This qualitative study examined lesbian, gay, bisexual, transgender, queer, intersex, asexual, and other minoritized sexual and gender identities (LGBTQIA+) primary care in Manila using a lens of minority stress theory within a social-ecological model. Focus group discussions, key informant interviews, and policy document analysis identified needs, barriers, and policy/institutional determinants. Unmet needs in mental health, sexual and reproductive health, HIV/sexual transmitted infections (STI) care, and gender-affirming care were analyzed. Individually, internalized stigma, anticipated disc

Tanzania faces increasing climate-sensitive health threats, including vector-borne diseases, waterborne infections, and malnutrition. Effective preparedness at both national and community levels is critical for health system resilience and livelihood security. This study applied a multilevel mixed-methods approach to assess Tanzania's policy structures and community-level experiences related to climate-health preparedness. Between January 2024 and May 2025, we conducted a concurrent mixed-methods study. At national level, we reviewed climate-health policy documents, mapped 30 key stakeholders,

Medicaid is the nation's largest public health insurer and the primary payer of perinatal care; it covers more than 70 million low-income residents and 41% of births. The One Big Beautiful Bill Act (OBBBA), which was signed into law on July 4, 2025, introduced the largest Medicaid funding reductions in the program's history-an estimated $1.1 trillion over 10 years. In this article, we describe the major OBBBA provisions related to eligibility and Medicaid financing and the anticipated effects of these provisions on the delivery of perinatal care services and related outcomes. Although pregnant

BACKGROUND: The growing need for availability of new vaccines, alongside constrained financial and health system resources, necessitates transparent and evidence-based prioritization.
METHODS: A multi-criteria decision analysis (MCDA) approach was applied by the Tanzania Immunization Technical Advisory Group (TITAG) to prioritize new vaccine introductions for 2026-2030. The World Health Organization's New Vaccine Introduction Prioritization and Sequencing Tool (NVI-PST) was adapted to the Tanzanian context. From thirteen candidate vaccines, seven were shortlisted: hepatitis B birth dose, hepa

The global health landscape is undergoing a significant transformation as traditional frameworks of cooperation face fragmentation amid geopolitical tensions. Declining support from Western nations, exemplified by US withdrawal from the World Health Organization and cuts to programs like the President's Emergency Plan for AIDS Relief, has exposed the profound instability of an aid architecture built on colonial dependencies. The COVID-19 pandemic, marked by vaccine nationalism, was a stark litmus test of this systemic failure for low- and middle-income countries (LMICs). This commentary argues

Introduction: In Nigeria, communities have long organised health and social actions that address local needs but remain weakly integrated into the formal primary healthcare (PHC) system. Understanding these approaches and how they interface with PHC is important for advancing universal health coverage (UHC). Objective: This study examined community-driven health and social actions and explored pathways for their integration into the formal PHC system. Method: A qualitative study was conducted in three purposively selected Nigerian states: Anambra, Akwa Ibom, and Kano. Data were collected throu

Mechanical circulatory support (MCS) and heart transplantation are established therapies for advanced heart failure and cardiogenic shock. Technological advances have improved outcomes, including randomized evidence supporting the use of temporary MCS, such as percutaneous ventricular assist devices, in selected patients with ST-elevation myocardial infarction-related cardiogenic shock, alongside substantial gains in survival with contemporary durable left ventricular assist devices (LVADs). Despite this progress, access remains highly inequitable. Although global transplant activity has incre

BACKGROUND: The rise in non-communicable diseases (NCD), such as hypertension and diabetes among people living with human immunodeficiency virus (PLWH), has increased the demand for integrated care due to multiple chronic care needs. However, there is a dearth of evidence on contextual factors implementing integrated hypertension and diabetes care with HIV care. This study aimed to identify facilitators and barriers that could affect the integration of hypertension and diabetes with HIV care at primary health care in Ethiopia.
METHODS: Five primary health facilities from five districts of the

In this paper we examine whether Brazil, Russia and India have similar financing patterns to those observed globally. We assess how national health allocations compare with epidemiological estimates for burden of disease. We identify the major causes of burden of disease in each country, as well as the contribution HIV/AIDS, tuberculosis and malaria make to the total burden of disease estimates. We then use budgetary allocation information to assess the alignment of funding with burden of disease data. We focus on central government allocations through the Ministry of Health or its equivalent.

