Selection bias and inferential validity in fever of unknown origin studies. Comment on: Poposki et al. Visceral leishmaniasis as a leading cause of fever of unknown origin in immunocompetent adults.

The article by Poposki et al. on visceral leishmaniasis as a cause of fever of unknown origin presents clinically valuable observations but exhibits substantial methodological limitations. Differential exclusion of undiagnosed cases introduces selection bias and precludes generalizable prevalence estimates. The analytical strategy lacks etiological coherence by comparing a specific diagnosis against heterogeneous conditions. Multiple univariate comparisons without adjustment for family-wise error rate and absence of multivariable analysis to control confounding limit causal inference. The cros

Atypical Presentation of Pheochromocytoma With Persistent Fever of Unknown Origin: A Case Report.

Pheochromocytoma is a rare neuroendocrine tumor classically associated with episodic adrenergic symptoms and hypertension, but its clinical presentation is highly variable and may be misleading. Fever is an uncommon manifestation and is rarely the dominant presenting feature, which can result in diagnostic delay and extensive evaluation for infectious, inflammatory, or malignant causes. We report the case of a middle-aged woman who presented with prolonged fever, weight loss, and systemic inflammatory features without hypertension or classic catecholamine-related symptoms. Extensive investigat

Fever of unknown origin: Clinical significance of the etiology and common inflammatory parameters.

OBJECTIVES: To analyze the fever of unknown origin (FUO) etiology and clinical characteristics and determine the diagnostic value of common inflammatory parameters. METHODS: A total of 853 patients with classic FUO were prospectively enrolled from January 2014 to May 2021. According to the etiological diagnosis, the patients were divided according to etiology: infectious diseases (n = 430); non-infectious inflammatory diseases [NIIDs] (n = 153); neoplastic diseases (n = 125); miscellaneous diseases (n = 48); and no identifiable etiology (n = 97). The clinical features and common inflammatory

Subacute Thyroiditis Causing Fever of Unknown Origin: A Reminder of the Uncommon Cause.

Subacute thyroiditis (SAT) is a transient inflammation of the thyroid gland that often occurs following a viral infection. It is an infrequent cause of fever of unknown origin (FUO). We present a 46-year-old gentleman who presented with two weeks of fever and some non-specific left-sided neck pain. His initial investigations and microbiological workup were unremarkable. He did not report any hyperthyroid symptoms. A computed tomography of the neck, chest, abdomen, and pelvis showed a heterogeneous appearance of his thyroid gland. Thyroid function was then performed, and it showed primary hyper

Fever of Unknown Origin: A Validation Study of Danish ICD-10 Diagnosis Codes.

BACKGROUND: Real-world data in form of routinely collected clinical data are a valuable resource for epidemiological research in infectious disease. We examined the validity of a discharge diagnosis of fever of unknown origin from hospital discharge registries. METHODS: We identified patients with a first in- or outpatient diagnosis (primary or secondary) of fever of unknown origin (ICD-10 code R50.0; R50.8, R50.9) recorded in the Danish National Patient Registry (DNPR) between 2010 and 2017 in the North Denmark Region. We based the validation cohort on a mix of patients diagnosed at a highly

Treatment of Elsberg Syndrome Causes Fever of Unknown Origin Attributable to Drug Reaction.

A 56-year-old male with a history of non-adherence to HIV anti-retroviral therapy (ART) presented with Elsberg syndrome - varicella reactivation causing fever, painful dermatomal rash, weakness of bilateral lower extremities, and urinary and bowel dysfunction. On the third day of hospitalization, the patient developed altered mental status. An investigation for encephalitis and myelitis revealed a CD4 count of 150 cells/uL, viral load of about 150,000 copies/mL, and MRI of the lumbar spine demonstrating thickening of the cauda equina. Cerebrospinal fluid (CSF) from lumbar puncture confirmed th

Fever of unknown origin, a rare presentation of metastatic prostate cancer: Case report.

Metastatic prostate cancer can have an initial presentation with fever and systemic inflammatory response syndrome. Accurate diagnosis allows to differentiate this type of cancer from infectious conditions and to start early treatment. We report one case, in which the patient presented to the emergency department with lower urinary tract symptoms, fever and consumptive syndrome. Acute prostatitis was initially suspected, but subsequently prostate adenocarcinoma was diagnosed. This case enables us to consider the possibility of prostate adenocarcinoma as the diagnose in patients with fever of u

Fever of unknown origin as the initial manifestation of valproate-induced Fanconi syndrome.

BACKGROUND: Valproate-induced Fanconi syndrome is a rare adverse effect of valproate. Severely disabled patients who require tube feeding are reported to be susceptible to valproate-induced Fanconi syndrome. Although most patients with valproate-induced Fanconi syndrome are asymptomatic and detected incidentally with findings such as hypophosphatemia, hypouricemia, increased urinary β2-microglobulin, and generalized hyperaminoaciduria, clinical symptoms such as bone fracture, fever, tachypnea, and edema have been reported. PATIENT DESCRIPTION: This 15-year-old, severely disabled, tube-fed, ma

[A case of fulminant ulcerative colitis initially presenting as fever of unknown origin].

