Rivaroxaban, a direct inhibitor of coagulation factor Xa, attenuates adverse cardiac remodeling in rats by regulating the PAR-2 and TGF-β1 signaling pathways.

BACKGROUND: Factor Xa (FXa) not only plays an active role in the coagulation cascade but also exerts non-hemostatic signaling through the protease-activated receptors (PARs). This study aimed to investigate whether the FXa inhibitor, Rivaroxaban (RIV), attenuates adverse cardiac remodeling in rats with myocardial infarction (MI) and to identify the underlying molecular mechanisms it uses. METHODS: An MI model was induced in eight-week-old, male Wistar rats, by permanent ligation of the left anterior descending coronary artery. MI rats were randomly assigned to receive RIV or protease-activate

Bioprosthesis leaflet thrombosis following self-expanding valve-in-valve transcatheter aortic valve replacement in patient taking factor Xa inhibitor and warfarin: A case report.

An 87-year-old female with symptomatic severe aortic stenosis underwent transcatheter valve replacement (TAVR) via the transfemoral approach with a 29mm self-expanding device. Moderate to severe paravalvular regurgitation led to the development of congestive heart failure and hospital readmission 1 month following TAVR. A second 29mm valve was placed to abolish the paravalvular regurgitation. Routine follow-up computed tomography (CT) imaging demonstrated leaflet thickening and decreased leaflet mobility suggesting valve thrombosis, despite adherence to Factor Xa inhibitor. Transthoracic echoc

Razaxaban, a direct factor Xa inhibitor, in combination with aspirin and/or clopidogrel improves low-dose antithrombotic activity without enhancing bleeding liability in rabbits.

Coactivation of platelets and the blood coagulation cascade contributes to the pathophysiology of arterial thrombosis. Combination therapy with antiplatelet and anticoagulant drugs may be needed for maximizing the prevention and treatment of arterial thrombosis. Few studies have thoroughly investigated the combined antithrombotic and bleeding effects of these antithrombotic agents. We, therefore, evaluated the antithrombotic and bleeding profiles of dual and triple therapy with razaxaban, a direct factor Xa inhibitor, plus aspirin and/or clopidogrel in rabbit models of electrolytic injury-indu

Evaluation of Risk Factors for Supratherapeutic Anti-Xa Levels With Treatment-Dose Enoxaparin in Hospitalized Patients Without Severe Renal Impairment.

The standard dose of enoxaparin for therapeutic anticoagulation is 1 mg/kg every 12 h in patients with a creatinine clearance (CrCl) greater than 30 mL/min. Besides pregnancy, obesity, and renal impairment, literature on other risk factors for supratherapeutic anti-Xa levels is sparse. The objective of this retrospective study was to determine novel risk factors for supratherapeutic anti-Xa levels and further inform empiric enoxaparin dosing. We included adult patients with CrCl greater than 30 mL/min that received 1 ± 0.09 mg/kg of enoxaparin every 12 h. The primary outcome was the correlatio

OC-03 - Modelisation of the procoagulant properties of cancer cells and their capacity to alter the antithrombotic efficiency of LMWHs and specific inhibitors of factor Xa.

INTRODUCTION: The risk of venous thromboembolism varies according to the histological type of cancer. The failure of antithrombotic treatment is more frequent in cancer patients as compared to non-cancer ones. AIM: We aimed to elucidate the mechanism of activation of blood coagulation induced by cancer, the impact of chemo-resistance phenotype on the capacity of cancer cells to trigger thrombin generation and the alterations of the efficiency of LMWHs and the specific inhibitors of factor Xa (fondaparinux and apixaban) in the presence of cancer cells. MATERIALS AND METHODS: Thrombin generati

Heparin-calibrated anti-factor Xa activity as a surrogate marker for direct oral anticoagulant levels in acute stroke care: a large observational single-center study.

BACKGROUND AND PURPOSE: Direct oral anticoagulants (DOAC) complicate therapeutic decision-making, as rapid assessment of anticoagulant activity is critical in emergency patients, particularly in acute ischemic stroke (AIS). The administration of systemic thrombolysis (IVT) in anti-coagulated patients requires careful risk-benefit evaluation, given the potential for hemorrhagic complications. Substance-specific DOAC plasma level measurements are often unavailable outside tertiary centers. This study evaluated whether widely available heparin-calibrated anti-factor Xa (anti-Xa) activity can reli

A narrative literature review on the clinical utility of activated partial thromboplastin time and anti-factor Xa activity assays in cancer patients with anticoagulant therapy.

