Intensified lipid lowering using ezetimibe after publication of the IMPROVE-IT trial: A contemporary analysis from the SPUM-ACS cohort.

BACKGROUND: The relevance of the IMPROVE-IT trial on real-life practice has not been explored in patients with ACS. METHODS: A prospective Swiss cohort of 6266 patients hospitalized for ACS between 2009 and 2017 with a one-year follow-up. The primary endpoints were the ezetimibe use overall or in combination with high-intensity statin at discharge and at one year after ACS. Secondary endpoint was LDL-C target achievement at one year in a subsample of 2984 patients. Relative Ratios (RR) were used to assess changes in primary endpoints before and after the publication of IMPROVE-IT, adjusting f

Lipid-Lowering Therapy With Ezetimibe Decreases Spontaneous Atherothrombotic Occlusions in a Rabbit Model of Plaque Erosion: A Role of Serum Oxysterols.

OBJECTIVE: Plaque erosion is increasing its importance as one of the mechanisms of acute coronary syndromes in this statin era. However, the clinical efficacy of currently used lipid-lowering agents in the prevention of thrombotic complications associated with plaque erosion has not been clarified. Therefore, we examined the therapeutic effects of ezetimibe or rosuvastatin monotherapy on spontaneous atherothrombotic occlusion. APPROACH AND RESULTS: Femoral arteries of Japanese white rabbits, fed a high-cholesterol diet, were injured by balloon catheter, and then angiotensin II was continuousl

Effect Modifications of Lipid-Lowering Therapy on Progression of Aortic Stenosis (from the Simvastatin and Ezetimibe in Aortic Stenosis [SEAS] Study).

Observational studies indicate that low-density lipoprotein (LDL) cholesterol acts as a primary contributor to an active process leading to aortic stenosis (AS) development. However, randomized clinical trials have failed to demonstrate an effect of lipid lowering on impeding AS progression. This study explored if pretreatment LDL levels and AS severity altered the efficacy of lipid-lowering therapy. The study goal was evaluated in the analysis of surviving patients with baseline data in the Simvastatin and Ezetimibe in Aortic Stenosis (SEAS) trial of 1,873 asymptomatic patients with mild-to-m

Usefulness of the electrocardiogram in predicting cardiovascular mortality in asymptomatic adults with aortic stenosis (from the Simvastatin and Ezetimibe in Aortic Stenosis Study).

Hypertension and coronary heart disease are common in aortic stenosis (AS) and may impair prognosis for similar AS severity. Different changes in the electrocardiogram may be reflective of the separate impacts of AS, hypertension, and coronary heart disease, which could lead to enhanced risk stratification in AS. The aim of this study was therefore to examine if combining prognostically relevant electrocardiographic (ECG) findings improves prediction of cardiovascular mortality in asymptomatic AS. All patients with baseline electrocardiograms in the SEAS study were included. The primary end po

High-intensity versus very-high-intensity statin, both on top of ezetimibe, in acute myocardial infarction: A pragmatic randomized study.

BACKGROUND: Early lipid-lowering therapy with "high-intensity" statins is recommended post-acute myocardial infarction (AMI), yet the optimal dose within such regimens remains uncertain. We compared efficacy and safety of very high-intensity versus high-intensity statin regimens, both combined with ezetimibe, initiated during AMI hospitalization. METHODS: In a stepped-wedge cluster randomized controlled single-center trial, patients with ST- or non-ST-elevation AMI were assigned in sequential 6-month blocks to either very high-intensity statin (Group A: atorvastatin 80 mg or rosuvastatin 40 m

Targeting the AMPK/SIRT1-NLRP3 inflammasome axis: Simvastatin, Ezetimibe, and their combination attenuate 3-nitropropionic acid-induced striatal neurodegeneration in rats.

