Human finger joints are conventionally simplified as rigid joints in robotic hand design and biomechanical hand modelling, due to their anatomic and morphologic complexity. However, our understanding of the effect of the finger joint configuration on the resulting hand performance is still primitive. In this study, we systematically investigate the grasping performance of the hands with the conventional rigid joints and the biomechanical flexible joints based on a computational human hand model. The measured muscle electromyography (EMG) and hand kinematic data during grasping are used as inpu

Primary synovial chondromatosis is a rare, benign proliferative disease of the joint synovium, tenosynovium, or bursal lining, in which cartilage metaplasia leads to the development of multiple intra-articular and periarticular loose osteocartilaginous bodies. This disease usually involves larger joints (knee, hip, elbow, and shoulder), but it has also rarely been reported in the hand. Patients with this disease complain of pain, swelling, nodules, and decreased range of motion of the affected joint. Due to its nonspecific symptoms and low prevalence, this disease often goes misdiagnosed, lead

BACKGROUND: Joint hypermobility refers to joints that move beyond their normal limits. Individuals with hypermobility of the fingers experience difficulties in activities of daily living. Finger orthoses are available for managing hypermobility of the fingers, but their effectiveness has received little attention in scholarly literature.
OBJECTIVES: To determine if use of custom fit finger orthoses leads to improvements in time needed to perform standardised hand function tests, and attentional demand required to perform these tests, in individuals with joint hypermobility syndrome, Hypermobi

BACKGROUND: General joint hypermobility is estimated to affect about 10% of the population and is a prerequisite of heritable connective tissue disorders where fragile connective tissue is a prominent feature. Pregnancy induced back pain is common whereas about 10% of women still have disabling pain several years after childbirth. The pathogenesis of the pain condition is uncertain, although several risk factors are suggested including general joint hypermobility. In the present study, the possible association of peripheral joint mobility in early pregnancy on the incidence of back pain with o

The presence of clinically palpable finger joint nodules (Heberden's and Bouchard's nodes) was documented in 123 consecutive cases with diffuse idiopathic skeletal hyperostosis (DISH) of the thoracic spine and 191 matched DISH negative controls. The prevalence of palpable finger joint nodules was almost twice as frequent in cases with spinal DISH compared to controls (46% versus 31%, chi 2 = 7.67, P less than 0.01; multivariate adjusted odds ratio OR = 1.84; 95% CI: 1.14-2.98). This increase was most marked at the proximal interphalangeal joint, in males and in patients up to the age of 65 yea

Background/Objectives: Ultrasound is the standard imaging modality to evaluate the inflammatory changes in hand joints of rheumatoid arthritis (RA) patients. However, it is operator-dependent and takes a long time to examine. In this study, we developed a radiograph-based deep learning (DL) model to support prioritization of finger joints for ultrasound (US) examination in RA patients. Methods: In this retrospective study, hand radiographs from RA patients who underwent same-day US examination of bilateral finger joints were analyzed. A DL model was developed using hand radiographs from 270 pa

BACKGROUND: Dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint can cause chronic pain, stiffness, and loss of motion. We present a minimally invasive method-extension block pinning combined with percutaneous intramedullary reduction and fixation-for unstable dorsal PIP fracture-dislocations and evaluate its radiologic and functional outcomes.
METHODS: Twenty consecutive patients with acute unstable dorsal PIP joint fracture-dislocations and persistent post-reduction subluxation were treated with this technique. In the treated cohort, volar articular involvement ranged fr

Objectives: This case series and focused literature review address the diagnostic challenges and management strategies for patients presenting with atraumatic, isolated swelling, and stiffness of a single finger's proximal interphalangeal (PIP) joint. The dual emphasis is on synthesizing the lessons learned from our case series and providing a structured framework for clinical reasoning, including imaging, biopsy, and laboratory assessment. Non-traumatic causes of finger swelling may include rheumatological disease, atypical infection, tumor, or metabolic conditions. Routine evaluation include

CASE: We present a unique case of a 45-year-old man with his right middle finger embedded with rings. Limited finger flexion was noted because of flexor tendon injury caused by the dorsal migration of the embedded ring through joint. The rings were removed under anesthesia, resulting in the resolution of swelling and recover of osseous structure. Follow-up examinations revealed no residual edema or numbness, indicating preserved neurovascularization, despite the dorsal migration of the ring.
CONCLUSION: Our unique case reveals continuous finger ring migration without compromising neurovascula

