Impact of facial fractures and intracranial injuries on hospitalization outcomes following firearm injuries.

IMPORTANCE: Firearm injuries (FAIs) play a major role in unintentional injuries, suicides, and homicides. It is important that policy makers, public health authorities, physicians, and the public are kept abreast of current trends in FAIs so that preventive programs can be tailored to the needs of cohorts that are at highest risk for such injuries. OBJECTIVES: To provide nationally representative longitudinal estimates of outcomes associated with hospitalizations attributed to FAIs in all age groups in the United States during the years 2003 to 2010; to obtain prevalence estimates of skull an

Gunshot wounds: a review of firearm type, range, and location as pertaining to manner of death.

Many studies have examined the characteristics of gunshot wounds by manner of death; however, no published study has directly compared these characteristics for the different types of firearms. This study was designed to address that deficiency. Existing data sets of nonaccidental deaths by handguns, shotguns, and rifles were reviewed. The victim data were analyzed by age and sex of the victims, wound location, range of fire, manner of death, and type of firearm. Handguns were the most common firearm used in both suicides and homicides, followed by rifles and then shotguns. For both homicides

Small-vessel pathology and anastomosis following maxillofacial firearm wounds: an experimental study.

This experiment was designed to provide a biological basis for early reconstruction of maxillofacial defects caused by firearms using free vascularized flap transfer. The pathologic changes of small-vessel injury, injury mechanism, and the feasibility for early anastomosis of these vessels in the maxillofacial region following high-velocity missile wounds were studied. Dogs' faces were wounded by a steel sphere, weighing 0.7 g and 1.03 g, with an initial velocity of 1,300 m/s or 1,500 m/s. Angiography and high-speed x-ray photography were used to record the effect of injury to blood vessels an

Missed Opportunities: Toxicology Screening Remains Incomplete Among Firearm-Injured Adolescents.

BackgroundFirearm injury remains the leading cause of death among children and adolescents in the United States. Firearm-injured adolescents are at increased risk for substance use and behavioral disorders, yet toxicology testing remains inconsistent despite national recommendations. We evaluated toxicology testing among firearm-injured adolescents compared with adults and temporal trends.MethodsWe retrospectively studied firearm-injured patients treated at an urban Level I trauma center from 2013-2024. Adolescents were classified as early (11-14), middle (15-17), or late (18-21) and compared

Firearm-related threats before migrating to the USA from Latin America and the Caribbean.

BACKGROUND: Every year, thousands of people from Latin America and the Caribbean are migrating to the USA. Policy-makers have argued that US firearms are fuelling violence in these countries and are contributing to migration. The objective of this article is to examine the proportion of immigrants from Latin America and the Caribbean arriving at the US border who have previously been threatened with a firearm. This article further explores sociodemographic factors associated with the likelihood of previous firearm-related threats, whether those threats are associated with post-traumatic stress

Community Firearm Violence, Youth Depression, and Suicide Risk in Philadelphia: 2017-2024.

OBJECTIVES: Youth suicide and depression rates increased during the COVID-19 pandemic. In Philadelphia, firearm violence surged and has since declined to pre-pandemic levels. We examined whether youth depression and suicide risk also declined and whether community firearm violence moderated these trajectories, given its documented associations with trauma and stress in youth. METHODS: This cross-sectional study examined 71 663 well visits among youth aged 12 to 18 years in a pediatric primary care network in Philadelphia, 2017 to 2024. Youth were screened for depression and suicidality using

Assessing Links Between Alcohol Exposure and Firearm Violence: A Scoping Review Update.

BACKGROUND: Firearm violence remains a leading cause of death and injury in the United States. Prior research supports that alcohol exposures, including individual-level alcohol use and alcohol control policies, are modifiable risk factors for firearm violence, yet additional research is needed to support prevention efforts. OBJECTIVES: This scoping review aims to update a prior 2016 systematic review on the links between alcohol exposure and firearm violence to examine whether current studies indicate causal links between alcohol use, alcohol interventions, and firearm violence-related outco

Damage control strategy in peripheral vascular injury caused by firearm: A case report.