BACKGROUND AND OBJECTIVE: Cancer patients face a substantially increased risk of venous thromboembolism (VTE), a leading cause of non-cancer mortality. Optimizing anticoagulation therapy in this population necessitates balancing thromboprophylaxis efficacy with hemorrhage risk, which is particularly challenging given comorbidities, polypharmacy, and tumor-specific factors such as gastrointestinal/urogenital malignancies amplifying bleeding risks with direct oral anticoagulants (DOACs). Monitoring anticoagulant activity is crucial to avoid subtherapeutic dosing or supratherapeutic dosing. This

Clinical impact of major bleeding in patients with venous thromboembolism treated with factor Xa inhibitors or vitamin K antagonists. An individual patient data meta-analysis.

Factor Xa (fXa)-inhibitors are as effective and safer than vitamin-K-antagonists (VKA) in the treatment of venous thromboembolism (VTE). We previously classified the severity of clinical presentation and course of all major bleeding events from the EINSTEIN, AMPLIFY and HOKUSAI-VTE trials separately. The current aim was to combine these findings in order to increase precision, assess a class effect and analyse presentation and course for different types of bleeding, i. e. intracranial, gastro-intestinal, and other. We classified the clinical presentation and course of all major bleeding events

Influence of patient characteristics and renal function on factor Xa inhibition pharmacokinetics and pharmacodynamics after enoxaparin administration in non-ST-segment elevation acute coronary syndromes.

BACKGROUND: The current standard of care for patients with non-ST-segment elevation acute coronary syndromes (ACS) includes antithrombotic therapy with aspirin and heparin. Although emerging data suggest that low-molecular weight preparations offer distinct advantages over unfractionated heparin, limited information on patient-related factors that may influence dosing, safety, and efficacy is available. PURPOSE: The purpose of our study was the determination of the impact of patient age, sex, body weight, and renal function on factor Xa inhibition pharmacokinetics and pharmacodynamics after e

Rivaroxaban suppresses factor Xa-Driven PAR-2-ERK inflammatory, catabolic, osteoclastogenic, and mitochondrial dysfunction signaling in osteoarthritis-relevant chondrocytes: a drug-repurposing strategy for joint inflammation.

BACKGROUND: Osteoarthritis (OA) is increasingly recognised as an inflammation-driven whole-joint disease in which cartilage catabolism, osteochondral remodelling, and mitochondrial dysfunction drive structural progression. Factor Xa (FXa), beyond haemostasis, is a signalling protease that activates protease-activated receptor-2 (PAR-2) and downstream MAPK-dependent inflammation; whether it directly drives this programme in OA-relevant chondrocytes, and whether rivaroxaban can intercept it, is undefined. OBJECTIVE: To determine whether rivaroxaban attenuates FXa-induced PAR-2/ERK inflammatory

How to deal with interference on heparin anti-Xa activity caused by oral factor FXa inhibitors: communication from the ISTH SSC Subcommittee on Control of Anticoagulation.

Monitoring unfractionated heparin (UFH) to ensure effective anticoagulation may be performed using anti-factor Xa activity (anti-Xa) instead of the activated partial thromboplastin time. However, in patients who have been treated with oral factor (F)Xa inhibitors (apixaban, rivaroxaban, and edoxaban) while switching to UFH therapy, there is a risk that these oral anti-FXa drugs could interfere with UFH-calibrated anti-Xa monitoring. This may lead to inappropriate anticoagulation management. This report of the International Society on Thrombosis and Haemostasis (ISTH) Subcommittee on Control of

Weight-based enoxaparin with anti-factor Xa assay-based dose adjustment for venous thromboembolic event prophylaxis in adult trauma patients results in improved prophylactic range targeting.

BACKGROUND: Venous thromboembolism (VTE) is a common morbidity in trauma patients. Standard VTE chemoprophylaxis is often inadequate. We hypothesized that weight-based dosing would result in appropriate prophylaxis more reliably than fixed dosing. METHODS: All patients admitted to a Level 1 trauma center over a 6-month period were included unless contra-indications for VTE prophylaxis existed. A prospective adjusted-dosing group was compared to a retrospective uniform-dosing group. The adjusted-dosing approach consisted of initial weight-based dosing of 0.5 mg/kg subcutaneously (subQ) every 1

Innovative Three-Step Microwave-Promoted Synthesis of N-Propargyltetrahydroquinoline and 1,2,3-Triazole Derivatives as a Potential Factor Xa (FXa) Inhibitors: Drug Design, Synthesis, and Biological Evaluation.

The coagulation cascade is the process of the conversion of soluble fibrinogen to insoluble fibrin that terminates in production of a clot. Factor Xa (FXa) is a serine protease involved in the blood coagulation cascade. Moreover, FXa plays a vital role in the enzymatic sequence which ends with the thrombus production. Thrombosis is a common causal pathology for three widespread cardiovascular syndromes: acute coronary syndrome (ACS), venous thromboembolism (VTE), and strokes. In this research a series of N-propargyltetrahydroquinoline and 1,2,3-triazole derivatives as a potential factor Xa (FX