Living organ donation involves significant out-of-pocket costs, which burden donor candidates and may be an obstacle to donation. There is a single US grant (the National Living Donor Assistance Center-NLDAC) to cover live donor travel costs. Although there may be center-specific variability in grant utilization, prospective donors-and their intended recipients-must also meet eligibility criteria. In fact, the NLDAC grant is used by <10% of US live donors annually. We studied 154 consecutive kidney donor clinic evaluations (November 1, 2014-August 30, 2015) to determine eligibility and usage p

The Global Fund to fight HIV/AIDS, Tuberculosis and Malaria (GFATM) was founded in 2002 in the context of increased political and financial commitments towards health and development, in the aftermath of the Millennium Declaration, and on track to implement the Millennium Development Goals (MDGs). As of today, the institution has mobilized over 16 billion US dollars through its partnership, and spent over 8 billion dollars through 620 contracts in 140 countries for these three diseases. Principles at inception were to accelerate and expand HIV, TB, and Malaria prevention and awareness, care, a

The National Hospital Discharge Survey was used to evaluate the trends in asthma hospitalizations among children under International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM): 1979 to 1987. During this period, asthma hospitalizations among children aged 0 to 17 years increased 4.5% per annum (95% confidence interval [Cl], 2% to 7.1%). The increase was largest among 0 to 4 year olds, 5.0% per annum (95% Cl, 3.4% to 6.7%), vs 2.9% per annum (95% Cl, -0.3% to 6.2%) observed among 5 to 17 year olds. BACKGROUND: A growing body of literature aims to improve understanding of the operations of drug trafficking markets through conducting interviews with dealers and traffickers. Insight into how these individuals conduct business can provide evidence to inform the efforts by policy makers, law enforcement and practitioners to disrupt illicit markets. This paper aims to make a contribution to this evidence base by extending the number of European countries in which interviews have been conducted with incarcerated drug dealers and traffickers.
METHODS: It draws on interviews with 135 men convict

The fate of the Patient Protection and Affordable Care Act of 2010 is uncertain. Much of the opposition is grounded in popular hostility to expansive federal control over individuals' health care decision-making. But the new law reinforces existing third-party payment, primarily through employers and government programs. This financing already restricts personal choice of health plans and coverage options. Private employers, managed care executives, and public officials make the key spending decisions in health care. Unlike consumers in other sectors of the economy, individuals are mostly pass

BACKGROUND: Neglected tropical diseases are widespread, particularly in sub-Saharan Africa, affecting over 2 billion individuals. Control of these diseases has gathered pace in recent years, with increased levels of funding from a number of governmental or non-governmental donors. Focus has currently been on five major 'tool-ready' neglected tropical diseases (lymphatic filariasis, onchocerciasis, schistosomiasis, soil-transmitted helminthiasis and trachoma), using a package of integrated drug delivery according to the World Health Organization guidelines for preventive chemotherapy.

The shortage of cadaveric organs and increased success of living donor transplantation support the use of living organ donors. Clinical social workers have the opportunity to explore a variety of donor-specific issues when performing psychosocial evaluations of living donors, including motivation, psychological stability, and personal and family consequences of donation, as well as the direct and indirect financial consequences faced by living donors. Although most donor-related medical costs are covered, other associated expenses are not reimbursable and may put donors at risk for financial h

This paper addresses key issues concerning the physician workforce and the financing of graduate medical education. The American College of Physicians recommends the establishment of a national advisory organization to develop a coherent and coordinated national policy on the health professions workforce. Given the increasing oversupply of physicians, the College recommends that no new medical schools be created, that total enrollment in U.S. medical schools not increase, and that the number of international medical graduates entering residency training in the United States be restricted. BACKGROUND/OBJECTIVES: Contemporary health systems continue to face persistent challenges in financing, equity, and governance, particularly under conditions of limited resources, workforce shortages, and demographic pressures. While the World Health Organization calls for strengthening social participation to achieve universal health coverage, evidence suggests that such participation often remains largely symbolic. Existing global examples of social participation in health are limited either by their short duration of implementation or by their limited scope. In contrast, the experience of f