In peripheral vascular injury, the need for amputation following prolonged ischemia can be avoided through rapid revascularization. However, critically ill patients often cannot undergo single-stage surgery; thus, the use of vascular shunts temporarily alleviates ischemia, protecting the limb until definitive treatment. The present study describes the case of a 28-year-old male patient with a vascular injury in the right leg secondary to a firearm wound. Following admission, he developed grade IV hypovolemic shock. The patient underwent surgery, where a complete transection of the popliteal ar

Nonresponse to an item assessing firearm ownership: Associations with suicide risk and emotional distress.

INTRODUCTION: Firearms account for more than half of suicide deaths in the United States (US) and both ownership and access are associated with increased risk of intentional and unintentional injury. Despite evidence linking ownership and access to suicide risk, individuals may be reticent to answer questionnaire items assessing ownership. The current study examined characteristics of individuals who do not provide a response (nonresponders) to a firearm ownership item in a community sample. METHODS: Data were collected from a cross-sectional online survey of 10,625 US adults. Univariate and

Sickle cell trait in non-firearm arrest-related deaths of Black persons.

The role of sickle cell trait (SCT) in sudden exertional death is well-recognized in sports and military training. However, it is not yet studied for non-firearm arrest-related death (NF-ARD). With extensive multi-pronged searches, a large database (n = 1389) of NF-ARDs was established. For the years 2006-2021 (inclusive) there were 50 NF-ARDs of Black persons in which postmortem evidence of SCT was found. A control cohort consisted of 414 NF-ARDs of Black persons with no reported SCT. The mean age for SCT cases was 33.1 ± 10.4 years versus 37.0 ± 10.4 years for the control group (p = 0.01). T

Relationships between socioeconomic deprivation and pediatric firearm-related injury at the neighborhood level.

BACKGROUND: Disparities in pediatric injury are widely documented and partly driven by differential exposures to social determinants of health (SDH). Here, we examine associations between neighborhood-level SDH and pediatric firearm-related injury admissions as a step to defining specific targets for interventions to prevent injury. METHODS: We conducted a retrospective review of patients 16 years or younger admitted to our Level I pediatric trauma center (2010-2019) after a firearm-related injury. We extracted patients' demographic characteristics and intent of injury. We geocoded home addre

Forensic diagnostics of the range of rifled firearm calculated by back spatter over clothing.

BACKGROUND: Numerous studies have been conducted by using sophisticated methodologies for the deduction of range in cases of firearm injuries. However, the stark dearth of literature for similar purpose by just simple analysis of blood traces on items of clothing is astonishing. PURPOSE: This study was done to determine the relationship between range of a rifled firearm and blood traces on the clothing of victims in firearm injuries. METHODOLOGY: Simulation experiments were performed by firing test shots in closed shooting range by using a Glock 17 pistol (9 mm luger) into pieces of clothing

Biting the Bullet: Firearm Ownership in Persons with Dementia. A Registry-Based Observational Study.

BACKGROUND: In Sweden, 2,296,000 firearms were legally owned by private persons in 2017 and there were 150,000 persons living with a dementia diagnosis. A proportion of these persons owning a firearm may pose safety concerns. OBJECTIVE: The aim was to describe firearm ownership in persons with dementia in Sweden and examine which characteristics are explaining physicians' decision to report a person to the police as unsuitable to possess a firearm. METHODS: This was a registry-based observational study. 65,717 persons with dementia registered in the Swedish Dementia Registry were included in

Patterns and Predictors of Firearm-related Spinal Cord Injuries in Adult Trauma Patients.

INTRODUCTION: Firearm-related spinal cord injuries are commonly missed in the initial assessment as they are often obscured by concomitant injuries and emergent trauma management. These injuries, however, have a significant health and financial impact. The objective of this study was to examine firearm-related spinal cord injuries and identify predictors of presence of such injuries in adult trauma patients. METHODS: This retrospective cohort study examined adult trauma patients (≥16 years) with injuries from firearms included in the 2015 United States National Trauma Data Bank. We performed

Firearm ownership among young adults: Associations with impulsivity and impulse control disorders.

BACKGROUND: Firearm ownership is extremely common in parts of the United States. Yet little research has examined whether it is associated with impulsive symptoms and traits. METHODS: Adults age 18 to 29 who gamble at least occasionally were recruited in 2 cities in the United States using media advertisements for an exploratory study examining general mental health, impulse control disorders, impulsive personality, and aspects of cognition (decision-making, response inhibition, and flexible responding). The participants were grouped as firearm owners and non-firearm owners; these 2 groups we
