes were compared with 325 108 patients who underwent other major ocular surgeries. Patients were propensity-score matched, and EE cases were stratified into trauma, blind painful eye, i

The Birmingham Eye Trauma Terminology - Extension from Lebanon (BETT-XL) classification: A multi-center validation in modern warfare.

BACKGROUND: In modern warfare, ocular injuries frequently result in severe injuries that challenge existing classification systems. The standardized Birmingham Eye Trauma Terminology (BETT) does not adequately capture the extensive globe or periocular damage encountered in conflict settings. We aimed to develop an extension to BETT capable of classifying the full spectrum of injuries. We further sought to validate the extended framework using a real-world cohort from the 2024 pagers explosions in Lebanon. METHODS: Phase I included a targeted literature review of severe ocular trauma cases unc

Successful Repair of Glaucoma Drainage Device Tube Exposure With a Corneal Patch Graft and Double-Layer Amniotic Membrane: A Case Report.

Tube exposure is a recognised complication of glaucoma drainage devices (GDD). We report a case of successful repair of GDD tube exposure using a corneal patch graft and double-layer amniotic membrane in a visually precious eye with previous multiple surgeries. A 47-year-old man with juvenile open-angle glaucoma (JOAG) underwent trabeculectomy in both eyes (OU) and two GDD implantations in the right eye (OD). The left eye (OS) trabeculectomy failed, leading to panophthalmitis and evisceration. Six years after the second GDD in the OD, the superonasal tube became exposed (3.8 mm). Initial repai
