Outcomes of salvage total hip arthroplasty after failed cannulated screw fixation of fractured neck of femur.

INTRODUCTION: In a UK setting, cannulated hip screws (CHS) are frequently used to fix femoral neck fractures. Although often a relatively quick procedure and one that is delegated to more junior surgeons, failure rates of up to 23 % have been reported. The salvage procedure is total hip arthroplasty (THA). In this paper we report the outcomes of a series of THA for failed cannulated screw fixation. METHODS: Retrospective analysis of one of the largest reported single cohort of 600 CHS procedures spanning 14 years from 2007 to 2020 from a single centre was performed. This identified 55 patient

Dual mobility versus unipolar total hip arthroplasty for neck of femur fractures : a single centre study.

Current national guidelines (NICE) recommends that all medically fit, independently-mobile patients without cognitive impairment receive a total hip arTHAoplasty(THA) for displaced intracapsular neck of femur (NOF) fractures. Dislocation is a concern(2-10%). Dual mobility cups have been suggested to address this complication. Our study sets out to compare dislocation rates between dual mobility cups versus unipolar cups. We performed a retrospective single centre multiple surgeon study of all THAs performed for NOFs between January 2012 and May 2018. A total of 322 total hip replacements (127

Bilateral undisplaced insufficiency neck of femur fractures associated with short-term steroid use: a case report.

INTRODUCTION: We present an interesting and unusual case of a 61-year-old woman with bilateral, undisplaced, stress neck of femur fractures associated with short-term steroid use. Insufficiency fractures of the neck of femur without preceding trauma have been described in the literature, although bilateral involvement is infrequent. These fractures have been associated with strenuous exercise, seizures, renal osteodystrophy, fluoride treatment, long-term corticosteroid use, amenorrhoea, abnormal anatomy and osteomalacia due to nutritional and/or hormonal factors. CASE PRESENTATION: The case w

[Decreased acetabular anteversion and femur neck antetorsion cause pain and arthrosis. 1: Statistics and clinical sequelae].

PROBLEM: This paper investigates which angles of acetabular (AA) and femoral antversion (FA) have to be considered as normal and which ones cause changes in range of movements, pain and osteoarthrosis. METHOD OF INVESTIGATION: 15-20 degrees of anteversion of AA and FA were tested regarding normality. Diminished anteversion angles were divided in groups -2 and -3, and increased angles in groups +2 and +3. RESULTS: The material consisted of 356 hip joints with computerized tomographies, investigated separately in different deformities. Changes in range of movement, pain and osteoarthrosis were

Evaluating the physical examination and diagnostic workup for occult Neck Of Femur fractures in a district general hospital.

BACKGROUND: Occult Neck of Femur (NOF) fractures are radiographically elusive injuries. Undiagnosed, they lead to delayed treatment, displacement, and increased morbidity. While MRI is the gold standard for detection, CT is frequently used when MRI is unavailable. Physical examination manoeuvres, specifically the hip log-roll (HR) and straight leg raise (SLR), are often employed to guide decisions regarding cross-sectional imaging. This study explores the utility of these physical examinations in guiding the workup for occult NOF fractures. METHODS: A prospective service evaluation was perfor

Increased femoral anteversion in females with anterior knee pain relates to both the neck and the shaft of the femur.

INTRODUCTION: Increased femoral anteversion (FAV) can have many clinical manifestations, including anterior knee pain (AKP). To our knowledge, no studies have measured the location of FAV in a cohort of female AKP patients. The objective of this research is to determine whether the increased FAV in AKP females originates above the lesser trochanter, below the lesser trochanter or at both levels. MATERIALS AND METHODS: Thrity-seven consecutive AKP female patients (n = 66 femurs) were recruited prospectively. There were 17 patients (n = 26 femurs; mean age of 28 years) in whom the suspicion for

Retrospective analysis of outcomes and complications after Korus hemiarthroplasty in elderly patients with neck femur fractures.

BACKGROUND: Femoral neck fractures (FNF) is one of the most common traumatic events in elderly patients: the choice of an appropriate treatment is necessary to decrease the related mortality and to achieve the best possible outcomes. Nowadays, it is still debated whether or not to cement the stem in hemiarthroplasty and above all, which stem to use to best respect the integrity of the elderly bone. METHODS: From January 2017 to December 2019, a bi-centric study utilizing prospectively collected databases of elderly patients with FNF treated with uncemented Korus stem hemiarthroplasty was perf

Neglected Neck of Femur Fracture Managed by Surgical Hip Dislocation: A Case Report and Literature Review.