A 40's man was referred to our hospital for the investigation of fever of unknown origin lasting for a month. The laboratory data showed a prominent inflammatory reaction and a high titer of PR3-ANCA. Despite the various imaging studies and bacteriological examinations, the cause of the fever could not be detected until he complained of abdominal pain with bloody stool that appeared during hospitalization and which prompted colonoscopy, resulting in the diagnosis of moderate ulcerative colitis of the descending colon. Although temporal improvement was achieved by mesalazine administration, the

F-18 FDG PET/CT in the diagnosis of fever of unknown origin.

METHODS: The utility of Fluorine-18 fluorodeoxyglucose positron emission tomography/CT in identifying the causal source was assessed in this retrospective study. A total of 68 patients (33 men, 35 women; age range, 23-91 years) with fever of unknown origin (FUO) underwent a positron emission tomography/computed tomography (PET/CT) scan. PET/CT was considered helpful when abnormal results allowed an accurate diagnosis, based on histopathology, microbiologic assays, or clinical and imaging follow-up. RESULTS: PET/CT demonstrated suspected pathologic foci of Fluorine-18 fluorodeoxyglucose (F-18

Epidemic of fever of unknown origin in rural Thailand, caused by influenza A (H1N1) and dengue fever.

In the late summer (rainy season) of 1987, a sharp outbreak of fever of unknown origin (FUO) in rural southern Thailand was investigated by a field epidemiology team. In a random survey of households, 40 percent of the children and 20 percent of adults were reported to have had febrile illnesses within the last month. There was at least one death, possibly from Reye's syndrome. Testing 34 pairs of acute and convalescent sera showed significant HI antibody titer rises to influenza A (Taiwan/(H1N1) (9 cases) and dengue virus (12 cases). Testing 79 single sera with the antibody capture ELISA test

Adult-Onset Still's Disease as an Uncommon Cause of Fever of Unknown Origin.

Adult-onset Still's disease (AOSD) is a rare systemic autoinflammatory disorder characterized by fever, rash, arthritis, and hyperferritinemia, often presenting as a diagnostic challenge due to its nonspecific manifestations. We report the case of a 35-year-old previously healthy male who presented with a 20-day history of pruritic erythematous skin lesions, high-grade fever, severe odynophagia, and symmetric inflammatory polyarthritis. Laboratory evaluation revealed leukocytosis with neutrophilia, elevated acute-phase reactants, liver enzyme abnormalities, and hyperferritinemia (1,050 ng/mL).

Adult-onset recurrent Kawasaki disease: an unusual cause of fever of unknown origin.

We present the case of a 20-year-old man with a background of transposition of the great arteries presenting with fever of unknown origin, who developed a shock like syndrome with respiratory failure necessitating intubation. After extensive investigation, a diagnosis of adult-onset Kawasaki disease was made, and he was successfully treated with IVIg and corticosteroids. We present the clinical findings clinicians should be aware of, and review the literature on managing this rare presentation in adult, highlighting the importance of early diagnosis in improving outcomes. Both children and adu

[Intravascular large B-cell lymphoma: an important cause of fever of unknown origin].

Intravascular large B-cell lymphoma (IVLBCL) is an important cause of fever of unknown origin (FUO) and multiple organ failure (MOF). Earlier, most IVLBCL cases were diagnosed only postmortem; however, now, it is possible to diagnose and treat these cases antemortem. Although hematogeneous dissemination of malignant tumor cells except lymphoma is beyond the scope of present treatment regimens, IVLBCL (hematogeneous dissemination of lymphoma) can be treated by chemotherapy so correct diagnosis is important. The onset and clinical course of IVLBCL is heterogeneous. Many IVLBCL cases show rapid d

Fever of unknown origin in a patient of systemic onset juvenile idiopathic arthritis.

Hemophagocytic lymphohistiocytosis is a potentially fatal condition characterized by pathologic immune activation, which can complicate infections, childhood systemic rheumatologic diseases and malignancies. Here we report a case of reactive hemophagocytic lymphohistiocytosis [macrophage activation syndrome] complicating systemic onset juvenile idiopathic arthritis, which was treated successfully with dexamethasone and cyclosporine. Reactive hemophagocytic lymphohistiocytosis or macrophage activation syndrome should be considered in patients of juvenile idiopathic arthritis with prolonged feve

Pyrexia as the primary paradox: celiac disease masked as fever of unknown origin.

Celiac disease (CD) can present solely with atypical extra-intestinal symptoms. We describe a 33-year-old woman hospitalized for a one-month history of fever, myalgias, and tachycardia. Extensive infectious, autoimmune, and malignancy investigations were unrevealing. However, hypochromic anemia and elevated CRP prompted serologic testing, revealing positive anti-transglutaminase IgA. Duodenal biopsy confirmed CD with Marsh IIIa villous atrophy. This case illustrates CD as a rare etiology of fever of unknown origin (FUO), emphasizing the importance of celiac serology in the differential diagnos

Adult-Onset Still's Disease Presenting as Fever of Unknown Origin: A Case Report.

Adult-onset Still's disease (AOSD) is a rare systemic autoinflammatory disorder that should be considered in patients presenting with fever of unknown origin (FUO). Diagnosis is challenging due to overlapping features with infectious, autoimmune, and hematological conditions. We report the case of a 19-year-old female presenting with a six-day history of persistent fever, later developing an evanescent rash and migratory polyarthralgia. Laboratory findings included cytopenias, elevated inflammatory markers, and markedly elevated ferritin levels. After ruling out infectious, autoimmune, and neo