Striatal neurodegeneration, a prominent pathological feature of several neurological disorders, is associated with progressive behavioral decline alongside cognitive and motor dysfunction, reflecting the critical regulatory role of the striatum in coordinating multiple aspects of brain function. Neuroinflammation, particularly through NF-κB/NLRP3-pathway activation, which triggers the cleavage and subsequent secretion of IL-1β/IL-18, contributes substantially to neuronal injury, with glial activation serving as a key driver, whereas the AMPK/SIRT1-signaling cascade exerts anti-inflammatory and

Ezetimibe protects against alpha-amanitin-induced hepatotoxicity by targeting the NTCP receptor: Mechanistic insights from in vitro and in vivo models.

BACKGROUND: α-Amanitin, the primary lethal toxin of Amanita phalloides, induces irreversible hepatotoxicity by selectively inhibiting RNA polymerase II, leading to transcriptional arrest. Despite advancements in managing mushroom poisoning, a targeted antidote remains unavailable. The sodium taurocholate co-transporting polypeptide (NTCP), a hepatic bile acid transporter, facilitates α-amanitin entry into hepatocytes. Pharmacological blockade of NTCP represents a promising therapeutic strategy. OBJECTIVE: To evaluate ezetimibe, an NTCP inhibitor, as a protective agent against α-amanitin-induc

Fixed-dose combination of obicetrapib and ezetimibe for LDL cholesterol reduction (TANDEM): a phase 3, randomised, double-blind, placebo-controlled trial.

BACKGROUND: Reducing LDL cholesterol prevents atherosclerotic cardiovascular disease (ASCVD) events. The aim of this study was to evaluate the LDL cholesterol-lowering efficacy of a fixed-dose combination (FDC) of obicetrapib, a CETP inhibitor, and ezetimibe. METHODS: This randomised, double-blind trial across 48 US sites including hospitals, private and group practices, and independent research centres included participants at least 18 years old with pre-existing or high risk for ASVCD or heterozygous familial hypercholesterolaemia with LDL cholesterol concentrations of 1·8 mmol/L (70 mg/dL)

Low-Intensity Statin Plus Ezetimibe Versus Moderate-Intensity Statin for Primary Prevention: A Population-Based Retrospective Cohort Study in Asian Population.

BACKGROUND: While moderate-intensity statin therapy is recommended for primary prevention, statins may not be utilized at a recommended intensity due to dose-dependent adverse events, especially in an Asian population. However, evidence supporting the use of low-intensity statins in primary prevention is limited. OBJECTIVE: We sought to compare clinical outcomes between a low-intensity statin plus ezetimibe and a moderate-intensity statin for primary prevention. METHODS: This population-based retrospective cohort study used the Korean nationwide claims database (2002-2019). We included adult

Similar major cardiovascular outcomes between pure statin and ezetimibe-statin in comparable intensity for type 2 diabetes with extremely atherosclerotic risks.

Atorvastatin 40 mg (ATOR 40) and ezetimibe 10 mg/simvastatin 20 mg (EZ-SIM 20) have similar reductions of low-density lipoprotein cholesterol (LDL-C) but cardiovascular (CV) outcomes between these two therapies are unclear. Our real-world cohort study is to test the hypothesis of pleiotropic effects of purely higher dose statin on CV outcomes beyond similar reductions of LDL-C, especially for extremely CV risk patients. Between January 1, 2007 and December 31, 2013, a total of 3,372 patients with type 2 diabetes mellitus (T2DM) admitted due to acute coronary syndrome (ACS) or acute ischemic st

The addition of evolocumab vs ezetimibe to statin therapy for coronary plaque regression and sd-LDL reduction in patients with ultrahigh-risk ASCVD.

BACKGROUND: Despite achieving target low-density lipoprotein cholesterol (LDL-C) levels, patients with ultrahigh-risk atherosclerotic cardiovascular disease (ASCVD) continue to experience substantial residual cardiovascular risk. Small dense LDL (sd-LDL) is increasingly recognized as a key atherogenic factor. OBJECTIVE: To compare the efficacy of adding evolocumab vs ezetimibe to statin therapy in promoting coronary plaque regression, and to evaluate the predictive value of sd-LDL reduction in patients with ultrahigh-risk ASCVD.