INTRODUCTION: Talus fracture is an uncommon fracture that can be encountered on day- to- day basis. However, it is the 2nd most common tarsal bone to get fractured after calcaneum and accounts for approximately 1% of all fractures around foot and ankle. The anastomotic ring around the talar neck is highly likely to get damaged at the time of the fracture, which, in turn, hampers the blood supply to the body of talus. As a result, the bone healing is delayed and the integrity of the healed fracture is poor which leads to poor functional outcome. Almost 39% cases are missed during the initial ev

Non-union fracture neck femur in a toddler: Reconstructed by valgus osteotomy - A minimally invasive approach.

BACKGROUND: Non-union is one of the devastating complications of fracture neck of femur. Though a very rarely encountered entity in a toddler (1-3 years paediatric age group), non-united femoral neck fractures are reported in developing countries because of mismanagement by quacks and delay in referrals. For operative treatment, many different procedures have been described, including close/open reduction and internal fixation using K-wires, cannulated screws, fibula or nails. There is no evidence in the literature that one or other implant influences the rate of postoperative complications, s

[Sequential changes in biomechanical competence of femoral neck and marrow cavity of proximal femur in ovariectomized rats].

OBJECTIVE: To observe the sequential changes in biomechanical competence of the femoral neck and marrow cavity of the proximal femur in ovariectomized rats. METHODS: Bone mineral density (BMD) and biomechanical properties of the femoral neck and the structural dimension of the proximal femur were measured 0, 3, 6, 9, 12, 15, 18, 21 weeks after ovariectomy (OVX) or sham operation (Sham) in 6-month-old female SD rats. RESULTS: The BMD of femoral neck in OVX rats was significantly lower than that in Sham group 6 weeks after operation, (0.195 +/-0.028) g/mm(2) vs (0.225 +/-0.036) g/mm(2) (P=0.03

Osteosynthesis and primary valgus intertrochanteric osteotomy in displaced intracapsular fracture neck of femur with osteoporosis in adults.

Fifty-three adults sustaining intracapsular femoral neck fractures (subcapital 38 and transcervical 15) with osteoporosis were treated primarily by osteosynthesis with valgus intertrochanteric osteotomy. Final evaluation was done in 50 patients (1 patient died and 2 lost to follow up, were not considered). Union was achieved in 47 (94%) patients in an average period of 12.2 weeks (range 10-18 weeks) with 100% union at osteotomy site. An axial collapse between 2 and 14 mm was observed in 74% of patients at the fracture site. Average neck shaft angle achieved was 141 degrees . Retroversion of th

[Functional intramedullary osteosynthesis of the femur neck with a bundle of nails (biomechanical, anatomo-morphologic and clinical substantiation)].

The authors prove by their biomechanical and anatomomorphological investigation that osteosynthesis with a three-blade nail is traumatic, and that it is little traumatic when performed with a bundle of rods (6 rods 1.8 mm in diameter and 6-10 rods 1 mm in diameter) which are dispersely introduced through the femoral neck diameter and that it is rigid enough for functional management of the patients during the postoperative period. 37 patients were operated. The support and movement function of the extremity was restored on the average 176 +/- 25 days and the consolidation took place 217 +/- 60

Proximal femur geometry to detect and distinguish femoral neck fractures from trochanteric fractures in postmenopausal women.

Some proximal femur geometry (PFG) parameters, measured by dual-energy X-ray absorptiometry (DXA), have been reported to discriminate subjects with hip fracture. Relatively few studies have tested their ability to discriminate femoral neck fractures from those of the trochanter. To this end we performed a cross-sectional study in a population of 547 menopausal women over 69 years of age with femoral neck fractures (n = 88), trochanteric fractures (n = 93) or controls (n = 366). Hip axis length (HAL), neck-shaft angle (NSA), femoral neck diameter (FND) and femoral shaft diameter (FSD) were meas

[Decreased acetabular anteversion and femur neck antetorsion cause pain and arthrosis. 2: Etiology, diagnosis and therapy].

ETIOLOGY: Differences in the anteversion of the acetabulum and the femur must be attributed to the different rotational postures of the fetus as investigations and experiments have shown. After delivery there is a spontaneous improvement, but in perhaps 15% of the joints, diminished or increased acetabular or femoral anteversion will persist during later life. The results of the investigation of Part 1 are compared with the literature. So far no correlations between AA/FA and different clinical consequences have been reported. The deformity of diminished AA and FA is found as a singular entity