The purpose of this study was to evaluate the mid- to long-term clinical outcomes of total finger joint arthroplasty using a costal osteochondral autograft for joint ankylosis. Twenty-three joints (three metacarpophalangeal joints, 20 proximal interphalangeal joints) in 23 patients (19 men and four women) were treated with a costal osteochondral autograft and were evaluated after a mean follow-up of 77 months (60-138). Mean age was 33 years (18 to 55). Significant improvement in active finger extension/flexion was seen from a preoperative mean of -24°/26° (arc: 2°) to -13°/75° (arc: 63°) at la

PURPOSE: To identify factors related to postoperative extension loss when treating mallet finger fractures with extension block pinning.
METHODS: We reviewed 31 consecutive patients with a mallet finger fracture treated with extension block pinning. We measured range of motion of the distal interphalangeal (DIP) joint including extension lag. We investigated the injury mechanism and checked radiographic factors such as DIP joint subluxation, fixation angle, fragment size index, fracture angle, and amount of articular involvement. We performed statistical analyses such as correlation analysis,

During the period 1969-1975, 12 disqualified U.S. Army helicopter aviators with various finger amputations were evaluated by the U.S. Army Aeromedical Consultation Service for return to flying duties. The evaluations included both medical examinations and in-flight performance observed by aviation medicine specialists and standardization instructor pilots. The criteria for aeromedical disposition are presented with a discussion of the hand and finger requirements to fly U.S. Army UH-1 rotary wing aircraft. All evaluated aviators were presented to a board of senior U.S. Army flight surgeons, wh

OBJECTIVE: To verify whether magnetic resonance imaging (MRI)-proven joint injury is sensitive as compared with joint injury determined by physical examination.
METHODS: MRI of the wrist and finger joints of both hands was examined in 51 early-stage rheumatoid arthritis (RA) patients by both plain and gadolinium diethylenetriaminepentaacetic acid-enhanced MRI. Synovitis, bone edema, and bone erosion (the latter two included as bone lesions at the wrist joints); metacarpophalangeal joints; and proximal interphalangeal joints were considered as MRI-proven joint injury. INTRODUCTION: The Self-Locking Finger Joint (SLFJ) prosthesis is a cementless surface implant that can be used to treat inflammatory or degenerative osteoarthritis of the proximal interphalangeal (PIP) joint. The aim of this study was to report the mid-term outcomes and to validate a radiological parameter for bone resorption and loosening.
METHODS: Sixteen patients (17 prostheses) who were treated with a SLFJ implant were retrospectively included, with a mean follow-up of 7 years (range 2-14). Outcomes included pain, range of motion of the PIP joint and complications. Implant subsidence and

Simultaneous dislocation of the distal and proximal interphalangeal joints is an uncommon injury. Treating chronic cases proves challenging due to severe contractures affecting both joints and surrounding soft tissues. Here, we report an unusual chronic contracture case in a 46-year-old man who suffered a dorsal dislocation of the distal and proximal interphalangeal joints of the little finger. The patient underwent primary joint distraction followed by open reduction, leading to stable radiographic restoration and a functional range of motion in the affected finger 6 months postoperatively. BACKGROUND: Intra-articular malunions of the finger can lead to deformity and loss of function and can be treated with intra-articular corrective osteotomies. The aim of this study was to evaluate radiographic joint congruency, feasibility and functional outcome of three-dimensional (3D) printed patient-specific instrumentation (PSI) for corrective osteotomies at the trapeziometacarpal and finger joints.
METHODS: Computer-tomography (CT) scans were acquired preoperatively for standard 3D planning, which was followed by calculation of cutting planes and the design of individualized bone surfac

INTRODUCTION: Collateral ligament sprains of the metacarpophalangeal joint (MP) of the long fingers are rare and mostly treated conservatively. Clinical examination to diagnose the severity of these injuries is mandatory. The purpose of our study is to report the results of 15 patients treated surgically.
METHODS: Twenty-three patients, mean age 48, underwent surgery for Stage 3 radial collateral ligament (RCL) injuries of the middle finger (12), the ring finger (4) and the little finger (7). The mean time from trauma to surgery was 53 days. The clinical evaluation consisted of measuring acti

OBJECTIVE: To describe the dorsal 4-finger technique (DFFT) in examining metacarpophalangeal (MCP) joints of patients with rheumatoid arthritis (RA) and compare it to the traditional 2-finger technique (TFT) using ultrasound (US) as a gold standard.
METHODS: Four rheumatologists evaluated 180 MCP joints of 18 patients with RA. All patients underwent US for greyscale (GSUS) and power Doppler US (PDUS). Agreements between rheumatologists, the 2 techniques, and US were evaluated using Cohen κ and the first-order agreement coefficient (AC1) κ methods.
RESULTS: The population comprised 17 females
