Intrapartum Atosiban Treatment During Tachysystole and Non-reassuring Fetal Monitoring: A Retrospective Cohort Study.

BACKGROUND: Uterine tachysystole during labor can lead to a decrease in fetal oxygen saturation and intracerebral oxygen saturation. Acute tocolysis using atosiban can inhibit uterine smooth muscle activity, potentially improving fetal status and facilitating vaginal delivery or allowing time to prepare for operative delivery. OBJECTIVES: To compare maternal and neonatal outcomes in cesarean and vaginal deliveries following atosiban administration during fetal prolonged deceleration and tachysystole at gestational age 37 0/7 to 43 0/7 weeks. METHODS: We conducted a single-center, descriptive

Transplacental Therapeutic Drug Monitoring in Pregnant Women with Fetal Tachyarrhythmia Using HPLC-MS/MS.

Fetal arrhythmia develops in 0.1-5% of pregnancies and may cause fetal heart failure and fetal hydrops, thus increasing fetal, neonatal, and infant mortality. The timely initiation of transplacental antiarrhythmic therapy (ART) promotes the conversion of fetal tachycardia to sinus rhythm and the regression of the concomitant non-immune fetal hydrops. The optimal treatment regimen search for the fetus with tachyarrhythmia is still of high value. Polymorphisms of these genes determines the individual features of the drug pharmacokinetics. The aim of this study was to study the pharmacokinetics o

Pituitary apoplexy during pregnancy with transsphenoidal resection and intraoperative fetal monitoring: A case report.

INTRODUCTION: Prolactinomas resulting in pituitary apoplexy are an uncommon obstetrical complication. The hemorrhage can cause compression and necrosis of the pituitary gland as well as the optic chiasm, necessitating surgical intervention. CASE: A 35-year-old woman, G0, presented for an infertility consult with a prior diagnosis of polycystic ovarian syndrome. Evaluation for oligomenorrhea found an elevated prolactin level of 69.76 ng/mL, an elevated DHEA-S of 524, and HgbA1c of 5.7%. The patient denied visual or neurological symptoms. Infertility treatment was started, and magnetic resonanc

The Goal of Continuous Fetal Heart Rate Monitoring During Labor: Have We Been Successful?

Despite its ubiquitous use, fetal heart rate (FHR) monitoring has not resulted in a significant reduction in hypoxic-ischemic encephalopathy following delivery. This manuscript reviews the reasons for this failure including limitations of FHR to accurately predict hypoxia, low prevalence of hypoxic-ischemic encephalopathy, and lack of standardization of interpretation and intervention. We propose an alternative goal for FHR monitoring during labor to provide optimal care by early identification of truly concerning features, initiation of appropriate interventions, clear documentation of concer

Interobserver agreement in CTG interpretation using the 2015 FIGO guidelines for intrapartum fetal monitoring.

BACKGROUND: Visual analysis of cardiotocographic (CTG) tracings has been shown to be prone to poor intra- and interobserver agreement when several interpretation guidelines are used, and this may have an important impact on the technology's performance. OBJECTIVES: The aim of this study was to evaluate agreement in CTG interpretation using the new 2015 FIGO guidelines on intrapartum fetal monitoring. STUDY DESIGN: A pre-existing database of intrapartum CTG tracings was used to sequentially select 151 cases acquired with a fetal electrode, with duration exceeding 60minutes, and signal loss le

Monitoring fetal heart rate during pregnancy: contributions from advanced signal processing and wearable technology.

Monitoring procedures are the basis to evaluate the clinical state of patients and to assess changes in their conditions, thus providing necessary interventions in time. Both these two objectives can be achieved by integrating technological development with methodological tools, thus allowing accurate classification and extraction of useful diagnostic information. The paper is focused on monitoring procedures applied to fetal heart rate variability (FHRV) signals, collected during pregnancy, in order to assess fetal well-being. The use of linear time and frequency techniques as well as the com

Fetal monitoring with computerized ST analysis during labor: a systematic review and meta-analysis.

BACKGROUND: Computerized ST analysis of fetal electrocardiography (ECG) combined with cardiotochography (CTG) has been introduced for intrapartum monitoring and is the prevailing method when ST analysis (STAN®) is used. OBJECTIVE: To assess the evidence that computerized ST analysis during labor reduces the incidence of fetal metabolic acidosis, hypoxic ischemic encephalopathy, cesarean section, instrumental vaginal delivery or the number of instances where fetal scalp blood sampling is used as compared with CTG only. METHODS: Search of PubMed, Cochrane Library, EMBASE, Web of Science, CINAH

Management of Graves' disease during pregnancy: the key role of fetal thyroid gland monitoring.

BACKGROUND: Fetuses from mothers with Graves' disease may experience hypothyroidism or hyperthyroidism due to transplacental transfer of antithyroid drugs (ATD) or anti-TSH receptor antibodies, respectively. Little is known about the fetal consequences. Early diagnosis is essential to successful management. We investigated a new approach to the fetal diagnosis of thyroid dysfunction and validated the usefulness of fetal thyroid ultrasonograms. METHODS: Seventy-two mothers with past or present Graves' disease and their fetuses were monitored monthly from 22 wk gestation. Fetal thyroid size and

[Monitoring of pregnancy complicated by maternal-fetal isoimmunization. A comparison between two clinical protocols].

AIM: Invasive techniques such as amniocentesis and cordocentesis are used for the diagnosis and treatment of fetus at risk for anemia due to maternal red-cell alloimmunization. The purpose of this study was to determine the value of non invasive measurements of the peak velocity middle cerebral artery in the fetus (PVMCA) for the diagnosis of fetal anemia. METHODS: From 1996 to September 2002, we studied 23 pregnancies with anti D title >1:32. In the 1(st) group of 11 women (from 1996 to 1999) fetal anemia was detected by invasive techniques (amniocentesis and cordocentesis). In the 2(nd) gro

Caregivers' perspective on the implementation of continuous antepartum fetal monitoring in hospitalized high-risk pregnancies.

BACKGROUND: Continuous antepartum fetal monitoring has been proposed as an alternative to the current practice of intermittent monitoring of fetal well-being in high-risk pregnancies. Healthcare professionals, however, do not yet fully embrace the concept of continuous monitoring. In order to optimize implementation of continuous fetal monitoring, the current study investigates caregivers' opinions towards continuous fetal monitoring, devices to use, and the implementation in clinical care. METHODS: This single center cross-sectional study was conducted at the Obstetric High Care unit of a te

Should fetal blood sampling still be a part of monitoring during labor in the modern era ?

Fetal monitoring during labor is crucial for detecting potential hypoxic situations that could lead to severe outcomes like cerebral palsy or peripartum death. The current standard, fetal heart rate (FHR) monitoring, is subjective and prone to variability, with limited accuracy in predicting neonatal acidosis or hypoxic ischemic encephalopathy. Secondary methods of foetal monitoring have been developed in an attempt to reduce unnecessary interventions due to continuous cardiotocography (CTG), and to better identify foetuses that are at risk of intrapartum acidosis. Very few studies directly co

Fetal monitoring: its effect on the perinatal mortality and cesarean section rates and its complications.

Direct fetal monitoring was employed in 608 patients, 96 per cent of whom were though to be at "high risk". A definite rise in the primary cesarean section rate occurred, but 107 cesarean sections were averted. Fetal deaths after admission as well as the total perinatal mortality rates were reduced. Apgar scores were recorded, and low Apgar scores correlated with fetal distress patterns. Complications consisted of only one uterine perforation and two cephalohematomas; no evidence of puerperal pyrexia was noted secondary to intrauterine catheter placement.

Prediction of fetal distress and poor outcome in prolonged pregnancy using Doppler ultrasound and fetal heart rate monitoring combined with stress tests (II).

Postterm pregnancies of more than 290 completed gestational days (n = 110) were simultaneously supervised after admission by pulsed Doppler ultrasound of the common carotid and the umbilical arteries, nonstress and contraction stress tests (NST/CST) and vibroacoustic stimulation tests (VAST). The results of these tests were blinded for the clinicians. Further decision making was based mainly on fetal heart rate (FHR) monitoring. The prognostic value of tests performed < 3 days before birth predicting fetal distress, low Apgar and pH values in the umbilical artery were compared by receiver oper

Human placental lactogen and unconjugated estriol concentrations in twin pregnancy: monitoring of fetal development in intrauterine growth retardation and single intrauterine fetal death.

Human placental lactogen and unconjugated estriol concentrations in maternal serum were evaluated in 100 uneventful twin pregnancies, and these values were compared with those observed in 16 twin pregnancies associated with intrauterine growth retardation or single intrauterine fetal death. In pregnancies associated with intrauterine growth retardation (n = 8), human placental lactogen levels were at the lower limit of normal range for singleton pregnancies, whereas estriol levels were normal in most cases. When one of the fetuses had died before week 33 of pregnancy (n = 5), both human placen

Perceptual distortions during simultaneous continuous monitoring of fetal and maternal heart rate in laboring patients.

OBJECTIVE: Our previous work examined the effect on assessment of fetal well-being when presenting twin fetal heart rate tracings (FHRT) simultaneously on screen. To further examine the potential impact of subconscious perceptual influences on fetal well-being, the current study presented singleton FHRT with and without simultaneous presentation of maternal heart rate in a within-subjects design. Tracings were taken from archival cases with either normal or abnormal Apgar scores to identify differential impact depending on final outcome. All tracings were evaluated for variability, acceleratio
